1.A Case of Clear Cell Hidradenoma.

Clear cell hidradenoma is a much less common primary cutaneous tumor originated from sweat gland. Clinically this tumor occurs more frequently in female than in male and in patients of any age although there is a predilection for the middle and older age groups. The lesion is solitary and represents a cutaneous nodular and cystic mass of slow expansible growth. By light microscopy the lesion has been characterized as a solid cataneous tumor composed of clear epithelial cells which are focally arranged in glandular and cystic patterns. We observed a 37-year-old housewife who had suffered from a tumor on her back for 3 years. The tumor was a pedunculated, soft, fixed mass and 3. 5 * 3 * 1. 5 cm in size. Histopathologic finding showed the tumor to be composed of two main types of cells, fusiform and clear cells.

The present study evaluates the type, frequency and location of basal cell epithelioma, as well as the age and sex distribution of patients and the recurrence rate of basal cell epithelioma. In this study 116 patients, who visited Severance hospital during the last 15 years from Jan. 1, 1967 to Dec. 31, 1981 and were diagnosed histopathologically as having basal cell epithelioma, were analysed. 4.Cutaneous Manifestations of Carbon Monoxide Poisoning.

Carbon monoxide is colorless, oolorless, tasteless and non-irritating gas produced by the incomplete combustion of carbonaceous rnaterial. It combines with hemoglobin and displaces oxygen because the affinity of hemoglobin for carbon monoxide is two hundred times greater than oxygen. Symptoma and signs of carbon monoxide poisoning include headache, nausea, vomiting, dizziness, collapse, unconsciousness, blindness, convulsion, coma and skin lesions. Recently we have observed seven patients with carbon monoxide poisoning who expressed cutaneous syrnptoms. In this work we investigated the pathogenesis of cutaneous manifestations of carbon monoxide poisoning through clinical, histologic and electronmicroscopic study. The results are summarized as follows: 1. Mental states of the patients were comatose in two, Semicomatcse in two, stuporous in two, and drowsy in one patient. In routine laboratory tests, we observed elevated blood sugar in six, elevated sorum creatinine phosphokinase in four and abnormal findings in urinalysis in all patients. 2. Cutaneous lesions were vesicobullae, plaque or swelling, erythema, gangrene and 'ulceration in order of frequency and located in the dependent areas in six caies. 3. Histopathologically, the sites of the bullae were subepidermal in four cases and intraepidermal in. one case and there was one case with ulceration. 4. 5.Surface Image of Normal Intervertebral Disc on 3 Dimensional CT.

PURPOSE: To evaluate surface configuration of intervertebral disc on three dimensional CT. MATERIALS AND METHODS: Three dimensional surface images reconstructed from CTscans(1 mm thick) of 20discs in 14 healthy adults were reviewed. Disc surface was classified into peripheral and central zones in contact with consecutive peripheral ring and central endplate. Local irregularity incidence, pattern in radial, concentric, or mixed form, size, location, and extension were observed. Incidence and severity ranges in 4 grades of general irregularity, and peripheral width relative to central radius were evaluated. RESULTS: Normal disc mostly showed smooth surface with few display of small local irregularity(6/20) which was mainly radial in pattern(4/5), posterolaterally located(4/6), and confined within peripheral zone(5/6). General irregularity displayed(5/20) was all grade I and peripheral width was 0.82 of central radius. CONCLUSION: Normal disc shows smooth surface but few may display small local irregularity maybe due to very early degenerative change.

6.A Case of Reticulate Acropigmentation of Kitamura.

Reticulate acropigmentation of Kitamura, described first by Kitamura and Akam atsu in 1943, is a pigmentary disorder that may be determined by an autasomal dominant gene. Skin lesions are characterized clinically by depressed brownisL pigmented spots, in a reticulated pattern, on the extensor surface of the acral parts of the extremities. Most reported cases are confined to Japanese. We report herein a case of reticulate acropigmentation of Kitarnura in a 28- year-old woman, with a depressed brownish pigmented spots, in a reticulate pattern, on the dorsum of hands and left foot. The authors make the diagnosis of this case a reticulate acropigmentation of Kitamura for the clinical symptoms and histopathologic findings.

7.The Clinical Study of Scabietic Patients.

