1.Development of Korean Version of Quality of Life Questionnaire in Patients with Erectile Dysfunction.

PURPOSE: To develop a quality of life questionnaire in erectile dysfunction patients culturally adapted to Korea. MATERIALS AND METHODS: Final Korean version was developed by 1st, 2nd and 3rd intermediary Korean version. 1st intermediately Korean version was made by forward translation, meeting and reconcilation by feedback from Mapi research institute. 2ndary intermediary Korean version was made by backward translation, meeting and reconcilation by feedback from Mapi research institute. 3rd intermediary Korean version was made by patient by international harmonization, meeting and reconciliation by feedback from Mapi research institute. RESULTS: Patient instruction, quertion items from 1 to 22, partner questionnaire, global assessment question, and event log were translated into Korean both conceptually equivalent to the original and easily understood by the people through 1st, 2nd, 3rd intermediary and final Korean version. CONCLUSION: Korean version of quality of life questionnaire in erectile dysfunction patients was developed.

2.The Effects of Part-Time Occlusion Therapy Shortened to One or Two Hours in Intermittent Exotropia.

PURPOSE: To evaluate the compliance level of therapy and the change in exo-angle after 1 or 2 hours of part-time occlusion therapy in intermittent exotropia. METHODS: This retrospective study included 97 patients with intermittent exotropia who were followed-up for more than 6 months after their initial visit. We evaluated the exo-angle and prescribed either 1 hour or 2 hours of part-time occlusion therapy for the fixating eye. After 6 months, the exo-angle was reevaluated and compliance was examined. Patients were divided into 2 groups according to the occlusion therapy time: the 1-hour patching group (33 patients) and the 2-hour patching group (64 patients). Compliance and deviation angle were retrospectively reviewed and compared between the 2 groups. We evaluated the exo-angle of patients with compliance over 50% (1-hour patching group (31 patients), 2-hour patching group (51 patients)). RESULTS: The mean exo-angles on the first visit were 22.61 +/- 5.78 PD (prism diopters) at distance and 22.88 +/- 8.11 PD at close-range in the 1-hour patching group, and 26.17 +/- 4.55 PD at distance and 25.27 +/- 8.54 PD at close-range in the 2-hour patching group. The proportion of patients with compliance over 50% was higher in the 1-hour patching group (93.94%) than in the 2-hour patching group (79.69%, p = 0.013). There were significant decreases in distant and close- range deviation angles after part-time occlusion in patients with compliance over 50% in both groups (1-hour patching group distant p = 0.042, close-range p = 0.002; 2-hour patching group distant p < 0.001, close-range p < 0.001). The magnitude of deviation angle decrease was not statistically significant between the 2 groups (ANCOVA; distant p = 0.461, near p = 0.761). CONCLUSIONS: One or 2 hours of part-time occlusion therapy in intermittent exotropia provided beneficial effects, including decreasing the deviation angle. The magnitude of deviation angle decrease, however, was not different between the 2 times of therapy. Compliance was higher in the 1-hour part-time occlusion therapy group. Therefore, 1-hour part-time occlusion therapy could be effective in patients with poor cooperation.

Congenital self-healing reticulohistiocytosis (CSHRH) is a rare Langerhans cell disorder showing spontaneous resolution within 3-4 months. By electron microscopy, the identification of many Birbeck granules and laminated dense bodies in the infiltrated cells is mandatory for the diagnosis of CSHRH. We describe a case of congenital self-healing reticulohistiocyt~osis in a 4-month-old male infant.

5.Job's Syndrome: Suspected Case.

Job's syndrome is an inflammatory skin disease characterized by (1) severe eczema, (2) recurrent staphylococcal infections of the skin and sinopulmonary tract, (3) cold subcutaneous abscesses, and (4) high serum IgE levels. We describe a 55-year-old woman with long-standing atopic dermatitis-like eczema, recurrent abscesses, and a high level of serum IgE. We suspected this case as Job's syndrome.

6.Annual Report of the Central Cancer Registry in Korea-1998 (Based on Registered Data from 124 Hospitals).

