3.The treatment of congenital cutis aplasia.

One case of congenital cutis aplasia is presented. The defect involved includes full-thickness skin defect of scalp and cranium. The patient was treated with debridement of dirty necrosed crust which covered exposed dura mater and with double opposing rotation flap including pericranium for bone regeneration. The donor site was covered with skin graft from right thigh. During operation, the superficial temporal artery was found to be short and weak. And after operation, the margin of flap were congested and finally necrotized. The necrotic wound was treated with conservative management. The vascular impairment is thought to be main course of congenital cutis aplasia. So we conclude that the treatment of choice is conservative management or careful flap surgery for coverage of defect area.

4.Pharyngoesophageal reconstruction.

Microvascular tissue transfers have facilitated primary closure of various complex defects after radical ablation of head and neck cancers. From Oct 1991 to Feb 1992, we used forearm free flap in two patients and delto-pectoral flap in one patient who had preoperative irradiation for pharyngoesophageal reconstruction. The stricture and fistula formation were most troublesome complication in forearm free flap, so we designed as lazy S shape in distal flap margin to prevent circular contraction and longitudinal margin was deepithelialized (5 mm) and sutured double layer to withstand fistular formation and this can be considered useful in place of a free jejuna transfer.

6.Poststerptococcal Glomerulonephritis Mimicking Henoch-Schonlein Purpura.

Among total 108 cases of biopsy-proven fibroadenomas of the breast, which obtained from the files of the Asan Medical Center during one year period from October 1998 to September 1999, 23 cases cytologically diagnosed as high risk group were reviewed to retrieve the mis-leading factors. Initial cytologic diagnoses of 23 cases were proliferative breast lesion with atypia(high risk) in 21 cases(91.3%) and papillary neoplasm in 2 cases(8.7%). When we reanalysed 23 cases by Masood scoring system, they were classified as one non-proliferative breast lesion(4.3%), 16 proliferative breast lesions without atypia (69.6%), and 6 proliferative breast lesions with atypia(26.1%). None were subject to the category of carcinoma. Cytologic features leading to the overdiagnosis of high grade epithelial lesions were as follows; cellular dissociation without nuclear atypia, nuclear pleomorphism, anisonucleosis, and occasional macronucleoli without nuclear enlargement, lack of myxoid stroma, and few naked stromal cells. To avoid cytologic overdiagnosis of fibroadenoma, mild to moderate nuclear pleomorphism without nuclear enlargement, and cellular dissociation without nuclear atypia should not be regarded as criteria of high risk group.

8.Needle Localization Biopsy of Nonpalpable lesions of Breast under the Local Anesthesia.

In performing needle localization breast biopsies(NLBB) under local anesthesia in an outpatient setting, we investigated, whether complication rates would be acceptable when compared with complications of the breast performed for palpable masses. We were also interested in determining whether our rate of missed biopsies would be acceptable or not. We performed the NLBB on 55 patients with a nonpalpable breast lesion, and open biopsy on 98 patients with a palpable breast mass under local anesthesia from 1992 to 1995. The missed biopsy rate was one (1.8%) of 55. Complication of NLBB occured in 4 cases(7.3%), including 1 case of seroma, 2 cases of hematoma, and 1 case of abscess. The complication rate was not statistically different from NLBB(7.3%) and open biopsy(6.1%). (P=0.25) Eleven carcinomas, including 8 cases of invasive and 3 cases of DCIS, were found with NLBB procedure.. But there was no in situ carcinoma among the palpable lesions. Needle localization breast biopsies can be performed under local anesthesia and complications rates were similar to those associated with biopsies of palpable lesions. But missed biopsy could occur if the localization was not performed exactly.

9.A Case of Overlapping Syndrome of Primary Membranous Nephropathy and IgA Nephropathy.

We report a rare case of primary glomerular disease with both features of IgA nephropathy and membranous glomerulonephritis in a 27 year-old woman with nephrotic syndrome. Histologically, glomeruli showed slight mesangial expansion, proliferation of mesangial cells, and short subepithelial spikes on capillary wall. Direct immunofluorescence demonstrated granular IgG and C3 deposits along the capillary walls, and IgA with C3 deposits in mesangium. Granular subepithelial and rnesangial deposits were observed by electron microscopy. The patient showed complete remission after 6 months therapy with steroid and ACE inhibitor.

10.The Protective Effect of Marly SkinR on Housewife' s Eczema.

BACKGROUND: It is important to avoid the direct contact with water of detergents as possible for the treatment and prevention of hand eczema. OBJECTIVE: The purpose of this study is to evaluate the protective effect of skin protector, Marly skin on housewifes eczema, METHODS: The patients with hand eczema apply skin protector, Maly skin, four times a day on their both hands. They are evaluated with the scores of subjective symptor is(itch, dryness, smarting) and objective signs(erythema, scaling, fissures), and skin replica which are taken from the specified site are analysed by using an image analyser and, scanning electron micrascipe. RESULTS: 4 weeks after application of skin protector, there are statstically significant decrease of total clinical symptom scores. Scanning electron microscopic findings of the skin replicas show marked changes between the beginning and the end of the study. On quantive image analysis of the skin replicas, the length of fissure is a little decreased and the depth of rrows is slightly increased than before application of skin protector, but these are not statistically significant. CONCLUSION: Skin protector, Marly skin, has a protective effect on the development of hand eczema.
