2.Biomechanical Study about Strength and Strain of Pretwist on Properties of Canine Central 1/3 Patellar Tendon.

Pretwisting the patellar tendon graft has been advocated hy many authors. Lambert has described a 90 degrees pretwist to benifit the fat pad vascular supply as well as to "" simulate the helicoid course of the ACL "". This technique has also been recommended to fascilitate fixation. Jackson has recommended a 90 pretwist to decrease the possibility of abrasive wear on the tunnel edge, and also to fascilitate fixation as much as a 180 degrees pretwist has heen recommended. However, in spite of all these recommendations, little work has been done on the effect of such pretwisting on the biomechanical behavior of patellar tendon grafts. The purpose of this study is to discover, by experiment, the effects of a 360 degrees external rotation pretwist on the following tensile mechanical properties of canine central 1/3 patellapatellar tendon-tibia complex : (a) ultimate stress, (b) ultimate strain, (c) average elastic modulus, and (d) strain energy density. Five groups of each S specimens were divided as group 1 (0 degrees external rotation), group 2 (90 degrees external rotation). group 3 (180 degrees external rotation), group 4 (270 degreesexternal rotation), and group 5 (360 degrees external rotation). The direction of pretwist meant the twist of tibia around patella. The effects of pretwist were analyzed using parameter one-way analysis of variance (ANOVA)(P1) and nonparameter Kruskal-Wallis one-way analysis of variance (ANOVA)(P2) with repeated measures followed by Scheff multiple comparison test. A p values of < 0.05 was considered statistically significant. Among each groups ultimate stress (P1=0.059, P2=0.095), ultimate strain(P1=0.154, P2=0. l65), and average elastic modulus(P1=0.814, P2=0.818) were not statistically significant. Only strain energy density was the statistically significant difference between group 2 and group 5(P1=0.008, P2=0.022). As a result, there were observed that the ultimate stress and strain energy density were decreased and ultimate strain was decreased but there were not statistically significant differences hetween groups because of the small number of samples, and there will be advantages for graft to avoid ahrasive wear on the tunnel edge and lateral femoral condyle. But, more studies are needed for human being in the future.

4.A Case of Neonatal Infective Endocarditis without Cardiac Anomaly.

Neonatal infective endocarditis is fatal and rare disease but recently there is a trend of an increase in incidence even without congenital heart disease when there are high risk as prematurity, central vein catheterization and neonatal cardiopulmonary resuscitation. Symchych reported that in about 3% of autopsy cases of an year, the findings of infective endocarditis were noted. In Korea, there was a case report but it was case of a premature infant who had umbilical vein catheterization and respiratory ventilator treatment. We experienced a case of neonatal infective endocarditis in a female infant who was delivered at 34 weeks gestational age without receiving central vein catheterization or neonatal cardiopulmonary resuscitation. Although she received intensive antibiotic therapy for staphylococcus cultured in blood, she died necessarily. We reported a case of neonatal infective endocarditis without cardiac anomaly with brief review of related literature.

5.One - staged Saucerization and Cancellous Chip Allograft for Treatment of Chronic Localized Osteomyelitis.

Treatment of chronic localized osteomyelitis is the same as the other osteomyelitis, that is, curettage and debridement of all dead tissue which is called saucerization. After saucerization there leaves empty cavity and autogenous bone graft has been used for filling the cavity in lower extremity hecause of weight bearing and avoiding fracture. We treated seven cases of chronic localized osteomyelitis with one-staged saucerization and cancellous chip allograft and retrospective analysis was done. All healed without complication and no recurrence was ohserved. Most orthopaedic surgeons are not willing to use allograft on the site of osteomyelitis because of the fear of recurrence. But. we think that as far as complete removal of infected and dead tissue, allograft could he a good solution in terms of rapid remodelling and early weight hearing. Another advantages of allograft are to get a large amount of bone and no damage of iliac apophysis. In summary, one-staged saucerization and cancellous chip allograft would he very useful treatment for chronic localized osteomyelitis.

6.The Resistance to Ciprofloxacin in Bacterial Ocular Disease.