PURPOSE: Central Cancer Registry Center in Korea conducted a nation-wide hospital-based cancer registry to provide the basic data on cancer statistics. MATERIALS AND METHODS: In 1998, 124 hospitals participated in the cancer registry program. All cancer registry data, submitted from the participating hospitals by diskettes during the year, were reviewed and sorted out by the committee members who were all board-qualified clinical oncologists and pathologists. To avoid duplication, every resident registration numbers were compared by the computer. Cases diagnosed by histologic examination were preferentially chosen. RESULTS: Of 89,226 cases registered, 9,163 (10.3%) duplication cases were excluded. Of the remaining 80,063 cases, 3,195 cases (4.0%) of carcinoma in situ (morphology code /2) were excluded. Finally 76,868 cases were analyzed. Of the analyzed cases, 44,037 (57.3%) were male and 32,831 (42.7%) were female. The leading age groups in the order of relative frequency were 60~64 years of age (15.3%), followed by 55~59 (14.4%). The leading primary cancer sites in the order of relative frequency were stomach (20.9%), followed by liver and intrahepatic bile ducts (12.2%), bronchus and lung (11.9%), colorectum (9.6%), breast (6.1%). In male, the leading primary cancer sites were stomach (24.4%), followed by liver and intrahepatic bile ducts (16.4%), bronchus and lung(16.0%), colorectum(9.2%) and urinary bladder (3.5%). In female, stomach (16.3%) was the most common site, followed by breast (14.1%), uterine cervix (13.0%), colorectum (10.1%) and liver and intrahepatic bile ducts (6.5%). Among the 1,190 cases of childhood malignancies, leukemia (33.4%), CNS tumor (15.7%) and sympathetic nervous system tumor (8.4%) were common. CONCLUSION: We analyzed and reported the registered cancer data from 124 hospitals during 1998.

7.MRI Findings of Intracranial Neurenteric Cyst: Cast Report.

Neurenteric cyst is very rare congenital disease and usually presents symptoms in adulthood. It can cause major neurological deficits if detection and treatment are delayed. It is also known to be a source of serious infection of the central nervous system. Neurenteric cyst is usually located in the ventral aspect of the lower cervical or upper thoracic spinal cord. Intracranial neurenteric cyst is very uncommon and less than 35 cases have been reported in the literature. We report MRI findings of two cases of intracranial neurenteric cyst and review the literature.

8.Muscular Injury of Iliacus and Gluteus Maximus after Mountain Climbing.

Iliacus muscle is the strongest hip flexor of gluteal region that acts with psoas muscle, whereas gluteus maximus muscle is the largest muscle engaged in extension and external rotation of the hip. Mountaineering requires strong contractile force of both flexor and extensor around the hip. A 57-year-old man presented to our hospital with severe pain in left groin after mountaineering for 5 hours without a break. Magnetic resonance imaging revealed incomplete rupture of iliacus muscle and strain of gluteus maximus muscle. Conservative treatment was done. At 3 months of follow-up, he returned to normal life. If we are going to climb mountain, it is important to start warming up with some stretches, take a break while climbing, and use climbing sticks. There have been no report about athletic injury of both iliacus and gluteus maximus after mountaineering. So we report this case with a review of the literature.

9.Lifestyle and Genetic Predictors of Stiffness Index in Community-dwelling Elderly Korean Men and Women.

PURPOSE: Differing lifestyle, nutritional, and genetic factors may lead to a differing stiffness index (SI) determined by quantitative ultrasound in elderly men and women. The purpose of this study was to determine SI and the gender-specific factors associated with low SI in a Korean elderly cohort. METHODS: This was a cross-sectional descriptive study identifying the gender-specific factors related to SI in 252 men and women aged 65 years and greater from local senior centers in Seoul, Korea between January and February 2009. RESULTS: The mean SI of elderly men was significantly higher than that of the women's. A multiple regression analysis reveals that age, nutritional status, and physical activity were predictive factors of lower SI in men, whereas age, alcohol consumption, educational level, and genetic polymorphism were predictive factors for elderly women. CONCLUSIONS: Low SI was common in both elderly men and women. We found gender differences in factors linked to low SI. In multiple regression analysis, nutritional status and physical activity were more important factors in men, whereas alcohol consumption, educational level, and genetic polymorphism were significant factors predicting low SI in women. Gender-specific modifiable risk factors associated with low SI should be considered when developing osteoporosis prevention programs for the elderly.

10.Pseudogout of the Knee after Intraarticular Injection of Hyaluronic Acid.

An 82-year-old woman underwent surgery for the left intertrochanteric fracture. However, during hospitalization, she complained of pain on the right knee. Radiographs showed moderately degenerative arthritis and intrameniscal calcification. Hyaluronic acid was administered by intraarticular injection. After injection, arthritic symptoms improved. Five days after injection, she complained of severe pain, swelling, and heating sensation in the right knee with chills and fever. Blood test showed elevated white blood cell (WBC) count and C-reactive protein. WBC in synovial fluid increased to 47,250/mm3. Antimicrobial therapy was administered under the impression of acute pyogenic arthritis by adverse reaction of hyaluronic acid injection. Polarization microscope showed calcium pyrophosphate dihydrate crystals. Synovial fluid culture was negative. Finally, she was diagnosed as pseudogout. The symptoms improved within one week. To our knowledge, there has been no report of pseudogout following intraarticular injection of hyaluronic acid in Korea. Therefore, we report this case with a review of the literature.
