Experiments were conducted to define the optimal constituents of culture medium and atmospheric condition for growth of Campylobacter pylori. Two clinical isolates were streaked onto various media, incubated in two different atmospheric conditions (microaerophilic condition and carbon dioxide incubator), and growth was assessed semiquantitatively according to relative colony size and extent of growth through the streak. The growth obtained on Campy media, composed of GC agar base plus 1% hemoglobin, 0.2% activated charcoal, 1% IsoVitaleX, vancomycin 6mg /L nalidixic acid 20mg/L and amphotercin 2 mg/L, was used as reference. Our conclusions were as follows: Tryptic soy agar base was not acceptable for the growth of C. pylori. The organism grew in both atmospheric conditions, but generally showed a scantier growth in the carbon dioxide incubator than under the microaerophilic condition, however GC agar containing 1% hemoglobin and 0.2% activated charcoal supported well the growth of C. pylori in the carbon dioxide incubator. The authors have found that the GC agar base supplemented with 1% hemoglobin and 0.2% charcoal was the most satisfactory medium and a microaerophilic condition was optimal atmospheric condition for the growth of Campylobacter pylori in this study.

A thymoma often occurs in patients with myasthenia gravis, but the development of multiple thymoma is very rare. The authors report the radiologic and pathologic findings of multiple invasive thymoma in a 59-year-old male with myasthenia gravis.

PURPOSE: To describe the CT and clinical features of tuberculosis involving the right middle lobe of the lung. MATERIALS AND METHODS: Among patients diagnosed with pulmonary tuberculosis at our hospital during the past three years, 16 cases (mean age of patients: 72 years) were reviewed for radiological and clinical presentation of patients that underwent CT and chest radiography and showed mainly right middle lobe involvement. RESULTS: Middle lobe collapse or consolidation (n=16) and bronchial stenosis or obstruction without the presence of soft tissue masses (n=15) were the main findings. Enlarged mediastinal or hilar lymph nodes (n=15), cavities within consolidated tissue (n=2), ill-defined centrilobular nodules (n=12), a tree-in-bud appearance (n=10), focal consolidations (n=7) and small nodules (n=4) were found. All patients were older than 64 years and most complained of non-specific symptoms. A sputum smear for AFB was positive in four cases. CONCLUSION: A diagnosis of tuberculosis in the right middle lobe is suggested in older patients with following CT findings: 1) middle lobe collapse or consolidation; 2) middle lobe bronchus stenosis or obstruction without the presence of soft tissue masses; 3) mediastinal or hilar lymphadenopathy; 4) cavities within consolidation, and centrilobular nodules with branching linear structure in the adjacent lungs. Further evaluation such as bronchoscopy is recommended for confirmation even when the sputum smear for AFB is negative.

A study to evaluate the diagnostic significance of M. pneumoniae Infection by measurements of cold agglutinin and antimycoplasma antibody titers is performed with 191 pediatric patients who have visited Yeungnam University Hospital during the period through January to July, 1987. Forty eight of 191 cases made follow up tests feasible. The results obtained are as follows: 1. It is necessary to perform routine combined measurements of cold agglutinin and antimycoplasma antibody titers for the all pediatric pneumonia caser since a large proportion of pneumonia in children is caused by M. pneumonia. 2. For the diagnosis of M. pneumoniae Infection, measurements of cold agglutinin titer alone seems to be less significant than to check both cold agglutinin and antimycoplasma antibody titers. 3. The measurement of antimycoplasma antibody titer appeared to be more specific than cold agglutinin test in the diagnosis of M. pneumoniae Infection. 4. The present study urges the necessity of follow up study of cold agglutinin and antimycoplasma antibody titer for those who initially presented with normal titers in both tests, but are clinically suspected for M. pneumoniae Infection.

Body fluid Lactate dehydrogenase and its isoenzyme Measurement was performed in 132 patients: 8 cases with peritonitis, 21 cases with malignant ascites, 43 cases with liver cirrhosis, 48 cases with tuberculous pleuritis, 12 cases with malignant pleural effusion respectively. Body fluid protein and glucose contents, red blood cell counts, white blood cell counts, cytologic examination were also performed as a comparative study. The results were as follows: 1. Measurement of total LD and protein amount could differentiate between transudate and exudates in the ascitic fluids. 2. In the malignant exudate of ascites and pleural fluid, the activity of LD2 isoenzyme was statistically increased compared with that of inflammatory exudates and the activity of LD4 isoenzyme was also incereased compared with that of serum (P<0.05). 3. The inflammatory exudates of pleural fluid and ascites demonstrated the increase of LD5 isoenzyme activity statistically compared with that of serum and malignant exudates (P<0.05). 4. A difference of total LD activity between malignant ascites and inflammatory ascites was significant statistically, while this was not observed in the pleural exudate. 5. Total LD and LD5 isoenzyme activity didn't correlated with the number of white blood cells in the exudate.

Antimicrobial susceptibility of the bacterial strains isolated from clinical specimens during the period from June, 1983 to June, 1986 in Yeungnam Medical Center was studied and the following results were obtained. 1. Staphylococcus aureus was highly susceptible to cephalothin and its susceptibility to methicillin was gradually reduced. 2. Streptococcus strains except enterococcus were generally susceptible to penicillin, while most enterococci were susceptible to only ampicillin. 3. Gram-negative rods including Escherichia coli were highly susceptible to amikacin and tobramycin. 4. Serratia were generally less susceptible to the amtimicrobials tested than other Enterobacteriaceae. Among them, Serratia marcescens showed the highest susceptibility to amikacin and chloramphenicol. 5. Pseudomonas aeruginosa revealed the highest susceptibility to amikacin and tobramycin and moderate susceptibility to carbenicillin and gentamycin. 6. Acinetobacter calcoaceticus revealed low susceptibility to most antimicrobials tested, showing only 30% susceptibility to amikacin, tobramycin and gentamycin in 1986.