PURPOSE: To evaluate the change of the sensitivity and resistance to ciprofloxacin of isolates in bacterial ocular disease, bacterial keratitis and conjunctivitis. METHODS: From January 1992 to December 1999 in KyungHee Medical Center, we carried out direct smear and culture from conjunctiva and cornea and studied retrospectively. RESULTS: A total of 420 bacteria and fungi were isolated and 39 strains were identified. Antibiotic sensitivity test was done in isolated bateria and fungi. Gram-positive bacteria accounted for 324 of all bacteria, gram-negative organisms for 81 in isolated causative organisms. Major causative organisms of bacterial ocular disease, bacterial keratitis and conjunctivitis are coagulase-negative staphylococcus, Staphylococcus aureus, Pseudomonas aeruginosa in that order. Among 420 isolates, 405 bacteria and 15 fungi was isolated. In vitro examination for susceptibility and resistance of causative organisms to ciprofloxacin was done in 67 of 420 isolates. Of the 67 isolates, 57 were sensitive to ciprofloxacin and 10 were resistant to ciprofloxacin. All resistant cases to ciprofloxacin was 2 cases in 1994, 7 cases in 1998, and 1 case in 1999. CONCLUSIONS: The resistance to ciprofloxacin has a increasing tendency. Therefore, ciprofloxacin should be used carefully, when used for prevention of infection and primary treatment in mild ocular diseases.

Benign solitary schwannoma of the chest wall is a rare disease of nerve sheath origin A 43-year-old female patient was admitted to our hospital for right upper quandrant pain Chest X-ray shows nonspecific finding, ultrasonography & CT scan revealed a solid tumor on the right lateral chest wall. On aspiration needle biopsy, this mass was turned out to be a schwannoma of the right lateral chest wall. This mass was excised surgically in an encapsulated state by right lateral thoracotomy. Histological section revealed a typical schwannoma, which is characterized by Antoni A and Antoni B tissues of high & low cellularity.

8.The Epidemiology of Childhood Enuresis in Seoul and Kyunggi Province.

PURPOSE: To establish the prevalence of childhood enuresis in Korea, an epidemiological study was performed. We also evaluated factors closely related to enuresis and investigated methods of managing enuresis in family. MATERIALS AND METHODS: A self-administered 23 questionnaires was completed voluntarily by the parents of 2,575 children aged 5 to 13 years living in Seoul and Kyunggi Province. RESULTS: From a response rate of 66%, the overall prevalence of enuresis was 8.74%(20.8% in pre-school children, 6.2% in elementary school children). The prevalence of enuresis was higher in boys(60.9%) than in girls(39.1%). The factors closely related to enuresis were deep sleeping, inability to wake-up for voiding, early steeping, family history of enuresis and daytime incontinence. The prevalence of primary and secondary enuresis was 78% and 22%, respectively. When analyzed by a 10-point visual analogue scale to measure parental concern about enuresis of their kids, the mean concern score was 5.37. There was a significant relationship between the frequency of enuretic episode and parental concern score(p < 0.05). Of the enuretic children, 27.6% had consulted with a health worker and the most favored method of treatment was herb medicine. CONCLUSIONS: These results suggest that prevalence rates of childhood enuresis in Seoul and Kyunggi province are similar to those of previous studies of other countries. The sleep pattern, daytime incontinence and family history are closely related to development of enuresis.

9.The Value of Colposcopy for Early Diagnosis of Cervical Cancer.

This study was performed to evaluate the diagnostic value of colposcopy by comparing colposcopic findings with cytology, histology of colposcopically directed biopsy and final biopsy result on hysterectomy. From January 1, 1988 to August 31, 1996, the colposcopic examinations was undertaken in 1200 patients, excluding grossly visible carcinoma of the cervix. The result were as follows : 1. Colposcopic examinations were satisfactory in 95.4% and unsatisfactory in 4.58%. 2. The accuracy rate of cytologic diagnosis was 73.1% and colposcopic impression was 93.1%. 3. The false negative rate of colposcopic impression was 2.6%, much less than 24,7% of cytologic examination. 4. The cytologic accuracy rate in cervicitis was higher than that of colposcopic impression but the accuracy rate of colposcopic impression in diagnosis of cervical intraepithelial neoplasia was higher than that of cytology. 5. Among 97 cases in which colposcopically directed biopsy were performed, more advanced lesion were disclosed in 12 cases(12.4%).

10.Percutaneous Transhepatic Venous Embolization of Pulmonary Artery Aneurysm in Hughes - Stovin Syndrome.

Hughes-Stovin syndrome is an extremely rare entity. We present a case of a 42-year-old man, who developed deep vein and inferior vena cava (IVC) thrombosis, repeated internal bleeding and pulmonary artery aneurysms (PAAs). The patient presented with massive hemoptysis and with PAAs of a 2.5 cm maximum diameter. We describe the successful percutaneous transhepatic venous embolization of the PAAs due to occluded common vascular pathways to the pulmonary artery.
