2.Report on a case treated with lingual multibracket appliance.

The lingual multibracket appliance was developed to overcome the unesthetic nature of traditional fixed orthodontic labial appliances; however, treatment with this appliance was regarded as very difficult as well as a time-consuming. Recently, these problems have been reduced because of advances in lingual bracket systems, improved indirect bonding technique and the development of more flexible wires. There has been a marked increase in the number of adults desiring orthodontic treatment over recent years and many of these patients tend to seek invisible orthodontic appliances due to esthetic reasons. Although the lingual multibracket treatment is not ideal, this form of treatment is currently the best option for the patients with esthetic concerns. Here, I would like to present a case treated successfully with lingual multibracket appliance.

3.Transcutaneous reduction and external bone fixation device for the treatment of zygomatic arch fracture.

Zygomatic arch is a prominent structure among facial bone and this is the major cause of vulnerability from the facial trauma. The fracture of zygomatic arch has been treated by ancillary methods of closed reduction approaching through temporal incision or intraoral incision. But, these methods gas a limitation of reduction force vector and it has a difficulty in maintaining the secure fixation of the reduced unstable fractured bones. All 22 cases of zygomatic fracture were reduced by transcutaneous screw and fixed with external fixation device successfully. There was no post-operative complication, such as non union, malunion, and visible scar. This method also has advantages of less post-operative edema and pain.

4.Surgical approach of orbital medial wall fractures.

Medial orbital wall fracture was described by Converse and Smith in the first time in 1957. These fractures which comprise more than 30% of all the orbital wall fracture are may easily be overlooked in routine orbital radiology. Accordingly, examination with orbital CT is essential for accurate diagnosis and appropriate treatment. These type of fracture are followed frequently by post-oprative complication such as diplopia and enophthalmos because it is very difficult to approach to orbital medial was in these operations. The mechanism of the orbital wall fractures are abruptly increased hydraulic pressure by sudden impact on orbital rim or soft tissue of orbital region. So patients who show the signs of subcutaneous emphysema, edema on the orbital region or diplopia need to get through examination with orbital CT for accurate diagnosis and immediate reconstruction of the fractured orbital wall. Conventional approaches in the operations of the orbital medial wall fractures are that with bicoronal incision, subciliary incision, transconjunctival incision and infra-orbital rim incision. But, approach with bicoronary incision, needs broader dissection and longer time for operation. Operations with transconjunctival incision and that with subciliary incision have difficulty to approach to fractured sites. Operations with other conventional methods may be also followed by scar problem. The authors performed reduction and reconstruction of the fractured orbital medial wall fractures successfully without any complications and difficulties by intra-eyebrow approach with which they made incision of 2 - 2.5 cm on mid-area of eyebrow to expose fractured medial wall with good operative field.

5.Computed tomographic findings in carcinoma of cervix

Accurate staging as well as diagnosis of cervix carcinoma is of paramount importance in planning treatment. Cervical smear cytologic studies have increased the detection rate of cervix carcinoma, but current radiologictechniques are limited in staging cervix carcinoma and precise visualization of lymphnodal metastasis. The CT scancan display the precise transaxial anatomical structures, permitting us not only to know the tumor size, shape andits extent but also lymphnodal enlargement in the pelvic cavity and around the abdominal aorta and secondarychanges of the kidney. The authors have tested the usefulness of CT in staging cervix carcinoma and detectinglymphatic involvement by analysis of 24 cases diagnosed by CT and confirmed histologically at St. Mary's hospitalduring the period from Oct. 1982 to May 1984. The conclusions are as follows; 1. The accuracy of CT instaging ofcervix carcinoma was 79%. 2. The accuracy of CT diagnosis of lymphatic involvement was 87.5%, sesitivity was85.7%, and speicificity was 88.2%.

6.The growth of rat mandibles following mandibular retractive force.

The purpose of this study was to investigate changes of the mandible of a growing rat when they are subjected to a retractive force and those after removal of the retractive force. The experimental animals were Sprague-Dawley male rats of four weeks of age. A mandible was retracted with 50 grams of force on each side in the posterior and superior direction for 8 hours per day. The animals were sacrificed after 1 week, 2-week and 4-week force application, and after 4-week force application-4-week force removal period. The changes of rat mandibular growth following retractive force on the growing rat mandible were observed histologically and biometrically. The findings were as follows; 1. Histologically, the thickness of the condylar cartilage was slightly reduced in the anterosuperior region with the retractive force. However, in the group of 4-week force application-4-week force removal, there was no significant difference in the thickness of the condylar cartilage. 2. There were no significant histological changes in the articular disk and glenoid fossa through the experimental period. 3. The length and anterior height of the mandible subjected to the retractive force were significantly smaller and greater than those of the control group. 4. There were no significant differences in the mandibular length between 4-week force application-4-week force removal and the control group. 5. It was concluded that a mandibular retractive force produced inhibitory effects in the growth of the mandible, but that these effects were not sustained during mandibular growth in this experimental model.

