1.Hydoroxocobalamin (Vitamin B12) Effects on Nerve Function Impairments in Leprosy.

95 patients in Sorokdo national hospital injected 1-5mg Hydroxocobalamin (5mg/2ml/1ample) in 5-7 days/week, duration 6.4 months, average age 71.6 years old. Effects are 1) softening of hand movement(22/95), early effect, 2) improving of motor function (8/95) 3) sensory function(8/95) 4) sweating (3/95) 5) neuropathic pain and headache(neuralgia-like) (7/95) 6) jaw and hand tremor (7/95) 7) no response (41/95) 8) side effect (1/95) Leprosy is demyelinating peripheral neuropathy, Vitamin B12 effects on myelin, but unkown it's mechnism. Vitamin B12 improve nerve function impairments in leprosy, especially neuropathic pain and tremor, which lead to deformities and disabilities.

2.A Study of Physical Disorder in a Geriatric Psychiatric Inpatients.

BACKGROUND: This study was designed to investigate the relationship between psychiatric disease and systemic disease in geriatric psychiatric inpatients who were consulted to department of internal medicine and were above 6th decade, and was directed to assess the physical problems and possible ways to resolve them in a closed psychiatric unit. METHOD: Through evaluation of medical records of 225 inpatients who had consulted in department of internal medicine in Seoul National Mental Hospital from January 1, 1998 to December 31, 1999. We classified the physical illness according to ICD-9-CM and psychiatric disease according to NTA. RESULTS: The result were as follows: 1) In age sex distribution, male was 62.7%, and 81.8% of the subjects was 6th decade. 2) The most common systemic diagnosis were disease of circulatory system, disease of digestive system, and disease of respiratory system. 3) The most systemic disease was disease of circulatory system in 6th and 8th decade. 4) Disease of circulatory system in all psychiatric disease was the most frequent. 5) Rate of circulatory disorder was the highest in organic mental disorder. Disease of digestive system and disease of endocrine system were the most frequent in alcoholics. Disease of respiratory system showed the highest rate in schizophrenea. CONCLUSION: The clinical characteristics of psychiatric diseases influenced to get systemic disease in geriatric patients.

5.Ciliated Foregut Cyst of the Liver: Report of a case.

We report a case of ciliated hepatic foregut cyst which was incidentally found in a 64 year-old man. The cyst, 6 cm in diameter, was unilocular, solitary and was located in the medial segment of left lobe, just below the Glisson's capsule. Microscopically, the cyst wall consisted of 4 layers; pseudostratified ciliated columnar epithelium, subepithelial loose connective tissue, smooth muscle bundles and an outermost fibrous capsule. Although cartilage or subepithelial sero-mucous glands were absent, the morphologic features of the cyst correspond with those of an incomplete form of brochogenic cyst.

6.A biochemical investigation of the IL-1beta role of upon inflammation in mouse.

Cytokines are intercellular peptide mediators that regulate homeostasis and host defense reactions in living body. Of the diversity of cytokines in terms of biological accomplishment, interleukin 1-beta (IL-1beta) and tumor necrosis factor(TNF) are the most conspicuous cytokines with a wide variety of effects on cells involved in inflammatory and immune responses, and likely to be involved in the inflammatory pathogenesis of oral tissue as well. The present study was designed to explicate the role of IL-1beta on inflammatory revelation of oral tissues in mice biochemically. In the Induced arthritis by injection of 10 microgram LPS shown the relaese of 0.93 microgram IL-beta/joint with a peak at 4-5 h and diminished at 24h, and the release of TNF alpha of 1.25microgram/joint with a peak at 2-3h and diminished at 6h. After injection of th IL-beta into the joint, the member of leucocytes proliferated with a peak at 4-5h and diminished at 36h and the loss of proteoglycan showed with maximum at 15-30h. After injection of IL-1,6 into the oral tissue, cycloosygenase metabolites (PGE(2)) accumulated in the oral tissue with dose dependant. These elucidated IL-1beta to be inflammatory mediator in the early phase of its pathogenesis. Intraoral injection of recombinant IL 1beta induced the proliferation of leukocytes in situ. IL-1beta took an pertinent part in the development of inflammation and the succession of cellular infiltration. The results exemplify that IL-1beta ulavs a significant role in mediating inflammatory response induced by LPS in oral tissue the inflammatory response is regulated by IL-1beta at an acute phase of pathogenesis.

7.Spontaneous Lumbar Artery Bleeding and Retroperitoneal Hematoma in a Patient Treated with Continuous Renal Replacement Therapy.

Rupture of the lumbar artery is usually associated with trauma but rarely has been reported in association with anticoagulation. We present a 71-year-old man who developed spontaneous rupture of the lumbar artery leading to a retroperitoneal hematoma while receiving continuous renal replacement therapy (CRRT). The bleeding was confirmed by computed tomography and angiography and was controlled successfully using selective angiographic embolization. We suggest that spontaneous retroperitoneal bleeding should be considered in a case of sudden decrease in hemoglobin in a CRRT patient.

9.A Case of Postpoliomyelitis Muscular Atrophy.

A relationship between preceding acute paralytic poliomyelitis and the later development of motor neuron disease has only occasionally been suggested since it was first postulated by Charcot in 1875. The authors recently experienced a 20-year-old male who was considered to have postpoliomyelitis muscular atrophy. We report this case in view of its rarity and necessity of differential diagnosis from other neuromuscular disorders. Clinical presentation included slowly progressive muscle wasting of left thigh for 4 years, mild weakness of left arm and both thigh, intermittent fasciculation, and previous history of acute paralytic poliomyelitis. Electromyographic findings showed fibrillation potentials, positive sharp waves, fasciculations, giant motor unit potentials and reduced interference patterns. Muscle biopsy revealed scattered small angulated fibers, individual myofiber degeneration and mild inflammatory cell infiltration.
