1.Decreased Hippocampal Metabolism in Amnestic Mild Cognitive Impairment: Positron Emission Tomography Study

Objective:
Little is known about factors influencing of hippocampal metabolism (HM) in mild cognitive impairment (MCI). Our objective was to determine whether HM in patients with amnestic MCI (aMCI) is decreased than non-amnestic MCI (naMCI).
Methods:
Overall, 32 MCI patients underwent 18F-fluorodeoxyglucose-positron emission tomography. They were characterized as aMCI (n=18) or naMCI (n=14) according to comprehensive neuropsychological criteria. Analysis of variances were used to assess differences on HM between aMCI and naMCI after controlling age, sex, education and Korean version of Mini Mental Status Examination.
Results:
We found that HM was more decreased in aMCI than naMCI. This result was not changed after controlling hippocampal volume.
Conclusion
Our findings suggest that aMCI is associated with decreased HM in MCI.
2.A Case of Esophageal T-cell Lymphoma Presenting as Multiple Discrete Ulcers on a Background of Normal Mucosa.

Gastrointestinal involvement by a lymphoma occurs in 20% of patients with a lymphoma and frequently involves the stomach, small bowel or colon, but rarely involves the esophagus. Esophageal lymphomas are usually secondary to an adjacent lymph node invasion or the contiguous spread from a gastric lymphoma. Primary involvement of the esophagus by a lymphoma is quite rare. Primary versus secondary esophageal involvement by a lymphoma is difficult to determine. The radiological features of esophageal lymphomas may manifest nodules of various sizes, enlarged folds, polypoid mass, ulceration, stenosis, varicoid appearance, and aneurysmal dilatation. The presentation of multiple discrete ulcers in the esophagus on the background of the normal mucosa is most unusual. We describe a case of an esophageal T-cell lymphoma with multiple discrete ulcers on the background of the normal mucosa. The patient was treated with chemotherapy. After chemotherapy, the endoscopic findings of the esophagus showed a normal esophageal mucosa. We report this case with a review of the relevant literature.

3.Experience of Development of Cataract Clinical Practice Guideline in Korea; Focus on Meta-analysis and Questionnaire Survey.

Background: By reviewing experiences and lessons from results of main studies such as systematic review of articles and meta- analysis, and the current situation and trend in the field of cataract and refractive surgery, we proposed practical cases about objective and scientifically evident approaches in process of developing cataract clinical practice guideline in Korea. Methods: Development of cataract clinical practice guideline composed of constructing development team, reviewing guideline- associated articles, conducting meta-analysis and survey about practice behaviors, and having workshop and council. Meta-analysis composed of search and collection of article published within current five years, selection of review principles, development of article assessment form, and investigation of practical variation by qualitative and quantitative meta-analysis. Based on Medline, Meta-analysis was conducted about three topics of IOP (Intra-ocular pressure) changes after inserting each healon, and comparison results after cataract surgery by incision point and occurrence patterns of acquired cataract by types of intraocular lens. Questionnaire survey was conducted about practice behaviors focused on main procedures and surgeries related to cataract. For developing and using clinical practice guideline, we developed a draft which was reviewed by each sub-ophthalmology society, opened symposiums and workshop, and developed final draft. Results: There was no significant difference between IOP (Intra-ocular pressure) changes after inserting each healon. After reviewing article about results after cataract surgery by incision point, we found that we couldn't conduct quantitative meta-analysis because surgeries method, incision lengths, measuring methods, and points at measuring showed different pattern. Occurrences of cataract in 24 months after cataract surgery between 'PMMA', 'Silicone', and 'Acrylic' group showed significant difference. Result of questionnaire survey showed that there was many variations on cataract clinical practice behaviors. Conclusions: Although we were lacking in articles, we could propose partial objective evidences for development of cataract clinical practice guideline. And we found that systematic researches and surveys should be conducted for more objective evidences.

4.An Analysis of Antituberculosis Drug Susceptibility Test Results in Kyung Hee Medical Center During Recent Four years.

