2.Pemphigus Vulgaris ; Report of a Case.

This 51 years old Korean house wife has been suffered from bullous and erosive eruptions on the abdomen, back, both thigh and legs, duration of three days with sudden onset Prior to admission she has had a erosion on the oral mucosa for one year. The diagnosis of pemphigus vulgaris was confirmed by histopathology and immunoelectrophoresis. This patient was improved with treatment of prednisolone bu Lever's "Logarithmic fashion" and used the maintenance dose of prednisolone without recurrence.

3.An Analysis on Factors Relating to Fiscal Deficit for Regional Health Insurance Program in Korea.

This study was designed to investigate factors relating to fiscal deficit for regional health insurance. The financial statements for the fiscal year 1990 of nationwide 254 regional medical insurance societies were analyzed. Important findings are summarized below: 1. There were differences in the main reason for the financial deficit among regions when deficit and surplus societies were compared by regions. The total revenue per enrollee, especially revenue from the premium contribution of a deficit society was significantly smaller than that of a surplus society in large cities and counties. On the other hand, the total expenditure per enrollee of a deficit society was larger than that of a surplus society in small cities. 2. Both low premium irate at the beginning of health insurance program and less effort to increase the premium rate were main factors for the smaller revenue from the contribution of a deficit society in large cities and counties. 3. Larger expenditures per covered person of a deficit society in small cities were explained with larger medical expenditures especially for out-patients services rather than larger administrative expenses. 4. A regression analysis showed that utilization rates in out-patient services were significantly associated with income and numbers of total medical care institution per capita within a region where a health insurance society located. Also expenses paid by insurer per visit were associated with the proportion of utilization for tertiary care hospitals as well as the proportion of utilization of public health centers.

4.Relationship between Metabolic Syndrome and Coronary Heart Disease in Elderly.

BACKGROUND: Metabolic syndrome, a concurrence of disturbed glucose and insulin metabolism, over- weight and abdominal fat distribution, dyslipidemia and hypertension, is associated with subsequent de- velopment of type 2 diabetes mellitus and cardiovascular disease, especially coronary heart disease. The aim of the study is to assess the relationship between metabolic syndrome and coronary heart disease in elderly greater than 65 years old. METHODS: Eighty two elderly patients greater than 65 years old who underwent coronary angiography were divided into two groups with metabolic syndrome or without metabolic syndrome, and assessed the association with coronary angiographic finding. The metabolic syndrome factors and cardiovascular risk factors of JNC 7 were investigated to assess the relationship with coronary heart disease in elderly. Coronary heart disease was defined as 50% or greater diameter in stenosis of coronary artery in coronary angiography. RESULTS: In elderly patients with metabolic syndrome, coronary angiographically abnormal findings(p<0.05) and multi vessel disease findings(p<0.05) were presented significantly higher than non metabolic syndrome patients. In elderly patients with 3 and more cardiovascular risk factors of JNC 7, coronary angiographically abnormal findings(p<0.05) and multi vessel disease findings(p=0.059) were presented more than the other patients. Diabetes mellitus was related significantly with coronary heart disease(p value 0.044). CONCLUSION: In elderly patients, metabolic syndrome was significantly related with coronary heart disease and diabetes mellitus had strong relationship with coronary heart disease. Metabolic syndrome and cardiovascular risk factors of JNC 7 should be further evaluated to assess the relationship with coronary heart disease in the future.

5.Relation Between Absoute Nucelated Red Blood Cell Count at Birth and Retinopathy of Prematurity.

PURPOSE: The aim of this study is to prove the association between potential fetal hypoxia and retinopathy of prematurity (ROP) development and absolute nucleated red blood cell (aNRBC) is used to evaluate it in premature infants without any hypoxic or ischemic history. METHODS: Medical records of 43 premature infants with ROP who were admitted to the neonatal intensive care unit at Wonkwang University Hospital from January 2004 to December 2014 were analyzed retrospectively. We excluded 15 infants who had a confounding medical condition that could have increased the aNRBC count. Finally, 28 premature infants affected by ROP were enrolled and compared with 28 pair-matched controls. The aNRBC counts at birth in these infants were compared. Statistical analysis was performed with a paired t-test for continuous data, and a Fisher's exact test for categorical data. P<0.05 was considered significant. RESULTS: There were no significant differences in perinatal characteristics such as gestational age (GA), birth weight (BWt), Apgar scores, premature rupture of membrane (>24 hours), prenatal betamethasone, surfactant or respiratory distress syndrome between the ROP and the control infants. In addition, neither group differed in major morbidities such as patent ductus arteriosus, periventricular leukomalacia, intraventricular hemorrhage (> or =Grade 2), or bronchopulmonary dysplasia. Regardless of the severity of ROP, the aNRBC counts at birth in premature infants with ROP were not higher than in the control infants. CONCLUSION: The aNRBC counts at birth may not be related directly to the development of ROP.

