1.Atypical Dermatoglyphics in Trisomy 18 (Edwards Syndrome).

A 19 month old girl with trisomy 18 is described. She showed loose folds of skin about the neck, a prominent occiput, a simian crease on both palms, epicanthal folds, acrocephaly, micrognathia, and unusual dermatologic features including total alopecia and no finger prints on either hands. Because of the simian crease on both palms, dermatoglyphics of both hands and total alopecia, a skin biopsy from the scalp and a chromosomal study were established at age 19-months, and an absence of hair follicles was observed, while peripheral blood lymphocytes demonstrated 47, XX, 4-18. To our knowledge, this would be the first recorded report on the dermatoglyphic pattern of Edwards syndrome in a Korean journal of dermatology.

3.A case of lymphangioma circumscriptum with cystic hygroma.

Lymphangiomas and cystic hygromas are congenital malformatiors of the lymphatic system. They are composed of lymph-containing endothelium-lined spaces which very in size from channels of capillary dimensions to cysts several centimeters in diameter. Both types of lesions are rarely present, in the same patients. We report herein a case of lymphangioma circumscriptum with cystic hygroma on the tongue and the right side of the neck in a 5 year-old boy. The patient had a grop of deep-seated, small, crystal-line vesicles resembling frogs spawn that contained a viscous liquid with hemorrhage in the tongue and a mobile non-tender cystic swelling in the right side of the neck.

4.CLINICAL EVALUATION ON THE MOUTH REHABILITATION USING DENTAL IMPLANTS.

The use of osseointegrated implants is an accepted procedure for the treatment of Total, or partial edentulism and offers good predictability of long-term success. Osseointegration implies a firm and direct interlocking between vital bone and screw-shaped titanium implants. There should be not to interposed tissue between fixture and bone. This study was undertaken to assess the clinical condition, complication, and prosthodontic aftercare of different implant systems. One hundred fifty-nine patients treated with a total of 503 endosseous implants (364 Steri-oss threaded type, 69 Integral cylinder with HA coated type, 35 Steri-oss threaded with HA coated type, 21 Steri-oss cylinder with HA coated type and 14 3i implant type), Most of the implant were placed in type B and C bone quantity and type 2 and 3 bone quality according to Lekhorm and Zarb. The success rate of Steri-oss threaded type during healing and function was 92%, Steri-oss threaded type with hydroxyapatite coated was 91%, Steri-oss cylinder type with hydroxyapatite coated was 90%, Integral cylinder type with hydroxyapatite coated was 90% and 3i implant type was 93%. One hundred twenty-nine patients had been treated with implant prosthesis. 79 of these patients had received a fixed type prosthesis and 50 patients had received a removable type prosthesis. There were no differences between the implant systems with regard to age, gender. Failures were associated with poor bone quality, smaller implant sizes, a surgical installation technique and stress distribution when in function. Visual analgoue scales recorded as satisfied results functionally and esthetically, but 15% dissatified with chewing ability.

5.C-type Anterior Lamellar Keratoplasty Using Cryopreserved Leftover Cornea for Terrien's Marginal Degeneration.

PURPOSE: To report a case of Terrien's marginal degeneration treated with C-type anterior lamellar keratoplasty using cryopreserved leftover cornea. CASE SUMMARY: A 63-year-old female visited our clinic because of left ocular discomfort and visual deterioration over several years. The patient had +2.25 Dsph = -5.00 Dcyl × 111° of astigmatism, and best corrected visual acuity was 20/20. Microscopic slit lamp examinations revealed an approximately 10.0 mm width semilunar shaped stromal opacity with surrounding stromal lipid deposit, as well as superficial neovascularization with thinning at superior perilimbal cornea. Anterior segment optical coherence tomography confirmed extreme thinning at the opacified cornea. The patient was diagnosed with Terrien's marginal degeneration. To prevent corneal perforation, C-type anterior lamellar keratoplasty using cryopreserved leftover cornea was performed. After 18 months after operation, donor graft was successfully attached via the anterior segment optical coherence tomography and microscopic slit lamp examination and graft rejection was not observed. CONCLUSIONS: C-type anterior lamellar keratoplasty using a cryopreserved cornea can be an effective therapeutic strategy for Terrien's marginal degeneration.