BACKGROUND: Tuberculosis is still one of the most seriously threatening infections in Korea, because of multidrug resistant tuberculosis. Results of antituberculosis drug susceptibility test can provide clinicians very important informations for selection of proper regimens for treatment. METHODS: In this study the results of antituberculosis drug susceptibility test of 298 cases at Kyunghee Medical Center from 2000 to 2003 were retrospectively analysed to evaluate the trend of antituberculosis drug susceptibility. The procedure of drug susceptibility test was based on the absolute concentration method using Lowenstein-Jensen solid media. RESULTS: The resistance rate of Mycobacterium tuberculosis to one or more drugs was increased from 29.3% in 2000 to 48.2% in 2003, and the rates of multiple resistance to two or more drugs increased from 13.3% in 2000 to 20.5% in 2003. The increase in resistance rate to individual drug during study period were 20.0% to 24.1% in isoniazid, 9.3% to 19.3% in rifampicin, 5.3% to 15.7% in ethambutol, 4.0% to 10.8% in para-aminosalicylic acid, 2.7% to 6.0% in kanamycin, 1.3% to 7.2% in ethionamide, 1.3% to 6.0% in capreomycin, 1.3% to 7.2% in prothionamide, 0.0% to 12.1% in ofloxacin, 6.7%to 3.6% in streptomycin, 6.7% to 7.2% in cycloserine, 10.7% to 8.4% in pyrazinamide, respectively. CONCLUSIONS: The resistance rate of M. tuberculosis has been increased with years and multidrug resistant M. tuberculosis was commonly encountered in the specimens from the patients visited Kyunghee Medical center.

5.A Study on the Development of M.D.-Ph.D. Program.

This study is to develop M.D.-Ph.D. program of medical school in Korea. The authors examined the related literature focusing on the current status and problems of training biomedical scientists. Next step was to make a collection of data related to American M.D.-Ph.D. program. The search for the materials was made through February 2000. The developed M.D.-Ph.D. program was modified and revised by the experts in the area and through public speech. The results of the study are as follows; first, the validity of M.D.-Ph.D. program was convicted by the need of interdisciplinary study, social needs, current problems of medical education and financial problems. Second, the development M.D.-Ph.D. program consists of educational period, admission procedure, entree examination, curriculum and its administration, degrees, school registration management, medical license examination, grants and problems related to military service. The expectations of the results are; first, M.D.-Ph.D. program will make Korea one of the leading countries in biomedical scientists and medical knowledge, and will activate the interdisciplinary research. Second, M.D.-Ph.D. program will facilitate to revise the educational problems of graduate school of medicine and medical schools.

6.The Study on The Nature of Fibrous Materials Extracted from Follicular Skin Lesions.

BACKGROUND: Comedo extractor is in general used for the management or diagnosis of the follicular cutaneous disorders such as acne and folliculitis. During observing such extracted materials, we happened to discover the extraordinary thread-like fibrous materials. OBJECTIVE: We tried to clarify the nature of these fibrous materials. METHODS: The extracted materials from 20 patients with follicular cutanous disorders including 10 acne vulgaris and 10 pityrosporum folliculitis and 10 normal persons were investigated by pKOH, Masson trichrome for collagen fiber, Gomori's reticulum for reticulum fiber, acid orcein for elastic fiber, periodic acid-Schiff for fungal element and alcian blue for mucin. And we performed above mentioned special stainings in two biopsy specimens. RESULTS: Most of the extracted fibrous materials were reticulum fibers(95%) and some of them were elastic fibers(35%). And we observed the reticulum fibers and elastic fibers taken off from the pilosebaceous follicle on the histopathologic study. CONCLUSION: The fibrous materials extracted from follicular lesions were reticulum fibers or elastic fibers. And the observed presence of elastic fibers and reticulum fibers might result from tissue necrosis produced by leukocytes during the inflammatory phase of pilosebaceous follicles.

7.Stability in the Bag of Foldable Silicone Intraocular Lenses.

To examine the stability in the bag in foldable silicone IOL implantation and determine its effect on visual acuity. This study comprised 66 eyes that had phacoemulsification with three-piece foldable silicone intraocular lens(IOL) implantation that were prospectively evaluated about 2 months after surgery. The examination included the status of the haptic and optic placement, extent of posterior capsular capsular opacification(PCO), and the best corrected visual acuity. Sixty-five eyes had an in-in haptic placement, one eye had in-out placement, and there was no eye in out-out placement. Capsular capture occurred in 13 eyes(19.70%). The extent of POC in eyes with capsular capture was greater than in those withot capture. But the best corrected visual acuity in eyes with capsular capture made no difference to those without capture. In addition, eyes with in-out and out-out haptic placement had worse PCO than the eyes with in-in placement. There was a high incidence of capsular capture after silicone IOL implantation. This complication, along with the undesirable placement of the haptics, led tod PCO and was anticipated visual inpairment in long term follow-up. Therefore proper amount of continuous curvilinear capsulorhesix opening and complete placement of lens in the bag ahould be necessary.

