1.Clinical Observation of Peripheral Nerve Injury of the Upper Extremity

Ninety two injuries of the main nerves in the upper extremity observed in 79 patients, were studied at the Department of the Orthopedic Surgery, 17th Army Hospital from May 1965 to Aug. 1968. The treatment given consisted of neurolysis in 15, neurorrhapy in 77(primary 43 and secondary 34) and tendon transplantation and transfer in 11 cases. The results for the median, ulnar and radial nerves after primary and secondary suture were analyzed with respect to the level of injury(high and lower). The results of the median nerve repair were more succesful than those of the ulnar nerve, In these two nerve injuries, the sensory recovery was better than the motor and secondary suture gave better results than primary suture. Thirteen cases failed to respond to primary or secondary nerve repair, among those tendon transfer or tendon transplantation was tried in eleven cases, from the latter approximately 25% of motor recovery could be seen.

7.Primary Cardiac Angiosarcoma: A case report and rewiew of literture.

We report a case of primary cardiac angiosarcoma in a 35 year-old woman. She presented with dyspnea, facial edema and neck vein distension during 1 month. The oval round large mass(8x4 cm) filling the right atrium infiltrated into the entire thickness of the lateral wall and extended to the vena cava and ventricle. The interatrial septum and pericardium seemed to be intact. Despite the surgical excision and adjuvant chemo-and radiotherapy, the patient died 8 months later due to tumor recurrence. The histologic findings of tumor varied from benign looking capillary proliferation mimicking granulation tissue to sarcoma composed of spindle cells. But anastomosing vascular channels lined by anaplastic polygonal cells and intracytoplasmic lumen containing red blood cells were characteristic findings. These tumor cells showed positive reaction to endothelial cell antigen and factor VIII-related antigen. The intercellular junctional complex and vascular channels formed by cytoplasmic process of tumor cells were identified by the ultrastructural study.

8.Determination of DNA-DNA Hybridization Condition for Rapid Identification of Mycobacterium Species.

Rapid identification of Mycobacterium spp. isolated from patients is important with increased isolation of mycobacteria other than tubercle bacilli (MOTT). DNA-DNA hybridization with streptavidin-peroxidase and tetramethylbenzidine (TMB) color reaction method was recognized as a useful tool for identification of various species of mycobacteria. In this study, optimum condition of the test was determined. The optimal concentrations of tetramethylbenzidine dihydrochloride and hydrogen peroxide for streptavidin-horseradish peroxidase were 0.3-0.6 ug/ ml and 0.16 mM, respectively. The TMB stock solution was stable when prepared in methanol and the dilution of TBM stock solution in 10 mM sodium citrate-10 mM EDTA solution (pH 5.0) gave highest peroxidase-TMB activity. The suitable composition of hybridization solution consisted of 2 x SSC, 10% dextran sulfate, 50 ug/ml salmon DNA, 5 x Denhardt's solution, and 50% formamide. The 5-minute heating at 100C of test DNA prior to photobiotin labeling significantly increased the reaction. In conclusion, DNA-DNA hybridization method with streptavidin-peroxidase and TMB color reaction method may be useful for rapid identification of Mycobacterium spp. isolated from patients.
