1.Mucoceles in Post-operative Maxillary Sinuses: CT and MR Findings.

PURPOSE: This study describes the CT and MR findings of mucoceles occurred in the post-operative maxillary sinuses. MATERIALS AND METHODS: CT and/or MR of 19 maxillary mucoceles in 16 patients who had been operated by CaldwelI-Luc procedure were reviewed. CT scans were performed after contrast enhancement in axial and coronal planes with 5mm thickness. Three cases were studied with a 2.0 T or 0.5 T MRI, which demonstrated multi-compartment lesions. RESULTS: The lesions were bilateral in 3 cases. Compartmentalization of the antral cavity was seen in 7 out of 19 involved sinuses. Surgical bone defect of the anterior wall was the most frequent route of extension (11 cases) and extension into the infratemporal fossa with erosion of posterolateral wall was seen in 7 lesions. In 6 cases, the lesions involved orbit. The expansile and erosive bone changes were localized in every case. In one case with multi-compartment lesion, both T1- and T2-weighted MR images showed different signal intensities in each compartment which represented different protein concentration. CONCLUSION: Post-operative maxillary mucocele showed CT findings of localized erosion and bulging most frequently at the anterior wall in which the bone windows were made during the previous surgery. Post-operative compartmentalization of maxillary antrum may cause eccentric expansion of mucocete and each compartment may show different signal intensities on MR.

2.Bvaluation of the Eyebrow Approach for Cerebral Aneurysm Surgery.

Although the pterional approach is the standard surgical procedure for cerebral aneurysms, an eyebrow approach has been recently explored as minimal microneurosurgery with a small cranial opening along the eyebrow. To evaluate the efficacy and limitation of the eyebrow approach, the authors randomly selected patients to undergo surgery via pterional approach (n=32) and via eyebrow approach (n=38) within a period of 12 months. There was no significant difference in age distribution between patients with eyebrow approach and those with pterional approach (p<0.01). Also preoperative clinical Hunt & Hess grades and CT Fisher grades were not significantly different in both groups (p<0.01). On average, it took 70 minutes less to open and close the bone flap using the eyebrow approach than with the pterional approach. The success of the procedure was graded as good, fair, poor, or death. Favorable outcome rate consisting of good and fair was 90% for eyebrow approach and 82% for pterional approach. Mortality rate was 2.6% for eyebrow approach and 3.1% for pterional approach. The clinical evaluation showed no significant differences between the groups (p<0.01). The eyebrow approach for cerebral aneurysm is a simple and timesaving procedure with good cosmetic results. Also it gives good sucess rate with minimal brain retraction. Therefore, this approach can be alternative procedure for cerebral aneurysms. It may also be more useful for patients with poor general condition and old age.

3.Teleradiology(TELEACE) system: Results of field trial.

We report the results of field operation of TELEACE system between Kyung-Pook National University Hospital and Ul-Jin Goon Health Care Medical Center from December, 1990 to September, 1991, which had been operated as a kind of Integrated Services Digital Nework projects by KOREA TELECOMMUNICATION Inc. Ul-Jin Goon Health Care Medical Center transmitted 414 plain radiographs to our hospital in speed of 9600BPS. Each image was composed of 1024X1024 pixelsX8 bits/pixel. In our hospital, the image files were displayed on high resolution monitor (1280×1024 pixets). Text files of image interpretations were transmitted to the health care medical center. The two radiologists who had interpreted the transmitted images, went to the health care medical center and read radiographic film with blind test method. We obtained the following results: false negative rate of 6.3%, false postitve rate of 2.4%, mean sensitivity of 81.4%, mean specificity of 96.3%, and mean accuracy of 91.3%. In predictive value of 0.05, there was no significant difference between results of these two types of radiographs. In conclusion, TELEACE system was valuable to the clinicians isolated from services of radiologists.

4.Cryptosporidium Infection of Human Intestine: An Electron Microscopic Observation.

Protozoa of the genus Cryptosporidium are small coccidian parasite known to infect the mucosal epithelium of a variety of animals including human, causing fatal course in immunodeficient patients as well as self-limited illness in healthy individuals. Various life cycle stages including trophozoite, meront, merozoite, gametocyte and oocyst in infected mucosa are a diagnostic feature. Electron microscopy (EM) provides sufficient findings for genus and species identification of this parasitic organism. The authors presented scanning and transmission EM findings of Cryptosporidium parvum infection in two children: one with acute lymphoblastic leukemia and the other without any evidence of immune compromise.

