1.The Evaluation of Capsule Contraction Syndrome.

As increasing demands for CCC (Continuous Curvilinear Capsulorhexis) fincreases, the incidence of capsule contraction syndrome is also increasing in cataract surgery. We tried to classify and treat this syndrome with 44 eyes who have capsule contraction. It is known that the basic mechanism of capsule contraction is fibrotic dysplasia of anterior capsule, and we found that capsular contraction can arise with glaucoma, diabetes mellitus, uveitis, retinitis pigmentosa, psuedoexfoliation syndrome and old aged group as ordered. It can also be frequently can be found in age 50 to 60. In order to decrease the complication of capsule contraction(IOL dislocation, retinal detachment, etc.), we incised the anterial capsule with Nd:Yag laser at the time of fibrotic change. For the case of weak zonules, we made a radial incision in the 12 o'clock direction after IOL insertion.

2.Type III Hyperlipoproteinemia.

We report a case of type III hyperlipoproteinemia which is called a broad-beta disease. A 53 year old female patient visited our clinic for the evaluation of multiple yellowish papules on extremities and eyelids. The patient showed various types of xanthoma includiiig eruptive, tuberous, tendinous xanthomas and xanthelasma palpebrarum, xanthoma striatum palmare. The blood chemistry revealed a marked elevstion of cholesterol and triglycerides and agarose gel electrophoresis showed a single peak at prebeta and beta portion without separation. On histopathologic studies, typical foam cells were showen.

4.A Case of Epidermal Nevus Syndrome.

We report a case of epidermal nevus syndrome. The patient was a 26-year-old female who had numerous linear verrucous plaques on her neck, upper back and anterior chest. Biopsy of these lesions revealed epidermal hyperkeratosis, acanthosis and papillomatosis. Variable sized cafe-au-lait spots were scattered around the verrucous nevi. The other associated findings were claw hand deformity and epilepsy. Electroencephalogram showed very irregular, random and slow waves confined to the right temporooccipital area.

6.Comparison of the optical properties of pre-colored dental monolithic zirconia ceramics sintered in a conventional furnace versus a microwave oven.

PURPOSE: The purpose of this study was to compare the optical properties of pre-colored dental monolithic zirconia ceramics of various thicknesses sintered in a microwave and those in a conventional furnace. MATERIALS AND METHODS: A2-shade of pre-colored monolithic zirconia ceramic specimens (22.0 mm × 22.0 mm) in 3 thickness groups of 0.5, 1.0, and 1.5 mm were divided into 2 subgroups according to the sintering methods (n=9): microwave and conventional sintering. A spectrophotometer was used to obtain CIELab color coordinates, and translucency parameters and CIEDE2000 color differences (ΔE 00) were measured. The relative amount of monoclinic phase (X(m)) was estimated with x-ray diffraction. The surface topography was analyzed by atomic force microscope and scanning electron microscope. Statistical analyses were conducted with two-way ANOVA (α=.05). RESULTS: There were small interaction effects on CIE L*, a*, and TP between sintering method and thickness (P<.001): L* (partial eta squared η(p)²=0.115), a* (η(p)²=0.136), and TP (η(p)²=0.206), although higher b* values were noted for microwave sintering regardless of thickness. Color differences between two sintering methods ranged from 0.52 to 0.96 ΔE(00) units. The X(m) values ranged from 7.03% to 9.89% for conventional sintering, and from 7.31% to 9.17% for microwave sintering. The microwave-sintered specimen demonstrated a smoother surface and a more uniform grain structure compared to the conventionally-sintered specimen. CONCLUSION: With reduced processing time, microwave-sintered pre-colored dental monolithic zirconia ceramics can exhibit similar color perception and translucency to those by conventional sintering.

7.Prognostic Significance of Histologic Features, DNA Content, Expression of Proliferating Cell Nuclear Antigen (PCNA), c-fos Protein and Transforming Growth Factor (TGF)-alpha and -beta in Giant Cell Tumor of Bone.

