Granulosa cell tumors are uncommon neoplasm of ovary that are characterized by their long natural history and for their tendency to recur years after an aparrent clinical cure. Endometrial cancer occurs in association with these tumor in at least 5% of cases and 25%-50% are associated with endometrial hyperplasia. In this study, we present a case of granulosa cell tumor of right ovary with endometrial cancer after left oophrectomy.

4.Study of Facts and Problems of Reports for Court about Traumatic Mental and Behavioral Disabilities.

Traumatic injury can result not only in physical disability but also in mental and behavioral sequelae. It is very important to properly judge the severity of disability as well as the need for assistance and future medical treatment. It is very difficult to establish objective guidelines for assessing mental and behavioral disabilities due to the inherent peculiarity of the disabilities. This study uses reports generated for court because they need to be more judicious than reports for other types of evaluation. We analyzed reports concerning the facts and problems associated with evaluation of mental and behavioral disabilities. We hope this study will help further our understanding of mental and behavioral disabilities.
5.The Clinical Change of Bone Mineral Density after Surgery and Hormone Replacement Therapy on Menopausal Woman.

The objective of this study is to compare bone mineral densities(BMD) between the symptomatic menopausal women and the asymptomatic menopausal women, between the spontaneous menopausal group and the surgical menopausal group, and especially between the HRT group and no-HRT group. From January, 1995 to December 1995, this study was prospectively analysed in 96 menopausal women(47 spontaneous menopausal women, 23 surgical nopausal women) who were admitted to Department of Obstetrics and Gynecology, Chosun University Hospital for HRT. The all women was measured T-score of bone mineral densities at L1, L2, L3 and L4 of lumbar spine and neck, trochanter, ward`s area of femur from dual-energy X-ray absorptiometry(DEXA). Student t-test and chi-squre test were used for statistical comparison. The results obtained were as follows: 1. The mean age of the osteoporotic group(54.96 +/- 6.6) is higher than the non-osteoporotic group(50.55 +/- 81). 2. No difference is detected in the BMD between the surgical menopausal group and the spontaneous menopausal group, because duration of the follow-up is too short and the study group is too small perhaps. 3. Descent of T-score in the symtomatic menopausal women is not severe than the asymptomatic group, but especially in L4 area, T-score of the symptomatic group(-1.76 +/- 1.3 < -1.04 +/- 1.4) is more decreased(p<0.04). 4. T-score between the long term HRT group and the no-HRT group is different markedly. HRT during only 1 year is not attributed to BMD loss. So the continuous HRT over 1 year and Follow-up is need to improvement of BMD in the menopausal women with risk factors.

6.A Case of Small Cell Carcinoma arising in a Benign Mature Cystic Teratoma of the Ovary with Metastasis to the Supraclavicular Lymph Nodes.

The development of cancer in a pre-existing benign cystic teratoma of the ovary with distant metastasis to the supraclavicular lymph nodes has not been previously reported. We experienced a case of a 28-year-old woman with distant metastasis from small cell cacinoma arising in benign mature cystic teratoma of the ovary to the supraclavicular lymph nodes. The staging laparotomy and total abdominal hysterectomy with both salpingooophorectomy were peformed. There was no metastatic site except the supraclavicular lymph nodes, thereafter adjuvant chemotherapies with bleomycin, etoposide, cisplatin have been doing. We report a case of benign cystic teratoma with malignant change and metastasis with the literature.

7.Lipoma on the Lower Lip

Lipoma is a very common benign tumor of the adipose tissue, but it occurs infrequently in the oral cavity. A 66-year-old man presented with an asymptomatic, solitary, 1×1-cm-sized yellowish mass on the lower lip, since 2 years. For differential diagnosis, skin biopsy was performed, and histopathologic examination showed features indicative of lipoma. We herein report a rare case of oral lipoma occurring on the lower lip.
8.Acquired Periungual Fibrokeratoma: An Atypical Case Covering the Entire Thumb Nail