We had observed 60 patients with scabies in Kangwon province around Wonju area from June to Aug. 1981. The purpose of our present study was the evaluation of the clinical manifestations and laboratory findings in patients with scabies. The results were summarized as follow; 1) The sex ratio of male to female patients was 1. 9: 1 and the most frequent affected age was 10-29 year-old group. 2) Family history was positive in 72.4% of all patients. 3) Ninety per cents of the patients had a history of previous treatment for their kin lesiona, but only 30. 0% of the patients were treated under the diagnosis of scabies. 4) We found the itch mite and it's eggs in 16. 7% of patients and the most frequent detection site was the burrow of interdigital webs, 5) Secondary infection and dermatitis were the most frequent associated complications. 6) The severity of thedisease was correlated with the rate of peripheral eosinophil count. 7) Oniy 4(22.2%) of 18 patients were sensitized to DNCB. 8) The result of VDRL test of the 40 patients whose age were over 17 years old was non-reactive in all.

8.A Case of Penile Horn.

Penile Horn occurs as a single or multiple lesions. It usually starts as warty growth that later becomes hyperkeratotic and assumes the appearance of a horn. A 22-year-old male patient was presented with a slow growing, asymptomatic, dark brownish colored, corn-shaped, protruding mass on prepuce of the penis for 5 months. Histopathologic finding showed verruca vulgaris. We think that penile horn in this case may be come from verruca vulgaris.

10.Chemotherapy of Leprosy with Newly Synthesized Thiocarbanilides.

Attention has been paid to the thiocarbanilide derivative and the authors synthesized some new compounds of thiocarbanilides for experimental studies on tuberculosis and leprosy. The series of newly synthesized chemical compounds of thiocarbanilides were studied for comparision with the already known antimycobacterial agents; INH, PAS, Streptomycin and D. D. S. The strains of Mycobacterium tubercubsis (H37 Rv, Ravenel, and B. C. G.) and Mycobacterium phlei were used for the in vitro experiments. In the in vivo experiments, the strain of Mycobacterium tuberculosis var. bovis (Ravenel) was employed. The strain of Mycobacterium leprae murium (Hawaiian strain) was used for the murine leprosy experiments. The experimental animals for the in vivo tests were white mice (CFW strain) and these were extensively employed in tuberculosis and leprosy as well. Sixteen cases of Various types of human leprosy, were treated with one of the newly synthesized thiocarbanilides (L-4). Among the newly synthesized chemical compounds of thiocarbanilides studied for their antituberculous and antimurine leprosy activity in vitro and in vivo experiments, two compounds were shown to be suppressive agents for those infections without significant toxicity. These two compounds were named tentatively as L-1 and L-4. 1) LD50 of L-1 was 1,054 mg/kg and that of L-4 was 1,028 mg/kg, while the LD50 of INH was 650 mg/kg and PAS was 4,000mg/kg orally in the experimental animals. 2) L-1 and L-4 showed remarkable suppressive activity in vitro using solid media with 100r/ml. concentration. These data were parallel to 1r/ml. of INH and 50r/ml. of PAS. The inferiority of L-1 and L-4 to INH and PAS in vitro studies might have been due to the water insolubility of these compounds while INH and PAS were readily soluble in water. 3) In vivo experiments with L-1 showed a much-more superior antituberculous effect than was found with INH and PAS. 4) A method of grading the bacterial count in a homogenized tissue suspension of visceral organs (lungs, liver, spleen and kidneys) using the simple technique of the Gaffky scale was accurate and time saving technique in screening the results of the chemotherapeutic agents in tuberculosis. 5) Among the newly synthesized compounds L-4 showed the most remarkable suppressive effect on murine leprosy. The suppressive results were similar to those of INH. 6) The method of measuring the size and the weight of leproma at the inoculated site was simple and is an adequate screening test for chemotherapeutic effect in murine leprosy. 7) In the trials with human leprosy 16 cases of various types, using L-4, the effectiveness in clinical as well as in bacteriological improvement was remarkable. a) After L-4 treatment decrease in bactriologica1 indices and remarkable clinical improvement after a relative1y short period of treatment were observed. b) L-4, up to the maximum daily dose of 500 mg, can be safely administered orally to the patients without any significant side reactions. c) L-4 could be used with remarkable clinical improvement for the patients in lepra reactions.
