1.A Case of Fiddler's Neck.

Fiddler's neck was first described by Peachey and Matthews in 1978 to denote a condition characterized by focal lichenification and pigmentation on the left side of violinist and violist neck just below the angle of the jaw. The etiologic facters are friction, local pressure, occlusion, poor hygiene and sweating. In only a few cases has a specific hypersensitivity to one or more of the ch!n rest material(wood, lacquer, dye) or the metallic fixing been found responsible. We report a case of 15-year-old female patient with Fiddler's neck who has played the violin for 8 years.

2.Tension Band Fixation in the Treatment of the Olecranon Fracture

Anatomically olecranon fracture is an intra-articular fracure, so reduction must be exact. Using the principle of Zuggurtungsosteosynthese by Weber, Pauwels' clinical study of olecranon fracture showed the superiority of tension band. 15 patients with olecranon fracture of the ulna who were treated with tension band and could be followed at the Department of Orthopedic Surgery of Han Il Hospital from January, 1974 to December, 1980 have been reviewed. The result obtained is as follows. We obtained the more rigid fixation with modifying the standard tension band locating the end of K-wire in the cortex of opposite side cortex of ulna. So, the result of treatment was satisfactory.

5.Chronic Prostatitis: Approaches for Best Management.

Prostatitis is a prevalent condition that encompasses a large array of clinical symptoms with significant impacts on men's life. The diagnosis and treatment of this disorder presents numerous challenges for urologists, most notably, a lack of specific and effective diagnostic methods. Chronic bacterial prostatitis is successfully treated with appropriate antibiotics that penetrate the prostate and kill the causative organisms. Prostatitis category III (chronic pelvic pain syndrome) is common, very bothersome, and enigmatic. Symptoms are usually prolonged and, generally speaking, treatment results are unsatisfactory. During the last decade, research has focused on the distress caused by the condition, but although our knowledge has certainly increased, there have been no real breakthroughs; controversies and many unanswered questions remain. Furthermore, the optimal management of category III prostatitis is not known. Conventional prolonged courses of antibiotic therapy have not proven to be efficacious. Novel therapies providing some evidence for efficacy include alpha-blocker, anti-inflammatory phytotherapy, physiotherapy, neuroleptics, and others, each offering therapeutic mechanisms. A stepwise approach involving multimodal therapy is often successful for treating patients. The UPOINT technique has been used to clinically phenotype these patients and drive the appropriate selection of multimodal therapy.

7.Three Cases of Membranous Reticular Inflammatory Polyp of the Colon.

Inflammatory polyposis of the colon is rare portion of the colon polyp entity. We experienced three patieats who suffered from colitis of unknown'etiology and discovered membranous retkular with/ without filiform polyp in the distal sigmoid colon of the them. Case 1, 56-year-old female, combined with basaloid cercinoma of the rectal canal that confirmed by pathology after abdominoperineal resection. So we obtained specimen of inflammtory polyp end tried to evaluate the mechanism of it. We found healed ulcer under the mucosal bridge. Case 2. 55-year-old male, had been suffered from irritable bowel syndrome. Prior to visit outpatient clinic, he complained tenesmus with diarrhea for a week. Inflammatory polyp was discovered by colonoacopy but his symptoms relieved by tranqulizers only. Case 3. 57-year-old male, has been a good health. He undertook colonoscopy for routine check thst revealed membranous reticular with filiform inflammatory polyp from distal sigmoid colon to hepatic flexure. Such shape of inflammatory polyp was not documented in Korea, so we reported three cases of it with review of the literature.

8.The Retinal Changes in an Experimental Vitreous Hemorrhage.

The effect of vitreous hemorrhage on the retinal structures was studied in twenty young adult pigmented rabbits weighing 2 to 3kgs. The autologous blood was injected intra-vitreally in one eye of the animals. Five animals of each group were sacrificed for light and electron microscopic studies of the retina near the visual streak in the posterior pole in one week, two weeks, one month and two months. In light microscopy of the rabbit retina, hemosiderin-laden macrophages are located in the vitreous cavity. Iron stain positive particles are present in the retina, particularly in the pigment epithelium. These particles are also scattered in the retina after two weeks on. By electron microscopy, there is no significant ultrastructural change in one week. Swelling of mitochondria in the bipolar and Miiller cells, and in the inner segments of the cone cells are noted along with disarrangement of disc stacking in 2 weeks. Many myelin figures appear in the cytoplasm of the inner segments of visual sells, outer plexiform and inner nuclear layers. Hemosiderin pigments appear in the pigment cells and in the cytoplasm Muller cells. In inner nuclear layer, a necrotizing cell demonstrates pyknotic nucleus and focal cytoplasmic degradation in one month. Hemosiderin pigments are present in ganglion cells, Miiller cells and pigment epithelial cells. Disc stacking is altered in outer segment of the visual cells, and the pigment epithelial cell demonstrate increased engulfment of outer segment of visual cells in two months. However, the electrophysiological study with ERG do not demonstrate any abnormality.

9.A Case of Benign Mixed Tumor Presenting as a Nodular Eyelid Lesion.

A 23-year-old woman visited our clinic for the evaluation of a small bean-sized mass in the right lower eyelid. She noticed a mass in the right lower eyelid 6 years ago. The mass was treated with incision and curettage under the impression of chalazion twice at the local clinic, but recurred soon after the operation. On examination, the visual acuity was 1.0 in both eyes. A firm, non-tender, smooth, movable and small bean-sized mass was palpable in the right lower eyelid. A white scar from the previous operation was seen on the palpebral conjunctiva of the right lower eyelid. The mass was excised through the conjunctiva. The histopathological diagnosis of the excised mass was benign mixed tumor. The patient was recommended of further radical excision. The pentagon-shaped excision of the right lower eyelid under frozen section control was done 2 weeks after the first operation. The patient is doing well as of now. The benign mixed tumor of the lower eyelid can arise from either ectopic lacrimal gland or accessory lacrimal gland.

10.Effects of Atypical Antipsychotics on Serum Prolactin and Testosterone Levels in Schizophrenic Patients.

OBJECTIVES: The dopamine-blocking effects and the associated side effects(amenorrhea, lactation, sexual dysfunct of classical antipsychotics in schizophrenic patients have been studied for a long time. The purpose of this study to find out these effects of new antipsychotics(risperidone, olanzapine) in schizophrenic patients treated with clinical relevant doses. METHOD: Plasma levels of both prolactin and testosterone were measured in 91 schizophrenic patients(28 taking haloperidol, 4-20mg/day ; 31 taking risperidone, 2-6mg/day/ 32 taking olanzapine, 5-20mg/day). RESULTS: In male schizophrenic patients, the prolactin levels of risperidone group(76.44+/-38.85ng/ml) and haloperidol group(60.26+/-20.74ng/ml) had no significant difference, but were significantly higher than that of olanzapine(26.90+/-5.36ng/ml). In female, the prolactin level of olanzapine group(36.66+/-17.55) was significantly lower than those of risperidone(121.7+/-.33) and haloperidol group(161.66+/-37.53). And prolactin level of risperdone group was lower than that of haloperidol group. While the testosterone plasma level of risperidone, haloperidol and olanzapine in both male and female schizophrenic patients had no significant difference. CONCLUSIONS: At doses known to be effective in popular clinical setting, prolactin level in patients taking risperidone we higher than that of haloperidol, while olanzapine showed no significant difference in terms of prolactin plasma level haloperidol. New antipsychotics may not influence the testosterone plasma level.
