Drug-Related Side Effects and Adverse Reactions*

BACKGROUNDS: A stroke is a disease of the nervous system which is accompanied by important chronic disabilities and emotional disorders. These disabilities causes loss of sensation(diminished basic movement). METHODS: Sensation rehabilitation therapy through Activities of Daily Living(ADL) Training is necessary. Reports show that patients who receive occupational therapy in the early stages have an extremely high rate of recovery in the early stages. It is important for a occupational therapist to provide continued reha- bilitative treatment to, enable the patient to accomplish Activities of Daily Living Training independently by treating everything from sustaining posture in bed to normal movement and improvement in sensory perception. occupational therapy refers to the `use of self` of a occupational therapist to help the patient overcome difficulties and increase abilities of the patients by applying knowledge, skill, care and basic patient merits. RESULTS: A occupational therapist must have the basic knowledge, self-confidence and individual skills for a systematic program of therapy in order to be able to communicate with the patient and increase the effectiveness of the therapy. CONCLUSION: This study showed that therapy for patients who received therapy soon after diagnosis of stroke showed to be greatly effective. A deeper study must be done in order to increase effectiveness of occupational therapy methods such as the NDT(treatment to improve normal movement sensation) and SI method(treatment to increase sensitivity of perceptive movement and to give the ability to position objects according to shape and location).

Clinical analysis was performed on 83 ankle fractures admitted and treated at the Orthopedic Service of the 17th Army Hospital during the period from April 1966 to April 1969. 1) Fourty five cases in the right ankle and the remainder in the left. 2) Closed reduction followed by adequate external immobilization was applied in 49 cases and the other 34 cases treated by open reduction included 20 cases in which initial manual reduction was failed. 3) Results of treatment in 33 cases could be evaluated as satisfying, in which reduction followed by short leg or patella setting below knee cast was adopted. 4) Average healing period was 2 to 3 months and in 5 cases it required more than 5 months. 5) Comminuted compression fracture and those with extensive soft tissue damage posed most difficult problems with ultimate poor functional restoration. In 8 such cases it was obligatory to apply a surgical fusion from the start of treatment.

We report a case of adult granulosa cell tumor arising in the scrotal tunics. The patient was a 34-year-old man who presented with right scrotal swelling, first noticed four months previously. Under the initial clinical impression of epididymo-orchitis, antibiotic treatment was instituted but there was no response. The paratesticular nodules revealed by ultrasound and magnetic resonance imaging mimicked intratesticular lesion, and radical orchiectomy was performed. Although several cases of adult testicular granulosa cell tumor, have been reported, the occurrence of this entity in the paratesticular area has not, as far as we are aware, been previously described.

BACKGROUND: The aim of this study was to assess demographics, clinical outcomes, and complications of classic floating elbow in adults. METHODS: Six patients with ipsilateral diaphyseal fractures of the humerus, radius, and ulna were reviewed retrospectively. All patients were treated operatively and available for follow-up at a minimum of 1 year after surgery. The average age of the patients was 45.2 years (22-64 years) and the average follow-up period was 37.0 months (14-103 months). They were evaluated with postoperative outcome measures, including a visual analog scale (VAS) for pain, Mayo elbow performance score (MEPS), and American Shoulder and Elbow Surgeons (ASES) shoulder score. Residual complications were also evaluated. RESULTS: Five patients (83.3%) had open fracture, and 4 patients (66.7%) presented with associated nerve injury. All fractures were united within postoperative 4 months, except 1 delayed union. The average VAS pain score, MEPS, and ASES shoulder score at the final follow-up examination was 2.5, 79.8, and 67.5 respectively. Three patients including 2 cases of joint stiffness with incomplete recovery from nerve injury and 1 case of complex regional pain syndrome had poor clinical outcome. CONCLUSIONS: Although the classic floating elbow is rare, these injuries potentially have associated problems such as open fracture or nerve injury. The presence of residual neurological symptoms predispose to poorer clinical outcomes.

The purpose of this study were to label nursing intervention activities performed by Korean nurses working in medical-surgical units by using Nursing Intervention Classification, and to compare nursing intervention classification in Korea with that in Iowa, and to link nursing interventions to nursing diagnoses. This research was done in two steps. At the first step, list nursing intervention activities were used by korean nurses working in medical-surgical units and categorize and label by using Nursing Intervention Classification. The 142 nursing interventions were identified. The nursing interventions had a three-tiered taxonomic structure composed 6 domains, 27 classes, and 142 nursing interventions. At the second step, 142 interventions were linked to 57 nursing diagnoses and validated by the following 27 experts: 6 nursing professors, 11 graduate nursing students who have specialty of medical-surgical nursing, 10 head nurses who have clinical experiences over 10 years. The method of expert survey used to established the content validity of linking nursing diagnoses and nursing interventions. Questionnaire was distributed to experts. In questionnaire, the intervention labels for each of 57 nursing diagnosis were listed. Exports were asked to rate each diagnosis according to the extent to which it was validated and to identify any interventions they believed needed to be deleted or added. The analysis of results indicated that the taxonomy was well developed. More than 80% of the respondents rates s wither very validate of validate on 44 nursing diagnoses. Only 13 diagnoses were rated as validate by less than 80% of respondents. Based on validity results, revisions were made by adding 60 interventions and deleting 1 intervention. Therefore, total 201 of nursing interventions for 57 nursing diagnoses were identified. The use of standardized classifications in the areas of nursing diagnoses and interventions facilitates communication and allow us to build large computerized databases which, in turn, through their accessibility will facilitate research and clinical decision making. A common nursing language will unit nurses and make nursing care an identifiable essence of health care. Also this study adds to the enhance quality of nursing care and build a unique body of knowledge in nursing.