8.Endoscopic Diagnosis of Primary Duodenal Cancers.

Primary cancer af the duodenum is rare. An extensive experience with fiberoptic eaophag-ogastro-duodenoscopy in our institution has indicated that duodenal cancer may be more frequent thsn suspeeted and readily diagnesed by this modality, With this in mied, clinical, pathological, diagnostic and therapeetie aspects of the 26 patients with primary duodenal cancer which had been endoscopically diugnosed were reviewed. The results obtained were as follows: 1) Ages ranged from 16 to 75, with the mean of 55.2 years. Seventeen patients were men and nine patients women being l. 9:1 of male-female ratio. 2) Hndoscoyic findings were 46. 1 percent ulcerative anld 34. 8 per cent ulcerative-infiltrative, 3) The diatribution within the duodenum was 50 per cent suprapapillary, 42. 4 per cent peri- papillary aecl 7. 6 per cent infrapapillary. 4) The twenty-three patients had adenocarcinoma, two patients leiomyosarcoma and 1 patient lymphoma. 5) Twenty patients were confirmed by duodenoseopic biipsies and six patients by operations. 6) The mast common presenting mmyliiints were abdominal pain, chronic blood lais leading anemia, jaundice, anorexia, indigestion, weight lass and palpable abdominal mass. 7) Of the 26 patients, eight patients underwent attempted curative resection, six patients underwent palliative bypasa procedures or exploratory laparotomy.

9.Endoscopic Retrograde Cholangiopancreatography in the Evaluation of Postcholecystectomy Patients.

Postcholecystectomy syndrome is the persistence or recurrence of symytom complex following cholecystectomy, The majority of the patients have mild symptoms. However, the cause of recurrent symptoms is often obscure and as a consequence, a plan of management is difficult to formulate. Recently ERCP has proved to be increasingly helpful in the investigation of postcholecystectomy patients. The purpose of this study was to test the effectiveness of the ERCP in the evaluation of patients with postcbiolecystectomy syndrorae. The results are as follows 1) 102 postcholecystectomy patients were studied by endoscopic retrograde cholangiopancreatography and successful cannulation with demonstration of at least one duct was achieved in 49 of 50 jaundiced patients and in 49 of 52 non-jaundiced patients. The overall success rate was 96. 1%. 2) The results of ERCP were normal in 26 patients(26.5%) and abnormal in 72 patients (73.5%), Only 3 of 50 jaundiced patients were normal, but 23 of 52 non-jaundiced patients showed no abnormal findings. 3) The most common abnormality was biliary stone in common bile duct and intrahepatic duct which were present in 51 patients(50%). Cholangitis without stone was next common finding which was in 13 patients(13%). Of the remaining patients have air biligram, 2 CBD stricture, 1 CBD aseariasis & 1 chronic pancreatitis. 4) Time lapse between onset of symptoms and cholecystectomy was variable. 31 patients were studied less than 2 years after cholecystectomy. 18 of these patients had jaundice and 13 had no jaundice. Within 2 years afte chklecystectomy, the biliary stone was most common finding which were present in 14 out of 31 patients.

10.Improvement of Pregnancy Rate by the Selection of Early Cleavage Embryos to 2-cell Stage in Human IVF.

OBJECTIVE: Evaluation of embryos using early cleavage to 2-cell stage has been proposed, but a critical time-point for selecting embryos is unclear. The aim of the present study is to provide a guideline including critical time-point in the selection of early cleaving embryo for the reduction of multiple pregnancies as well as the increase of pregnancy rate in human IVF. METHODS: This prospective study was performed in 116 cycles from 85 patients who underwent conventional IVF or ICSI at the infertility clinic of Good Moonhwa Hospital from January 2002 to December 2003. Early cleavage (EC) of embryos to 2-cell stage was assessed at 25 h and 27 h postinsemination/microinjection. Embryos that had early cleaved at each time point were designated as EC-1 and EC-2, respectively, while others were designated as non-early cleavage (NEC). RESULTS: At least one early cleavage embryo was observed in 54 (46.6%) for the EC-1 and 84 (72.4%) for the EC-2 of the 116 cycles assessed. Clinical pregnancy rates (PR) were significantly higher in the EC-1 group (66.7%) compared to the EC-2 group (53.6%) or the NEC group (31.2%) (p<0.05). Significant improvement of the pregnancy rate was found when at least two or more embryos were early cleaved at 25 h postinsemination or when the proportion of early cleavage embryo at 25 h postinsemination was higher than 20% (p<0.05). CONCLUSIONS: The critical time-point for the selection of early cleavage embryos with high implantation potential is more effective in 25 h postinsemination/microinjection compared to 27 h. The proportion as well as number of early cleavage embryos is also an important factor for the prediction of pregnancy outcome and the chance of multiple pregnancies. These results demonstrated that the evaluation of early cleavage embryos to 2-cell stage is an easy, simple, and objective method for the selection of good quality embryos suitable for embryo transfer.
