Double elevator paralysis is a syndrome in which the superior rectus and the inferior oblique muscle of the same eye are paralyzed. The authors have experienced a case of double elevator paralysis with 35 prism diopters hypotropia and 20 prism diopters exotropia on the right eye. The patient was 19 years old female who visited our hospital because of poor cosmetic appearance. Knapp procedure was performed primarily to correct hypotropia, but 20 prism diopters hypotropia remained. 5 months later, a second operation was performed consisted of 7.5 mm recession of the left lateral rectus and 5mm recession of the right inferior rectus in order to correct the exotropia and residual hypotropia. After the second operation, ocular position in primary position was nearly orthophoric and upward movement of the right eye was considerably improved.

Nasal dorsal suppor may be weakened or lost during the corrective rhinoplasty procedures such as resection of dorsal deviation, osteotomies and reposition of deviated nasal bones, and incision, fracture, resection or scoring of the dorsal septal strut especially in the small flat noses. Partial or total augmentation of the nasal dorsum has an important role in the correction of asymmetry and restoration of aesthetic balance in the considerable amount of traumatic nasal deformities. We reviewed medical records, pre- and postoperative photographs of 53 patients with traumatic nasal deformities, which had been corrected by augmentation rhinoplasties. The majority of the patients were male (70%), ages of twenties and thirties (79%), injection by automobile accident, fist blows, and sports activities (76%), and were operated more than 3 years after injury (74%). Types of deformities were deviation only (47%), depression only (23%), deviation and depression (19%), deviation and hump (8%), and depression and widening (6%). At least two corrective procedures were needed in the deviated deformities and one procedure in the depressed deformities. Materials for augmentation were silicone implants in 55% and autogenous tissues in 45%. In terms of postoperative complications, displacement and/or absorption were observed in 6 of 24 (25%) autogenous augmentations and displacement and/or swelling in 8 of 29 (27.6%) silicone augmentations.

PURPOSE: This study analyzes the relationship between work-life conflict and turnover intention among hospital nurses. Furthermore, it seeks to determine whether the relationship between the two varies depending on the shift work condition of the nurses. METHODS: A total of 435 nurses working at five tertiary hospitals were included in the study. The questionnaire included items on turnover intentions, work-life conflicts, sociodemographic factors, working conditions, and internal and external resources. RESULTS: The level of work-life conflict among shift work nurses was higher than that of non-shift work nurses. The turnover intention was also higher for shift work nurses than for non-shift work nurses. Results from the multivariate logistic regression analysis revealed that the work-life conflict of nurses had a positive relationship with turnover intention. The effect of work-life conflict on turnover intention was greater for shift work nurses than for non-shift work nurses. CONCLUSION: Work-life conflict had a greater impact on the turnover intention of hospital nurses working under shift work conditions. Based on this study's findings, it can be suggested that, in order to reduce high turnover intention, it will be necessary to make specific efforts to balance work-life conflict and improve working conditions.

OBJECTIVE: The purpose of this study is to evaluate clinical and radiological outcomes analysis of the laminoplasty in the elderly patients, and to compare with the non-elderly patients. METHODS: A retrospective study of the short term result in patients who had treated with the laminoplasty for cervical spondylotic myelopathy (CSM) was performed. From January 2008 to December 2012, total 62 patients were operated with single open-door technique because of CSM; 28 patients were the elderly and 34 patients were the non-elderly. We evaluated some factors including sex, symptom duration, estimated blood loss during operation, operation time, hospitalization day, complications, pre- and postoperative modified Japanese Orthopedic Association (mJOA) score, recovery rate of mJOA score, achieved mJOA score, mean cervical canal width and expansion ratio of antero-posterior diameter in order to identify difference between the two group. Clinical outcomes were calculated with the recovery rate of mJOA score at the time of one year after operation. RESULTS: Mean age were 71.9 in the elderly group and 52.9 in the non-elderly group. Although postoperative mJOA score in the elderly group was lower than that of the non-elderly group, achieved mJOA score was statistically same between the two groups. Other clinical and radiological outcomes were also statistically same. CONCLUSION: We conclude that the laminoplasty also assures good clinical outcomes in the elderly patients with CSM, same as in the non-elderly group.

A 49 years old male patient who suffered from deterioration of posterior neck pain, left hand numbness, left lower limb pain and gait disturbance for 3 years visited our outpatient department. He had been diagnosed as non-dysraphic cervical intradural lipoma and operated in August 1990. On the radiologic images, we found the regrowth of non-dysraphic cervical intradural lipoma from C2 to C7 level, which surrounds and compresses the cervical spinal cord. Previous subtotal laminectomy from C2 to C7 and severe cervical lordosis were also found. Appropriate debulking of lipoma mass without duroplasty was successfully done with intraoperative neurophysiological monitoring (IONM). We are following up the patient for 24 months via outpatient department, his neurologic symptoms such as hand numbness, gait disturbance, left lower limb pain and posterior neck pain have improved. We describe a rare case of regrowth of non-dysraphic cervical intradural lipoma.

Direct arterial pressure monitoring by means of an intra-arterial catheter has been considered benefit for assessment of the critically ill patients, safe conduct of controlled hypotension and frequent obtaining arterial samples for blood gas analysis. However in stead of these advantages, there would be high incidence of potential complications of arterial catheterization, such as pain, trauma to the artery and surrounding tissues(e. g., nerve), hematoma, infection, thrombosis, and distal embolization of air, clot, pieces of the catheter, and other debris. We have recently experienced an unexpected episode of amputation of the upper extremity resulting from axillary arterial occlusion following accidental injection of diphenylhydantoin through the radial arterial catheter in 57 year old neurosurgical patient. To prevent these serious and unexpected complications following arterial cannulation, we have to keep a continuous interest and vigilance to those who have invasive monitorings and those who are stranger to handle the arterial cannulation.