PURPOSE: To determine the relation between clinical results and correction angle in proximal tibial osteotomy through the retrospective study. MATERIALS AND METHODS: Seventy-two valgus osteotomies of the tibia were performed in forty-nine patients from 1985 to 1992; average follow-up period was 7 year 11 months(range, 5 to 12 years). We classi- fied the patients into three groups according to the tibiofemoral angle measured at the last follow-up. Group I includes patients with less than valgus 6 degrees of tibiofemoral angle, Group II with from valgus 7 degrees to 9 degrees and Group III with more than valgus 10 degrees. RESULTS: The Insalls Hospital for Special Surgery knee score was gradually decreased in time as 95 at postoperative 2 years, 92 at 5 years and 82 at the last follow-up. The average preoperative tibiofemoral angle was varus 3.4 degrees and the average postoperative tibiofemoral angle was corrected as valgus 8.0 degrees, 7.9 degrees at two years, valgus 7.0 degrees at five years and valgus 5.4 degrees at the last follow-up. The group I, 40 cases(mean 9 year 7 month follow-up), showed postoperative valgus 6.4(3-15) degrees and val- gus 2.6 degrees at the last follow-up. Loss of correction was 3.8 degrees and knee score was 76 at the last fol- low-up. The group II, 21 cases(8 year 1 month), showed postoperative valgus 9.0(7-11) degrees and 7.6 degrees at the last follow-up. Loss of correction was 1.4 degrees and knee score was 83.5 at the last follow- up. The group III, 11 cases(7 year 7 month) showed postoperative valgus 11.4(10-15) degrees and 10.9 degrees at the last follow-up. Loss of correction was 0.5 degrees and knee score was 97 at the last follow-up. The group III showed better maintenance of postoperative correction angle and clinical results than the other gf OUpS. CONCLUSION: Deductively, it is essential to correct tibiofemoral angle more than 7, especially ranged from 10 to 15 degrees for the best long-term clinical results.

Successful repair of lacerated tendons must restore continuity of the tendon and should yield a strong tenorrhaphy. Mechanical strength of repair should be adequate to early postoperative motion and mobility, The optimal repair technique must be able to withstand the rigors of early motion and also must not interfere with tendon healing. The relative strength of three suture methods of lacerated tendon were measured by mechanical disruption in effort to determine the strength of suture technique. Fifty-four Calcaneal tendons of 27 the New Zealand white rabbit were transected at mid portion and repaired with the three suture technique: group 1, Kessler suture, group 2, Pennington’s modified-Kessler suture and group 3, augmented- Becker suture technique. Each group was composed of 18 calcaneal tendons. Nine rabbits were sacrified immediately after suture, nine in postoperative 2 weeks and nine in postoperative 4 weeks Six calcaneal tendons in each three experimental group were obtained immediately after suture, at postoperative 2 and 4 weeks respectively. Tensile strength, maximum strength and modulus of elasticity of all experimental specimens were measured with Instron-UTM-4-100(Toyo-Baldiwin, Japan). The results were evaluated statistically to compare the strength of the three suture technique at three different periods. The tensile strength was predominantly strongest in augmented-Becker method among three suture technique at immediate suture, postoperative 2 weeks and 4 weeks respectively. The augmented Becker repair was strongest in maximum stress among Kessler and modified Kessler repair at immediate operation, postoperative 2 weeks and 4 weeks respectively. The augmented Becker repair was highest in modulus of elasticity than Kessler method and modified-Kessler method at immediate operation, postoperative 2 weeks and postoperative 4 weeks respectively. Tensile strength, maximum stress and modulus of elasticity were gradually increased from immediate operation to postoperative 4 weeks, but there were not statistically significance between experimental three suture methods at postoperative 4 weeks.

PURPOSE: To compare the accuracy of computed tomography (CT) and ultrasound (US) in the diagnosis of bezoars in small bowel obstruction. MATERIALS AND METHODS: During a recent eight-year period, 23 patients underwent surgery due to small bowel obstruction caused by bezoars, and 14 who underwent both CT and US were included in this study. The scans obtained were retrospectively reviewed by two radiologists working independently, and the usefulness of each modality in terms of diagnostic capacity, determination of the level of obstruction and detection of additional bezoars was assessed. RESULTS: At surgery, a total of 22 bezoars were removed: 19 from the small intestine and three from the stomach; multiple bezoars were found in five of the 14 patients. The presence of bezoars was demonstrated by CT in all patients, and by US in ten (71%) (p=0.125). The level of obstruction was correctly predicted in 57% of cases at US and in 100% at CT (p=0.03). Overall sensitivity was 55% for US (12/22) and 96% for CT (21/22) (p<0.01). In three patients with associated gastric bezoars, US revealed only one, whereas all were demonstrated by CT. In five with multiple lesions, only five (39%) of 13 were apparent at US, but 12 (92%) were revealed by CT. CONCLUSION: Both US and CT were useful for the diagnosis of small bowel obstruction caused by bezoars. CT however, was more accurate in determining the level of obstruction and in revealing additional bezoars in the small bowel and stomach.