6.A Clinical Evaluation of Hypotensive Anesthesia for Maxillofacial Reconstruction and Plastic Sugery .

Deliberate hypotension reduces bleeding into a wound, thereby providing the surgeon with both better visilibility and technical freedom for a more definitive dissection and minimizes the need for blood transfusion, thereby reducing the risks of transfusion reasctions, hepatitis and acquired immunodeficiency syndrome (AIDS). In this study we evaluated the hypotensive anesthesia induced by sodium nitroprusside for maxillofacial reconstruction and plastic surgery performed at Seoul Paik Hospital from March 19S8 to February 1989: The results of our study were as follows: 1) Halothane and enflurane were used as the main anesthetics in 77 and 6 cases respectively. Nitroprusside was used for inducing hypotension and the mean amount infused was 10.5+/-7.7 mg. 2) The mean arterial pressure at 30min after inducing hypotension was 64.5+/-5.3mmHg. 3) The mean blood loss was 736.9+/-394.5ml and the mean amount of transfused blood was 497.1+/-451.1 ml. 4) The intraoperative arterial blood gas analysis shoueds no significant abnormality. 5) There were no significant perioperative and postoperative complications due to induced hypotension itself. In conclusion, the technique of using induced hypotension with inhalation anesthesia supplemented by nitroprusside under 10 head up position is relatively safe and useful one to use in performing maxillofacial reconstruction and plastic sugery.

7.Fibroblast Growth Factor 4(FGF4) Expression in Malignant Skin Cancers.

PURPOSE: FGF4(fibroblast growth factor 4) is a newly characterized gene which was found to be a transforming gene in several cancerous cells. FGF4 expression and amplification has been subsequently observed in several human cancers including stomach cancer, breast cancer, head and neck squamous cell carcinoma, lung cancer and bladder cancer. This study was designed to measure the protein expression of FGF4 in malignant skin cancers. METHODS: We examined 8 normal skin tissues and 24 malignant skin tumor tissues which were 8 malignant melanomas, 8 squamous cell carcinomas and 8 basal cell carcinomas. The specimens were analyzed for the protein expression of FGF4 using immunohistochemical staining. To evaluate the amount of expression of FGF4, the histochemical score(HSCORE) was used. RESULTS: FGF4 was expressed more intensely in malignant melanoma, followed by SCC and BCC in immunohistochemistry. The average HSCORE was 0.01 for normal skin, 2.02 for malignant melanoma, 1.28 for squamous cell carcinoma , and 0.27 for basal cell carcinoma, respectively. The expression of FGF4 in malignant melanoma and squamous cell carcinoma was increased in comparison with normal tissues and basal cell cancer, and the difference was statistically significant(p<0.05). The difference between malignant melanoma and squamous cell carcinoma was not statistically significant. CONCLUSION: These findings provide evidences that the expression of FGF4 plays an important role in malignant melanoma and squamous cell carcinoma progressions. This article demonstrates expression of FGF4 in human skin malignant tumors, and suggests that FGF4 is more expressed in highly aggressive skin tumors.

9.Prevalence and Risk Factors of Diabetic Retinopathy: In First Fundus Examined Diabetic Patients.

We performed a epidemiologic study to find a prevalence and risk factors of diabetic retinopathy. We reviewed 130 diabetic patients who had been performed the fundus examination for the first time for retinopathy at the ophthalmic department of Chung Ang university hospital from March 1993 to December 1993. We studied epidemiologic work-up sheet and performed ophthalmic and laboratory examination. The patients' ages ranged from 25 to 81 years, with an average age of 58.9 years. The age, sex, intraocular pressure, fasting blood sugar, post cibos 2 hours (Pc2hrs), age of onset, residence, smoking, alcohol drinking, diet pattern, blood type, treatment method and menstruation status were not significantly correlated with prevalence of diabetic retinopathy statistically(p>0.05). But the duration of diabetes(p<0.01), glycosylated hemoglobin(HbA1C) and cataract operation(p<0.05) were significantly correlated with diabetic retinopathy statisically. The prevalence of diabetic retinopathy was 28.5%(background changes in 21.5%, preproliferative changes in 4.6% and proliferative change in 2.4%). The mean interval between diagnosis of diabetes and the first examination of retinopathy was 6.62 years.
