3.A Case of Vogt-Koyanagi-Harada-like Syndrome Associated with Immunotherapy (Pembrolizumab)

Purpose:
To report a case of Vogt-Koyanagi-Harada-like syndrome that occurred during immunotherapy (pembrolizumab) in a patient with non-small-cell lung cancer (NSCLC).Case summary: A 64-year-old female visited our clinic complaining of decreased visual acuity in both eyes. The patient had NSCLC and had been prescribed immunotherapy (pembrolizumab) by the Department of Pulmonology. Fundus examination and optical coherence tomography revealed multiple intraretinal cysts and subretinal fluid in both eyes. After evaluation using fluorescein and indocyanine green angiography, Vogt-Koyanagi-Harada (VKH) disease was diagnosed in both eyes and oral prednisolone 40 mg/day was prescribed for 2 weeks. At an outpatient visit at that time, the symptoms had improved, and fundus examination and optical coherence tomography revealed decreases in the number of intraretinal cysts and the subretinal fluid level. Oral prednisolone was reduced to 20 mg/day for 2 weeks. At an outpatient visit at that time, the symptoms had further improved; no cysts or subretinal fluid were apparent on fundus examination or optical coherence tomography, and only a small amount of subretinal fluid was present near the macula of the right eye.
Conclusions
VKH disease may develop during immunotherapy (pembrolizumab) but can be rapidly diagnosed and treated with oral steroids.
4.Acute Gastric Volvulus due to Diaphragmatic Eventration.

We present the case report of a 72-year-old female in whom diaphragmatic eventration and secondary gastric volvulus developed 10 years after a left partial pneumonectomy for a bronchiectasis. Eventration of the diaphragm is defined as an abnormal elevation of an intact diaphragm. The abnormally wide subdiaphragmatic space provides the potential for a gastric volvulus, which results from the strong negative intrathoracic pulling force created by the potential subphrenic space and paradoxical movement of the diaphragm. Unless this strong negative force is first eliminated, gastropexy alone will lead to recurrence. Obliteration of the subphrenic space by colonic displacement is a simple and effective way of abolishing this negative subdiaphragmatic pulling force. Treatment of gastric volvulus requires immediate surgical repair to prevent subsequent necrosis and perforation, with surgical correction of the underlying anatomic abnormality being the treatment of choice for gastric volvulus. We experienced a case of gastric volvulus due to diaphragmatic eventration who was treated with colonic displacement.

7.A Case of Generalized Granuloma Annulare with Perforating and Subcutaneous Granuloma Annulare.

We present a case of generalized grannuloma annulare with perforating and subcutaneous forms which occurred in a 15-year-old girl. Two years ago, multiple umbilicated, match head to pea sized, erythematous papules were noted on the upper extremities. Then the patient also experienced crusted or ulcerated erythematous papular lesions on the lower legs. Recently, tender, subcutaneous nodules appeared on both soles. The biopsy specimens from the skin lesions of the hand and the sole showed typical findings of granuloma annulare in the dermis and the subcutis, respectively. On the histopathological examination of the calf lesion, palisading granuloma as well as transepidermally eliminating dermal collagenous and necrotic material were seen. The skin lesions were almost completely improved by treatment with dapsone, 100mg a day, for 8 weeks orally.

8.A Case of kerion Celsi in An adult Caused by Trichophyton rubrum.

Kerion celsi caused by Trichophyton(T.) rubrum is rare endemically and there has been no report of it in Korean literatures. We report a case of karion celsi caused by T. rubrum in a 71-year-old male, who showed anindurated, swollen, boggy mass exuding pus on the right parietal scalp for about 7 weeks. The colonies and microconidia were identified as T. rubrum by fungus culture in potato dextrose agar. After administration of itraconazole for 11 weeks, the lesion was healed and the regrowing of new hairs was observed.

9.Change of Serum IGF-l level in Knee Osteoarthritis.

BACKGROUND: To examine serum Insulin-like growth factor-I(IGF-I) level in patients with knee osteoarthritis(OA) and to determine the relationship between the serum level of IGF-I with pain intensity and severity of OA knee. METHODS: Serum IGF-I levels measured by standard radioimmunoassay techniques were obtained from 30 knee OA patients and 30 normal controls matched by age and gender. Pain intensity was measured by pain rating scale(PRS) and visual analogue scale (VAS). Kellgren grade also used for assessing the severity of knee OA. RESULTS: No difference was found in serum IGF-I levels between the cases and controls(cases: 279+/-128 ng/dL, controls: 282+/-113 ng/dl). There was no correlation between pain intensity and serum IGF-I levels. There is no significant association between IGF-I level and Kellgren grade. CONCLUSION: Serum IGF-I level does not decrease in knee OA. The serum IGF-I levels can not indicate pain intensity and severity of knee OA.

10.Clinical and Mycologic Studies of Dermatophytosis in Chungchong Province.

OBJECTIVE: The study was made in order to inverstigate the clanical and mycological status of dermatophytosis in Chungchong province, incidence and friquency of disease, sex and month distribution of the disease, varieties of the causative organism. METHODS: We performed clinical and mycological studies of 86 cases of dermatophytosis which have showed positive on KOH examination and/or positved result in culture among out- patients of Dermatologic Clinics of Chungnam National Univesy during 7 years, from 1986 to 1993. RESULTS & CONCLUSION: 1. The ratio of male to female patients 330: 256 and make little ifference. 2. The incidence rate was bighest in the third decade(25.4%) 3. The seasonal prevalence of dermatophytosis was highest in the summer 4. The incidence of dermaiophytosis was T. pedis(30.2%), T. (22.9%), T. ungium(13.1%), T. corPoris(10.1%), T. capitis(9.0%) in decreasing order of freqercy 5. A mean positive rate for the KOH mount examination A. 5. 83.8%, T. 90.2%, T. corporis 89.8%, T. ungium 86.5% in decreasing order of frequency 6. A mean positive rate of culture was 77.1%; T. capities 94.3, . faciale 87.0%, T. cruris 84.3 %, T. cpor6 78.0% in decreasing order of frequency 7. T. rubrum was the mot common causative organism of cernatophytosis(69.5%), with T. mentagroPhyte(16.1%), M. canis(13.5%), M floccosum(0.7%), 4 gypseam(0.2%) presenting in decreasing order of frequency