Acquired periungual fibrokeratoma, first reported by Cahn in 1977, is a rare, benign fibrous tissue tumor located in the periungual area. The pathophysiology of acquired periungual fibrokeratoma is unknown. In general, trauma is often thought to be a predisposing factor. A 57-year-old woman presented with a 15-year-old asymptomatic, solitary, 1.8×1.5-cm, skin-colored mass on the Lt. thumb nail. The patient reported that they had been wounded on the nail root and the mass had since grown. For differential diagnosis and to determine the extent of the tumor, MRI of the finger was performed. Radiologic presentation showed a non-enhancing T2 low, T1 low signal mass above the nail plate. Histopathologic examination demonstrated a dome-shaped tumor consisting of thick dermal collagen bundles with increased capillaries. Through clinical, radiological, and histopathologic findings, we diagnosed the tumor as acquired periungual fibrokeratoma, and the lesion was removed through complete excision. Herein, we present a rare case of acquired fibrokeratoma covering whole thumb. The specific diagnosis was aided by MRI more through the acquisition of more accurate information about the location and signal characteristics that reflect underlying pathological features.
9.Comparison of the Efficacy of Antibiotic Monotherapy and Antibiotic Plus Alpha-blocker Combination Therapy for Patients with Inflammatory Chronic Prostatitis/Chronic Pelvic Pain Syndrome.

PURPOSE: The objectives of this study was to compare the efficacy of antibiotic monotherapy with antibiotic plus alpha-blocker combination therapy for the treatment of inflammatory chronic pelvic pain syndrome (CPPS) patients. MATERIALS AND METHODS: Between October 2005 and May 2006, 69 patients who were diagnosed as CPPS(National Institutes of Health; NIH-catagory IIIa), were included in this study. The patients were randomly placed into two groups: group I was treated with gatifloxacin alone(35 patients), and group II was treated with gatifloxacin and doxazosin(34 patients) for 6 weeks. For all the patients, the urinalysis, expressed prostatic massage, the National Institute of Health-Chronic Prostatitis Symptom Index(NIH-CPSI) and a distal rectal examination(DRE) were performed at the initial visit. The NIH-CPSI was compared both before and after the treatment. RESULTS: On the initial diagnosis, the mean CPSI of the group I patients was 24.0+/-6.3, and that for the group II patients was 24.7+/-6.9. After the treatment, that of the group I was 16.6+/-5.4, and that of group II was 13.4+/-5.3. After 6 weeks of treatment, the changes in the total CPSI scores had significantly improved in group II compared with group I(p<0.05). A statistically significant improvement occurred in the pain score, the voiding symptom score and the quality of life in the group II compared with group I(p<0.05) CONCLUSIONS: This study suggests that combination therapy of antibiotic plus alpha-blocker would be more effective than antibiotic monotheraphy for treating patients with inflammatory chronic prostatitis/chronic pelvic pain syndrome.

10.Clinical Correlates of Metabolic Syndrome in Patients with Chronic Schizophrenia.

OBJECTSIVE: This study investigated the frequency of metabolic syndrome, associated factors and its risk for cardiovascular morbidity in schizophrenic patients. METHODS: In this cross-sectional study, nineteen-six patients with schizophrenia defined by DSM-IV criteria were included. All subjects were assessed for the presence of metabolic syndrome by means of the criteria of the National Cholesterol Education Program-Adult Treatment Panel III. In addition, we compared clinical characteristics, laboratory variables and risk for cardiovascular morbidity between patients with and without metabolic syndrome. RESULTS: Of patients, 43% had metabolic syndrome. Metabolic syndrome was associated with illness chronicity and current status of antipsychotic agents use, total duration of atypical agents used, but not with daily chlorpromazine equivalent dose. Compared with the patients without metabolic syndrome, patients with metabolic syndrome had higher risk for cardiovascular morbidity. CONCLUSION: This study suggests that metabolic syndrome is common among patients with chronic schizophrenia. Clinicians should be cautious to aware the increased risk for the metabolic syndrome and intervene actively to prevent and treat metabolic morbidity among chronic patients with schizophrenia.
