1.Persistent light reaction.

Persistent light reaction is a condition of chronic photodermatitis in which photosensitive reaction persists even after the rernoval of all photosensilizers. A 56-year-old man had experienced a recurrent dermatitis involving primarily the face, neek, forearms and hands for 9 years, this condition was aggravated by sunexposure. Photopatch testing disclosed a strongly positive reaction to chloropromazine, promethazine, acid trichlorocarbanilide, Phototesting also revealed lowered MED with UVA and UVB thar norrmal mean value.

2.Systemic Scleroderma in Childhood.

The onset of systemic scleroderma in the first. decade of lif occurs in less than 1.5% of instances. Systemic sclerodrma in childhood have been characterized by less severe visceral involvement and a benign course We report a boy, aged 7 years, with a history of Raynaid; phenomenon and dysphagia. He had a mask-like face, flexion contracture of hands and knee joints, digital pitting scars, ichthyosiform skin change on leg and hyperpigrnented induration on all extremities and trunk. Antinuclear antibodies were positive in nucleolar type and Scl-70 antiocy was also present. On radiologie study, the pattern of reflux esophagitis and resorption of the listal ends of all phalanges were nated. Histopathological findings were compatible with sclerodma. He was treated with Penicillamine-D and oral corticosteroic with good improvernent.

3.Personality Characteristics and Insight Acquisition in Schizophrenia.

OBJECTIVES: We attempted to investigate personality characteristics influencing insight acquisition in patients with schizophrenia. METHODS: Tridimensional personality questionnaire(TFQ) for neurobiologically based personality characteristics assessment and Barron's ego strength test were administrated to 30 patients with schizophrenia who had good insight and 30 patients who had poor insight. Each group was also compared with 453 normal health controls. RESULTS: There were no significant differences between good and poor insight acquisition groups on ego strength. Poor insight acquisition group showed significantly higher reward dependence dimension on Tridimensional Personality Questionnaire than good insight acquisition group, though the other two personality dimensions showed no difference. The good insight group showed no difference on three TFQ dimensions from normal healthy control group. However, the poor insight group demonstrated higher score in reward dependence dimension than the healthy control group but not in the other two dimensions. CONCLUSION: These results suggest that neurobiologically determined personality characteristics rather than ego strength would be closely related with insight acquisition in schizophrenia. Reward dependence as groups of behavioral characteristics, mainly derived from central norepinephrine mediated behavior would be related with insight formation. The roles of norepinephrine in schizophrenia have not been lolly understood yet. Further understandings regarding insight formation process and role of norepinephrine In schizophrenia could give some clues on insight acquisition in patients with schizophrenia.

Staphylococcus aureus is one of many causative agents of the fu uncles. As the S. aureus strains resistant to penicillinase-repsistant penicillins (methicillin-resistant S. aureus, MRSA) are more frequently isolated, their accuiate identification became a very important step in selection of an appropriate therapeutic regimen. We report herein a case of multiple furunculosis caused by metkicillin-resistant S. aureus in a 11-year-old male patient, who was successfully treated with sulbactam/ampicillin and amikacin.

8.Percutaneous Transhepatic Venous Embolization of Pulmonary Artery Aneurysm in Hughes - Stovin Syndrome.

Hughes-Stovin syndrome is an extremely rare entity. We present a case of a 42-year-old man, who developed deep vein and inferior vena cava (IVC) thrombosis, repeated internal bleeding and pulmonary artery aneurysms (PAAs). The patient presented with massive hemoptysis and with PAAs of a 2.5 cm maximum diameter. We describe the successful percutaneous transhepatic venous embolization of the PAAs due to occluded common vascular pathways to the pulmonary artery.

9.Circulating Immune Complexes in Patients with Leprosy.

The occurrence of immune complexes in the serum from rats infected with M. leprae-murium and 38 patients with leprosy were studied by the polyethylene glycol precipitation complement consumption (PEG-CC) test and the results were compared in the various forms of the disease. Circulating immune complexes (CIC) were significantly increased in the sera from rats infected with M. lepraemurium compared to normal control rats (P < 0.005). There were no significant differences between the the level of CIC in the sera from lepromatous leprosy patients and that from tuberculoid leprosy patients, but in the sera from patients with erythema nodosum leprosum (ENL) the level of CIC was significantly increased (P < 0.005). And although we couldn't find a corre1ation between the level of CIC and bacterial indices in lepromatous leprosy patients, CIC tends to de-crease after negative conversion of their bacterial indices. These findings suggested that the detection of CIC can be of some practical interest in the early diagnosis of ENL and can be a valuable assessment in following the therapy after negative conversion of their bacterial indices.
