1.Mohs Micrographic Surgical Approach in a Case of Extramammary Paget's Disease.

A 67-year-old man noted a 4×6cm sized, erythematous, erosive, oozing patch on the penile shaft, which resisted conservative therapy for three years. Histopathologic examination revealed typical findings of extramammary Paget's disease. Interestingly, nests of Paget's cells in some hair follicular structures were shown in the middermis. The lesion was excised by microscopically controlled excision, using the fresh-tissue technique. There were histologically involved areas extending to quite a distance beyond the clinically normal appearing skin. Mohs micrographic surgery might offer the unique advantage of effective surgical treatment with microscopically controlled tumor free borders in the extramammary Paget's disease.

2.Colloid Milium : Electron Microscopic Findings.

Ultrastructural study was taken in a 48 year old male farmer with colloid milium. Electrcn microscopically, colloid was mainly composed of protofilaments, amorphous substance and degenerating collagens surrounded by halos. Colloidal filaments were curved and branched at their ends. Fibroblasts near the colloid island contained amorphous materials and protofilaments idnitical to that of colloid materials, Thus we suggest that. colloid milium may be produced by the abnormal fibroblasts or may be derived from actiinically damaged collagen fibers.

Acquired perforating disease(APD) is characterized by hyperkeratotic papules with transepidermal elimination of degenerated material and is associated with renal disease and/or diabetes. Particular attention has been directed to transepidermal elimination because the articles on perforating diseases among patients with chronic renal failure and/or diabetes have been increasingly reported. We describe four patients with chronic renal failure and/or diabetes whose skin biopsy specimens showed transepidermal elimination.

Diffuse neonatal hemangiomatosis is a fatal disorder characterired by multiple cutaneous and visceral hemangiomas. The organs most commonly affected are the gastrointestinal tract, brain, liver and lung. The complications are high output cardiac failure, gastrointestinal bleeding and hydrocephalus. We present a patient with diHuse neonatal hemangiomatosis associated with massive hepatic involvement and high output cardiac failure, which was successfully treated with high dose corticosteroid.

6.Two Cases of "Pinch Modification" of the Linear Advancement Flap (Peng Flap) in Repair of Round Defects on the Scalp.

Pinch modification of the linear advancement flap(Peng flap) is a hybrid of the transitional linear advancement flap and two bilateral rotation flaps. The advantages of this technique are a wider and shorter flap and an increase of the skin survival rate. This technique can be applied not only to the dorsum of the nose, but also to large defects of the scalp secondary to the surgical excision of benign or nalignant. lesions, scars, aplasia cutis and cicatricial alopecia. We pres ent, two cases of round defects on the scalp. The first case is an 8-year-old boy suffering from cicatricial alopecia on the right temporal area. The second case is a 12 year-old boy who had had nevus sebaceus on the right temporal area since birth. Pinch modification of the linear advancement flap can be erformed after excision without changing the direction of hairs.

7.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterised by diffuse symmetric deposits of adipose tissue, predominantly on the neck, shoulder, back and upper extremities. We report a case of benign symmetric lipomatosis in a 61-year-old-man, which was first noted one year previously. It began growing rapidly about 3 months ago and was associated with alcoholic liver disease. Histopathological findings showed that the reticular dermis had been replaced by normal uncapsulated mature fat cells that were slightly increased in number in the fibrous connective tissue. He was treated with theophylline 300mg/day for two months.

8.A Case of Maffucci's Syndrome.

Maffuccis syndrome was first reported by Maffucci, in 1881. This syndrome is manifested by multiple enchondromas and hemangiomas, characterized by non-hereditary mesenchymal neoplasia that has a propensity for malignant transformation. Involvement of long bones, particularly the bones of the hands and feet, is most common. The most important complication of this syndrome is the high frequency of chondr osarcomatous metaplasia. We report a case of Maffuccis syndrome in a 56-year-old woman with multiple hemangioma on both the hands and forearms, left foot and enchondromas of multiple bones. The disease started at the age of 8 and was associated with deformity, but malignant transformation was not found.

9.A Case of Dermatofibrosarcoma Protuberans Treated with Mohs Micrographic Surgery and Purse - String Suture Repair.

Dermatofibrosarcoma protuberans is an uncommon, locally aggressive fibroblastic tumor, characterized by infrequent metastasis and a marked tendency of local recurrence after excision. Histologically, it shows large uniformed spindle shaped cells arranged in a cartwheel or storiform pattern. We report a case of dermatoribrosarcoma protuberans on the lower abdomen of a man aged of thirty, treated with Mohs micrographic surgery and purse-string suture repair.

10.Recent Trends of Syphilis Prevalence in Normal Population in Korea-1990.

From January to December of 1990, we have evaluated VDRL reactivity among 17,142 apparently normal Korean adults which consisted of 9,151 blood donors in Seoul area, 2,682 pregnant woman delivered at Severance Hospital, Yonsei University and 5,309 physical examinees. The VDRL-positive rates were 0.3% in the blood donors, 0.1% in the pregnant women and 0.8% in the physical examinees but the rates were higher in the age group of 50 years or older. The VDRL titers were below 1:2 in 94% of the VDRL-positive pregnant women and physical examinees. From the comparison of the results obtained by the present author group since 1977, i.e., 2.5 % in 1977, 1.1 % in 1981, 0.6 % in 1986 and 0.4 % in 1990, it is evident that the prevalence of syphilis in Korea is continually decreasing.