5.Measurement of Olfactory Threshold in Normal Korean Adults with Combined Use of Bounded CCCRC Test and Step Method.

The high variability of measured olfactory thresholds in humans reflects very broad range of sensitivity within and among individuals. This can be derived from the strongly compressive nature of olfaction, which makes it relatively hard to compare the size of two stimuli in odor sensation. However, we know that some portion of the variation is the result of extrinsic factors e.g. the method used to measure the olfactory threshold. For this reason, in order to address the bias and reliability of a psycophysical method, we combined the use of the CCCRC (Connecticut Chemosensory Clinical Research Center) test with the step method in 40 normal Korean adults twice a week for the evaluation of threshold. The measured thresholds showed high test-retest reliability and the threshold estimates remained consistent during the testing without trial-biases. Based on the results, we can conclude that a combined test using the CCCRC test and step method can be used in the measurement of olfactory threshold in a clinical context.

7.Antibiotic Concentrations of Cerebrospinal fluid in Patients with Various Amount of Extraventricular Drainage during Intraventricular Infusion.

Phamacokinetic study of the antibiotic concentrations in the cerebrospinal fluid was done with high pressure liquid chromatography of CSF samples from twelve patients who had various amount of continuous extra-ventricular drainage during antibiotic administration. Five patients with large amount(over 150cc/day) of drainage had decreased CSF antibiotic concentrations dramatically below the effective concentration within 4 hours after intraventricular antibiotic administration, but six patients with small(less than 150cc/day) drainage maintained CSF antibiotic level above effective concentration even 24 hours after administration. In conclusion, repeated intraventricular administration of antibiotic is necessary to maintain the effective drug level in the patient with CSF drainage over 150cc/day.

8.A Case of Orbital Neurilemmoma.

Neurilemmoma (Schwannoma) is a extremely rare benign tumor which origin is thought to be of nerve sheath of peripheral, cranial and sympathetic nerve. Here we report a case of orbital neurilemmoma which has grown more than two years in the female patient of thirty years old. She visited to our hospital because of decreased visual acuity and protrusion of the right eyeball. Surgery of the orbit revealed a wall-nut sized, well encapsulated, gray colored, egg-shaped tumor which was confirmed by biopsy.

9.The Roll of Continuous ICP Monitoring for the Application of Shunt Operation in Adult Ventriculomegaly Patients.

We evaluated one month follow-up results of the 12 adult patients who showed ventriculomegaly in the aspects of symptoms. Evans ratio, periventricular low density, grades of radio-isotope cisternography, and grades of continuous intracranial pressure monitoring that were classified on the pressure wave and basal pressure level. The grades of continuous ICP monitoring were compared to the grades of radio-isotope cisternography. History, symptoms, CT findings, and radio-isotope cisternography are often helpful to decide shunt operation but sometimes continuous intracranial pressure monitoring gives invaluable information.

10.A Case of Posterior Reversible Encephalopathy Syndrome with Bilateral Visual Impairment.

PURPOSE: To report a case of recovery of bilateral cortical blindness in a patient with posterior reversible encephalopathy syndrome. CASE SUMMARY: A 46-year-old female visited the ophthalmology department due to abrupt visual acuity decrease. Ten days earlier, she had received conservative management due to anemia caused by menorrhagia and uterine prolapse. She underwent a gynecological operation to remove a uterine myoma two days previously, and was given a blood transfusion postoperatively because of excessive bleeding. After the transfusion, she complained of acute blurred vision. Her best corrected visual acuity (BCVA) was hand motion 10 cm in both eyes. There were no abnormal specific findings except retinal dot hemorrhage at the temporal side in the left eye on fundus examination. Her pupillary light reflex was normal and optical coherence tomography examination was unremarkable in both eyes. However, flash visual evoked potential findings showed reduced P100 amplitude in both eyes and she was diagnosed with posterior reversible encephalopathy syndrome based on brain magnetic resonance imaging. After close observation without any treatment, the symptoms gradually improved. Finally, her BCVA recovered to 1.0 and P1 and P100 amplitudes were restored to normal range in both eyes at 16 weeks from the first diagnosis. CONCLUSIONS: A patient complained of bilateral visual loss without other neurological symptoms after chronic blood loss and blood transfusion. She recovered visual acuity completely with prompt diagnosis of posterior reversible encephalopathy syndrome and close observation only.
