1.Photochemical Treatment Utilizing Mercury Out-door Illumination Lamp.

In previous paper we reported on the radiation spectrum of commercially ava ilable out-door illumination lamp and suggested the use of this lamp as long ultraviolet light source until such time we have adquate medical equipments availahle. We tried to dectermine the erythema dose of above lamps in conjunction with topical psoralen and tar. The skin lamp distance was kept 50cm. The test sites were the backs of 30 young volunters. 200 watt bulbs were used. l. 30 minutes topical application of 0.25% Meladoxen solution required 2 to 6 minutes of exposure to inducs erythema in 48 hours. 2. 6 hours of topical tar application needed 2 to 8 minutes of exposure. 3. With 0.25% Meladoxen solution; 30 minutes of topical application produced more phototoxic effect tnan 2 or 24 hours. 4. With 5% Tar ointment; the longer the duration of topical application the stronger was the phototoxic effect. But topical application time exceeding 6 hours failed to increase phototoxic effect furtber. 5. With 0.3% Meladoxen cream the thicker the applied cream the stronger was the phototoxic effect up to 25mg per cm, over which phototoxic effect decreased, probably due to the screening effect of the cream, It is concluded that in the photochemical treatment of vitiligo and psoriasis psoralen sould be applied for 80 minutes, the tar for 6 hours or ovemight before irradiating the lesions with long wave ultraviolet rays. In this way the time required for each session of photochemical treatment of psoriasis or vitiligo will be greatly reduced while obtaining maximum effect. This is more significant in the view of long term treatmant required for the management of these embarrasing dermatoses.

3.Familial Absence of the Patella: A Report of three Members is a Family

Bilateral absence of the patella as an isolated congenital abnormality is an extremly rare condition. Three members in a family were found to be afflicted with the condition. Clinical examination failed to reveal any stigmata of hereditary osteo-onychodysplasia.

4.A Case of Atraumatic Aseptic Necrosis of Both Talus: A Case Report

We report one case of aseptic necrosis of both talus which has developed without any evidence of previous trauma. Aseptic necrosis of the talus is known to occur after any trauma which result in iterruption the blood supply to talus. Atraumatic aseptic necrosis of talus is extremely rarre condition. Some cases of atraumatic aseptic necrosis of talus were repoted by some authers, in whom Garcia described the first case in 1928. Most of the reported atraumatic aseptic necrosis were due to long term steroid therapy for unrelated medical problems and others were related to Alcoholism, Sepsis and Tuber culosis. ln this case we think for it may be caused or contributed by steroid therapy.

5.Erythrocyte 2,3-DPG and ATP Concentration in Intraoperative Salvaged Blood and CPD Stored Blood.

The purpose of this study is to compare the quality of intraoperative salvaged blood with that of Citrate Phosphate Dextrose(CPD) stored blood in 32 patients having cardiac surgery involving cardiopulmonary bypass. 20 patients in the control group received one week cild CPD stored blood and 12 patients received autologous intraoperative salvaged blood which prepared and processed with the Hemonetics Cell Saver Plus autotransfusion device. 1) Salvaged blood had a higher hemoglobin concentration(l7.6 vs 13.1 g/dl, P<0.005), a higher 2.3-diphosphoglycerate concentration(3.1 vs 0.6 mmol/L, P<0.005), higher pH(7.6 vs 6.8, P<0.005) than CPD stored blood group. Salvaged blood had a lower ATP concentration was(47.7 vs 67.7 pmol/dl, P<0.005) than stored blood group. 2) Platelet count was l3.4 X 10(9)/L and WBC count was 5.09 X 10(9)/L in salvaged blood. 3) Salvaged blood volume was 1411+/-487.9 ml(range 580 to 2325 ml). Intraoperative salvaged blood had a superior oxygen carrying and delivery capacity, but modification of washing is required to lessen the platelet and white blood cell count in the salvaged blood.

8.Apoptosis in Rat Thymus after Bolus Intramuscular Injection of 5-Fluorouracil.

We induced apoptosis in normal rats by intramuscular injection of 5-fluorouracil (5-FU) and immunohistochemically evaluated the thymus for the TdT-mediated dUTP biotin nick end labelling on the 1st, 3rd, 6th, 9th, 15th and 21st days following the bolus intramuscular injection. The injections of 5-FU induced a greater extent of apoptosis in the thymus. In the thymus, a mild increase in apoptosis was observed 24 hours after injection. The greatest number of apoptotic cells were seen at 72 hours. The size of the thymus decreased and the cortex thinned with hypocellularity. The injection of 5-FU caused massive cell loss in the thymus. Most apoptotic cells were scattered in the cortex and lower levels of apoptosis were also observed in the medulla. After 72 hours, the level of apoptosis returned to the control level. Considering the above results, we think that 5-FU induced toxicity may be related to 5-FU induced apoptosis in normal tissue, especially the thymus.

10.Cardiac Arrhythmias in the Perioperative Period.

80 patients, 40 patients without preexisting EKG abnormality (group 1) and 40 patients with preexisting abnormalities of EKG (group 2), receiving general anesthesia in the operating room were monitored continously with electrocardioscope, Servomed SMK 155-1 and were recorded. 1) 7 cases(17.5%) of group 1 developed a variety of arrhythmia, and 13 cases (32.5%) of group (32.5%) of group 2 developed a variety of arrhythmia. 2) The most common arrhythmia was premature ventricular contraction including bigeminies (13 cases, 60% of the total arrhythemia) and the most serious arrhythmia was seen 1 case of rapid ventricular tachycardia without artrial activity. 3) Continous cardiac monitoring is valuable, easy and practical in virtually all instance during anesthesia and surgery.
