1.A Clinical Result of Modified Phemister Method for Acute Acromioclavicular Dislocation

There are many procedures for the treatment of acute A-C injury which have many complications such as limitation of shoulder motion, post traumatic arthritis, recurrence of dislocation etc. From September 1985 to February 1992 at Haesung Hospital, Ulsan, 62 patients with grade I complete A-C dislocation had been treated surgically by modified Phemister method. We report 42 cases with at least 1 year follow up. The following results are obtained. 1. There are no limitation of shoulder motion in all cases. 2. The comparision of coraco-clavicular interval after surgery(1.43mm) with that of follow up (2.13mm) shows no significant difference. 3. We experience 15 cases who have resorption of distal clavicle or arthritic change in x-ray film. 4. Clinical results shows that excellent in 20 cases, good in 10 cases, fair in 11 cases, poor in 1 case. From the above result, we suggest the modified phemister method is good procedure in the treatment of acute A-C dislocation.

2.Influence on Changing of Area of Spinal Canal after Reduction by Posterior instrumentation in Thoracolumbar & Lumbar Burst Fractures

There have been many debates concerning operative decompression of treatment of thoracolumbar burst fractures with retropulsed bone fragment. From March 1988 to February 1992, authors treated thirty-three thoracolumbar burst fractures by using transpedicular screw fixation and posterior fusion via the posterior approach. We attempted to reduce retropulsed fragment by ligamentotaxis alone and not to do posterolateral nor anterior decompression. After the reduction of fractured spine by posterior instrumentation, we tried to determine the efficiency of reduction of the retropulsed fragment by ligamentaxis along. As a method, we compared the change of anteroposterior, transverse to diameter and area of spinal canal of fractured spine between preoperative and the postoperative situation. The results were as follows; 1. The mean anteroposterior and transverse diameter of the spinal canal on computed tomogram film was 10.1mm & 21.8mm preoperatively & 12.4mm & 23.2mm postoperatively, showing an increase. 2. The area of spinal canal of involved spine on CT film was evaluated preoperatively & post-operatively, the mean spinal canal invasion rate decreased from 36.3% preoperatively to 14.3% postoperatively. 3. The degree of reduction of middle height on plain x-ray and reduction of spinal canal invasion on computed tomogram were statistically correlated(p < 0.01). 4. There was no correlation between the degree of canal narrowing and degree of neurologic impairment. also, there was no correlation between the reduction of retropulsed fragments and subsequent neurologic impairment. 5. There was the relatively satisfactory enlargement of the spinal canal on computed tomogram at the follow-up So we suggest that it is possible to get enough decompression through reduction of retropulsed fragment by ligamen to taxis alone without posterolateral decompression.

3.Treatment of Radial Head Fracture

The radial head forms articulation with radial head fossa of proximal ulna and capitellum, and it ditectly contributes pronation and supination of forearm and also flexion and extension of elbow. There were debates in treatment of radial head fracture especially in displaced or communited fracture. From January 1982 to February 1992, we experienced and analysed 29 cases of radial head fracture. They were treated with conservative treatment or operative treatment according to type The results were as follows; 1. We could get better results with open reduction and internal fixation with miniscrew than radial head excision in type 11 radial head fracture. 2. There were unfavorable results of valgus instability, weakness in all cases of radial head excision.

4.Effects of Intake of Alcohol on the Concentration of Urinary 4-hydroxyproline.

BACKGROUND: We studied whether alcohol intake influences in urinary 4-Hyp concentration and whether 4-Hyp concentration is change by time in healthy office workers for 2 weeks. METHODS: Three healthy male workers, who has a same job, recruited for this study, aged from 29 to 33 years, who had been any diagnosed or treated to the alcohol or bone related diseases. Urine samples were picked about 30 ml in screw capped vials for 2 weeks, and analysing method were used to previous published method for determination of urinary 4-Hyp with dansyl chloride. RESULTS: From the ANOVA-test results about objects' Hyp concentration that classify by urine picking time, When subjects did not ingest alcohol, urinary 4-Hyp concentration was difference according to subjects for-work and post-work. But, did not show difference when subjects ingested alcohol. While, T-test result that do alcohol intake existence and nonexistence and urine picking time by variable, urinary 4-Hyp concentration did not show difference for-work and post-work, but there was difference in post-work that is next day that ingest alcohol. When did not ingest alcohol, mean concentration of urinary 4-Hyp increased go by post-work (3.80+/-6.47 nmol/mg creatinine) in pre-work (1.46+/-1.61 nmol/mg creatinine). But when ingested alcohol, mean concentration of urinary 4-Hyp decreased by the next day morning (3.32+/-6.36 nmol/mg creatinine), order of for-work (1.54+/-1.59 nmol/mg creatinine) in alcohol intake future generations post-work (5.09+/-6.41 nmol/mg creatinine). CONCLUSION: We confirmed that alcohol is influencing in urinary 4-Hyp concentration of three healthy male workers, who has a same job, for 2 weeks, and mean concentration of 4-Hyp increasing go by the post-work in the pre-work, could confirm that influence in the next day morning if ingest alcohol in the evening.

5.Female Lung Cancer: Re-Analysis of National Survey of Lung Cancer in Korea, 2005.

PURPOSE: Female lung cancers have different clinical features and therapeutic results as compared to those of male lung cancers. The aim of this study was to analyze the differences of Korean men and women with lung cancer. MATERIALS AND METHODS: We re-analyzed the results of a national survey of lung cancer conducted by the Korean Association for the Study of Lung Cancer in 2005. RESULTS: Of the 8,788 patients, 2,124 (24.2%) were female. The mean age at the diagnosis was 62.5 years for the females and 64.8 years for the males and the difference was significant (p<0.001). An age <50 years was more frequent for the women than for the men (16.2% vs. 7.9%, respectively; p=0.001). The stages between genders were different for the patients with non-small cell carcinoma (NSCLC) (p<0.001), but not for the patients with small cell carcinoma. The overall survival time was longer for woman than that for the man (p<0.001). However, the male patients had longer survival for the smokers with adenocarcinoma and the smokers with squamous cell carcinoma. The never smoker female patients had a better survival time than did the smoking female patients, but the male patient' survival was not influenced by the smoking status. The stage-specific survival rates were better for the women at all stages of NSCLC (p<0.001). The women who received chemotherapy had a longer survival time did the men who received chemotherapy (p<0.001). CONCLUSION: Women with lung cancer were relatively overrepresented among the younger patients and they smoked less intensively, raising the question of gender- specific differences in the carcinogenesis of lung cancer. Over-representation of adenocarcinoma was observed in the women regardless of their smoking status. Women with lung cancer had a better prognosis than men; however, the smoking females showed the worst prognosis. Gender and the smoking status are clearly important factors in the therapeutic approach to lung cancer.