8.Cormeal Endothelial Cell Loss in Phacoemulsifications of Sup. & Temporal Scleral Pocket Incisions.

Recently, phacoemulsification is a widely used technique gor extraction of opaque lens. Phacoemulsification thorugh a small incision prvides early visual rehabilitation. However, the side effects of phacoemulsification such as endothelial cell loss have been reported continuously. The aim of this study was to analyze the influence of incision site on corneal endotheliu. Central corneal endothlial cell loss(ECL) was compared following insertion of posterior chamber PMMA intraocular lens with 5.5mm superior scleral pocket incision in 30 eyes and 5.5mm temporal scleral poket incision in 31 eyes. Cell density of the corneal endothelium in all eyes was examed by auto-focus Non Contact Specular Microscopy preoperatively and 2 months postoperatively. The average cell loss was 7.41+/-6.9% and 6.61+/-7.1%, respectively. Phacoemulsification with temporal 5.5mm scleral pocket incisions produced slightly less endothelial cell loss(ECL) than superior incisions. But, there was no statistically significant difference between the eyes with temporal 5.5mm scleral pocket incisions and those with superior 5.5mm scleral pocket incisions. The endothelial cell loss(ECL) is not associated with the direction of corneal incision. If the appropriate direction of corneal incision is chosen by pre-poerative corneal astigmatism, it seems that we will find the better results.

9.Apoptosis and Expression of bcl-2, p53, and Ki-67 in Mycosis Fungoides.

BACKGROUND: Mycosis fungoides(MF) is a form of cutaneous T cell lymphoma with clonal differentiation of helpr' T cell. It has a patch, plaque, and tumor stage. But pathogenetic factors controlling the development and progression of MF are still unclear. Apoptosis plays a major role in developmental biology and homeostasis. The bcl-2 oncogene prolongs ce11 life by inhibiting apoptosis. The mutant pS3 gene induces apoptosis indirectly. Ki-67 antigen is the cell proliferation marker. Recently, it has been shown that the relationships among them are important in the tumorigenesis of the various tumors. OBJECTIVE: The aim of this study was to examine the expression of these genes and apoptotic rate and clarify the relationship among them in the development and progression of MF. METHODS: The eighteen specimens from 8 patients with MF and 10 specimens from benign lymphocytic infiltrating diseases including 5 lichen planus, 3 lupus erythematosus, and 2 contact dermatitis were included. We performed immunoperoxidase staining(LSAB technique) using monoclonal antibodies including bc1-2, p~53, and Ki-67(MIB1). We used ApoptaqTM(Oncor) in situ labelling kit for detecting apoptotic cell.

10.Bacteriologic Study of Chronic Suppurative Otitis Media.

BACKGROUND: Chronic suppurative otitis media is one of the most common disease in the otolaryngologic field. It is important to choose of antibiotics in the management of chronic suppurative otitis media. OBJECTIVES: Bacteriologic studies can make it possible to use appropriate antibiotics. MATERIALS AND METHODS: The bacteriologic study was made on 98 cases of chronic suppurative otitis media who visited the department of otolaryngology, Dae Dong Hospital in Pusan from Nov. 1989 to Dec. 1995 and the following results were obtained. RESULTS: 1) In 90 cases in which pathogenic organism was isolated, single infection was 81 cases(90%) and mixed infection was 9 cases(10%). 2) The most frequent pathogenic organism was Staphylococcus(46.5%) and Proteus(16.1%), Pseudomonas(14.1%), Providencia(10.1%) were the next. 3) In the aspect of the sensitivity to antibiotics, Ciprofloxacin, Ceftriaxon and Amikacin were sensitive drug generally. 4) Staphylococcus was sensitive to Vancomycin and Ciprofloxacin. 5) Proteus was sensitive to Amikacin, Ciprofloxacin, and Ceftriaxon. 6) Pseudomonas was sensitive to Ciprofloxacin, Amikacin and Piperacin. 7) Providencia was sensitive to Ciprofloxacin, Ceftriaxon, Amikacin and Piperacin. 8) Methicillin Resistant Staphylococcus Aureus(MRSA) was 19 Strains(45.5%). CONCLUSION: Recently, pathologic strain and it's sensitivity to antibiotics has changed. So, we recommand that periodic bacteriologic study and sensitivity test should be done for effective management of chronic otitis media.