6.A Study on Blood Lead Levels in Pediatric Inpatients.

Lead is one of the most widespread environmental toxins and its poisoning in children was considered a rarity, usually resulting from unique circumstances such as inappropriate use of leadbased body cosmetics or direct administration of lead-containing folk medicines. The increasing concern about children with lower levels of lead exposure has developed, but there are no data regarding the mean blood lead levels and the incidence of symptomatic or asymptomatic lead poisoning in Korean children. We analyzed the zinc protoporphyrine (ZPP) values and blood lead concentrations in 163 pediatric inpatients for a prospective study of lead exposure. The blood lead concentrations in all 163 children were 15~54 g/dl, of whom 111 children (68.1%) were 25~54 g/dl which needs decision to chelate based on the EDTA provocation test. Among 111 children whose blood lead concentrations are 25~54 g/dl, 59 children(53.2%) were between 7 months and 3 years of age, which revealed no significant higher incidence of lead exposure than any other age group. The ZPP values in 126 children (77.3%) were above 35 g/dl. The mean blood lead concentration and ZPP values are 27.8 g/dl and 48.8 g/dl, respectively. We conclude that there are many asymptomatic children with increased absorption of lead in the urban area of Korea, and we need further studies regarding lead poisoning. It is important that there must be a national counterplan and that pediatricians continue to pay attention to lead posioning in children.

7.Significance of ST-Segment Level, ST-Segment Slope, ST-Segment Index and ST-Segment Integral in Exercise ECG as an Indicator of Myocardial Ischemia.

In an attempt to investigate which of the ST-segment parameters, e.g., ST-segment level, ST-segment slope, ST-segment index and ST-segment intergral obtained by mannual drawing can most sensitively indicate quantitatively the extent of exertional myocardial ischemia in patients with angina pectoris, 26 patients with angina pectoris underwent exrecise stress testing, using a bicycle ergometer and thallium-201 myocardial perfusion scintigraphy(Thallium-201 MPS). The exrecise was terminated either when symptoms or signs indicative of myocardial ischemia developed or when the heart rate reached the predicted maximal heart rate during exrecise. The myocardial ischemia region was quantitated by Thallium-201 MPS. The number of myocardial ischemia segments was similar in patients who had positive exrecise test(4.9+/-1.9) and in those who had negative result(3.7+/-2.2). However, it was greater in patients who showed pseudonormalization of the T wave in exercise ECG(6.3+/-2.6) than in those with negative exercise test(3.7+/-2.2)(P<0.05). The extent of myocardial ischemia region expressed by the number of myocardial ischemia segments correlates best with the ST-segment index(r=-0.83) among ST-segment slop(r=-0.60), ST-segment intergral(r=-0.59) and ST-segment(r=0.50). These data suggest that the ST-segment index is the most reliable indicator to reveal the extent of exertional myocardial ischemia among the ST-segment parameters and that pseudonormalization of T wave in exercise ECG is a finding indicative of exertional myocaridial ischemia in patients with angina pectoris.

9.Surgical treatment of hypothalamic hamartoma

Hypothalamic hamartomas are often associated with early onset gelastic seizures, precocious puberty,
behavioral problem and suboptimal response to antiepiletptic drugs. Until now, four surgical options
have been reported to reduce seizure by >50%. Surgical excision have good seizure outcome but
postoperative complications were not infrequent, whether by pterional or transcallosal interforniceal
approach. Radiosurgery is noninvasive alternative to resective surgery but the effect usually does not
appear until several months later. Radiofrequency ablation is less invasive than surgical resection and
its effect is immediate, but lacks long term follow-up data. It also requires three dimensional analysis of
the lesion to enhance effi cacy and safety. As hypothalamic hamartoma is intrinsically epileptogenic and
epileptogenic discharges spread from the lesion, blocking the seizure propagation through endoscopic
disconnection is regarded as an effective and safer option. Surgical choice for a particular patient
should take into account the hamartoma’s size, location, surgeon’s preferences, possible complication
as well as the effect and risk of the various surgical methods. In the present review, open surgery,
endoscopic disconnection, radiosurgery and radiofrequency ablation are discussed.
