1.Clinical Observation on Chronic Prostatitis.

A clinical observation was performed on 154 cases of chronic prostatitis in the Department of Urology, National Seoul Hospital from January 1981 to March 1982. The results are as follows: 1. 121 cases or 78.4 percent of the cases were observed among the ages between 20 to 40 years are more than one half of these cases were seen between the ages of 26 to 35 years. 2. Previous diseases were Non-gonococcal urethritis on 82 cases (53.2%), Gonococcal urethritis on 45 cases(29.2%) and previous prostatitis on 15 cases (9.7%). 12 cases (7.8%) were free of previous diseases. 3. Most frequently observed symptoms consisted of those of mild chronic urethritis such as morning drop, urethral discomfort, perineal discomfort and frequency. 4. In microscopic findings of wet smear of the prostatic secretion, 125 cases (81.2%) showed W.B.C. more than10/H.P,F. 29 cases (18.8%) were within normal limit. 5. In 125 cases in which prostatic gram stain was done, microorganisms were gram (+) cocci on 26 cases (20.8%),gram (-) bacilli on 12 cases (9.6%) and gram (+) bacilli on 10 cases (8.0%). No microorganism were on 77 cases (61.6%).

2.Singificance of Bone Scan in the Early Diagnosis of Acute Osteomyelitis

Bone imagings have played important role in early detection of acute osteomyelitis. In acute osteomylitis, bone scan findings precede the appearance of bone change on radiograph. Also, recent studies have reported accuracies of bone scan in diagnosis of osteomyelitis ranging from 84% to 100%. But, in above datas, bone scan reading were only qualitative studies. So we have reviewed Technetium-99m labelled methylen dipho sphonate(99mTc-MDP) scan on 21 patients who had been diagnosed acute osteomyelitis by clinical symptoms, blood culture, pus culture and operative findings from January, 1984 to September, 1985. The purpose of this study is to establish the quantitative study of bone scan using computed pixel counting method. The 21 cases were classified into group I and group II according to initial radiographic findihgs. The group I showed normal radiographic finding or soft tissue swelling without osseous change. The group II showed osseous change. The results obtained were as follows: 1. We obtained positive bone scans in the all cases which were confirmed acute osteomyelitis. 2. We experienced diagnosis of acute osteomyelitis by bone scan which was confirmed as early as 48 hours after onset of clinical symptom. 3, In group I, the average uptake ratio of 99mTc-MDP was 3.22±0.82 ranging from 2.20 to 4.80. 4. In group II, the average uptake ratio of 99mTc-MDT was 6.20±2.27 ranging from 4.00 to 9.40. (p <0.001).

5.A Case Report of Enchondroma on Right Ischjum

The Enchondroma is a benign cartilage forming harmartoma arising chiefly from short tubular bones on hands and feet. If on a long tubular bone or flat bone such as femur or ilium, it is most likely to be a true neoplasm such as chondrosarcoma, not a harmartoma. A case of enchondroma was reported. It occurred over right ischium of 19 years old male and was found incidentally by roentgenogram. The lesion was biopsied and diagnos's was confirmed by histologic examination. It was treated by curretage with filling of cavity with bone chips. Post operative-course was uneventful.

6.Determination of High Density Lipoprotein Cholesterol in Psoriasis Patients.

In orper to measure the levels of high density lipoprotein cholesterol in psoriasis, 60 psoriatic patients and 30 healthy subjects were included in this study. Lopez-Virells methos was applied for measuring the serum level of high density lipoprotein cholesterol. The results were obtained as follows. The level of serurn HDL cholesterol was 58. 39+17. 40 mg/dl in psoratcs and 50.3+0.31 mg/dl in healthy subjects and 50. 43+ 10. 31 mg/dl in healthy subjects. No significant differences were noted between psoriatics & healthy subjects. 2. The level of serum HDL cholesterol was 56. 40+19. 10 mg/dl in male group of psoriatics and 60. 00+15.47 mg/dl in female group of psoriatics and 48.3+9. 50 mg/dl in healthy male group and 52.60 -10.59 mg/dl in healthy female group. No significant differences of serum HDL cholesteol levels were noted in both sexes.3. The mean value of serum HDL cholesterol by age groups of 10, 20, 30, 40, and 50 years old with psoriasis were 50. 70 mg/dl, 61. 97 mg/dl, 57. 44 mg/dl, 49. 11 mg/dl and 70. 36 mg/dl, respectively and those of healthy groups were 57. 25 mg/ dl, 45. 17 rng/dl, 50. 97 rng/dl, 48. 07 mg/dl and 46. 98 mg/dl, respectively.

9.Immunohistochemical Identification of Keratin in Cutaneous Tumors.

Sixty cutaneous tumors of different types were investigated by means of immunoperoxidase technique(avidin-biotm-peroxidase complex method) with the use of antikeratin antibody. The results were as follows: 1. Squamous cell carcinomas, keratoacanthomas, and Bowen's disease exhibited strong reactivity with antikeratin antibody. 2. In squamous cell carcinomas and keratoacanthomas, the most heaviest staining patterns were detected in areas of keratinization and horn pearl formation. 3. Basal cell carcinomas and seborrheic keratosis were moderately stained, whereas Paget'a disease and metastatic carcinoma(oat cell carcinoma) did not contain keratin. 4. In some cases of basal cell carcinomas, the tumor cells were partially positive. 5. Malignant melanomas, nevi, malignant.lymphomas, neural tumors, hemangiomas, and dermatofibromas were uniformly negative.

10.Immunopathological studies in pemphigus vulgaris, bullous pemphigoid and epidermolysis bullosa acquisita.

Pemphgus vulgaris (PV), Bullus pemphigoid (BP), and Epidermolysis bullous acqusita (EBA) are autoimmune bullous dermatoses, characterized by circulating IgG autoantibodies. These antibodies react with antigens located at the intercellular substance (ICS) of epidermis, basement membrane zone (BMZ), and subepidermal anchoring fibril zone (AFZ), respectively. The subclass distribution of IgG autoantibodies, and the properties and degrees of complement fixing activities of these autoantibodies in each of the above diseases have not been well understood. Indirect immunofluorescence and in vitro complement stainings were performed for the titration of subclasses of IgG antibodies and for the immunofluorescence staining reactivities of complement components C3, C4, C5b-9, H, C4bp, and S. Each serum specimen from five cases of PV, five cases of BP. and three cases of EBA was tested. The findings of multistep technique with monoclonal and polyclonal antibodies are as follows : All four subclasses of IgG antibodies were identified at the antigenic sites in these group, however there were some differences in the antibodies titers. In PV and BP the dominant subclass of highest antibody titer was IgG1 and/or IgG4. In EBA only IgG4 was dominant in all three cases. The results of complement component stainings, in most of the cases of PV, showed positive for C3 and C4 but were negative for the other components or inhibitor proteins at the ICS of epidermis. In BP most of the cases revealed positive staining reactivities at the BMZ for C3, C4, C5b-9, H, and C4bp-9 with no staining reactivities for the inhibitor proteins No significant relevancy was found between the titers of complement fixing IgG subclasses and the numbers of positive complement staining reactivities for complement components. The results suggest that the complement system may contribute more strongly to the formation of bullous lesions in BP and EBA than in PV.