PURPOSE: This study was attempted to investigate the prevalence of the expression of c-fos protein, TGF-alpha and -beta, PCNA , DNA ploidy pattern and histopathological parameters of giant cell tumor (GCT) of bone and to correlate with prognosis and to extend our understanding on tumorigenesis of GCT. MATERIALS AND METHODS: Twenty eight cases of paraffin-embedded tissue were studied, classified as recurrent (5 cases) and non-recurrent group (12cases) within the limits of the cases which afforded surgical material on first operation. RESULTS: No significant difference was observed in cellularity of stromal cells, atypia of stromal and giant cells, presence of hemorrhage and necrosis between recurrent and non-recurrent group. However, presence of more than 10 mitotic figures in 10 high power fields in recurrent group was significantly higher than non-recurrent group (p<0.05). The immunoreactivity for PCNA was seen only in nuclei of stromal cells, whereas nuclei of giant cells showed negative staining. The positivity of PCNA revealed no significant difference between non-recurrent (mean; 40.9%) and recurrent group (34.4%). The expression of c-fos oncogene was seen in 5 cases (100%) in recurrent group, and 8 cases (66.7%) in non-recurrent group, and no significant difference was seen. No significant difference of expression of TGF-alpha was seen in 5 cases (100%) in recurrent group and in 11 cases (91.7%) in non-recurrent group. The expression of TGF-beta in stromal cells was significantly higher in non-recurrent group (80%) compared to recurrent group (100%) (p<0.05). In DNA analysis out of 18 cases, 4 cases (22.2%) were aneuploidy and 14 cases (77.8%) were diploidy. Among 4 aneuploidy cases, 3 cases (75%) had no recurrence, and 1 case (25%) had metastasis to lung and expired. No significant difference of DNA ploidy pattern was seen between the recurrent and non-recurrent group. CONCLUSION: Presence of more than 10 mitotic figures in 10 high power fields and less expression of TGF-beta are related to higher possibility of recurrence and it is suggested that the number of mitotic figure (more than 10/10HPF) and expression of TGF-beta could be helpful parameters in predicting recurrence of GCT.

8.Clinical Study of 25 Cases of Erythema Dyschromicum Perstans.

We studied 25 cases of clinically diagnosed erythema dyschrornicum perstans (EDP) who were seen in our clinic within 11-year period, from 1975 through 1985, and compared light and electron microscopic findings, and direct immunoflunrescence findings. The results are summarized as follows: 1. There were 16 males(64%) and 9 females(36%). The peak age incidence was in the second decade. 2. The mean age of onset was 18 years(M: 16. 6, F: 19. 4) and the rnean duration was 1.8 years(M: 2.6, F: 1.0). 3. The annual incidence increased from 1981 and was peaked in 1985. 4. The most commonly involvement site was the trunk(92%), the least common involvement(4%) was one of the face, neck, and shoulder combined. 5. The histopathologic findings in most patients shows focal vacuolization of the basal cell layer, pigment incontinence, dermal melanophages. 6. Electron microscopic findings of five patients consist of vauoles with abundant melanosomes in the basal cells, widened intercellular spaces and dermal melanophages.

Intravascular papillary endothelial hyperplasia is a benign solitary turnor that may be histologically misdiagnosed as angiosarcoma. We present herein a case of intravascular papillary endothelial hyperplasia occuring in 40-year-old female who hsd a slowly growing tumor on the tongue for 3 years. the histopathologic findings revealed. papillary endothelial hyperplasia in blood vessel accompanied with thrombi.

10.A Case of Leiomysarcoma.

This paper presents a case of leiomyosarcoma which developed in the retroperitoneal cavity. On admission, this 4 months old male infant disclosed a adult fist sized mass with abdominal distention and scattered nevus lesions on the left sided abdomen on physical examination. IVU examination revealed urinary collecting system to be unremarkable, but right kidney and ureter were displaced to upward and posterior respectively. On 6th hospital day, we performed explorative laparotomy and removed the tumor mass measured 15X11cm in size. Histologic finding revealed leiomyosarcoma. Review of literature was also made breifly.
