3.The Effect of Hypertonic Saline and Mannitol against Edema Formation after Cryogenic Brain Injury in Rats.

BACKGROUND: After experimental cryogenic cerebral injury, severe focal brain contusion develops due to blood-brain barrier breakdown and vasogenic cerebral edema formation. This study has been conducted to find out the effects of hypertonic saline against cryogenic brain edema in rats. METHODS: Thirty rats of either sex weighing 250 to 300 g underwent a 60 seconds of cryogenic brain injury. All rats were randomly divided into one of three groups; control group (n = 10), 7.5% saline group (n = 10), and 10% mannitol group (n = 10). The water contents were measured 60 minutes after cryogenic injury by using the dry-weight method. RESULTS: The water contents in the 7.5% saline and 10% mannitol groups were significantly decreased compared with the control group. The levels of edema in the 7.5% saline and 10% mannitol groups were also significantly decreased compared with the control group. Although it appeared as if that 10% mannitol might decrease edema formation more than 7.5% saline, there were no statistical differences between the 7.5% saline and 10% mannitol groups. CONCLUSIONS: Hypertonic saline (7.5%) may be as effective agent to reduce edema formation after brain trauma to the same degree as mannitol.

5.MR Imaging of Anterior Cruciate Ligament Injury: Associated Findings.

PURPOSE: Authors investigated the associated findings and their value in the diagnosis of anterior cruciate ligament(ACL) injury in MR image. MATERIAL AND METHODS: The knee MR images of 47 patients with ACL injury(complete; 24, partial; 23) and 61 patients with normal ACL confirmed by the knee arthroscopy or operation were reviewed retrospectively. The degree of anterior translocation of tibia and the degree of posterior cruciate ligament(PCL) buckling were evaluated. The prevalence and pattern of associated adjacent bone, ligament and meniscus injuries were studied. RESULTS: The means( +/- 2standard errors) of anterior translocation were different significantly in statistical analysis(p<0.001, student t-test) between injury group(7.51 +/- 1.16mm) and normal group(-0.56 +/- 0.92mm). In the level of 5mm of anterior translocation for the criteria of ACL injury, the sensitivity, specificity, accuracy were 78.7%, 89.5%, 84.3% for each. The means of PCL buckling ratio were also different statistically b. etween injury group(0.23 +/- 0.02) and normal group(0.17 +/- 0.01)(p<0.001). In the level of 0.20 for diagnostic criteria of ACL injury, the sensitivity, specificity, accuracy were 71.4%, 83.6%, 78.4% for each. Thirty one medial meniscus tear(66%), thirteen lateral meniscus tear(28%), ten medial collateral ligament injury(28%), one PCL injury (2%) were associated with ACL injury. The twenty nine bone marrow changes were found in twenty patients {43%) which included acutely injured seven patients. In acute cases, the bone marrow changes were depicted as diffuse or focal high signal intensity lesions in lateral femoral or tibial condyles in contrast to the changes in chronic cases depicted as focal low signal intensity lesions in variable location. Lateral femoral condylar notch depression were found in nine patients(19%) and avulsion fractures of anterior tibial spine in four patients(9%). CONCLUSION: The associated findings with ACL injury (anterior translocation, buckling of PCL, associated bone, ligament and meniscus injuries) are considered to be helpful in the diagnosis of ACL injury on Knee MR images, when the findings of anterior cruciate ligament itself are not confirmative.

7.MINE (mesna, ifosfamide, mitoxantrone, etoposide) Chemotherapy as a Treatment of Relapsed or Refractory Aggressive Non-Hodgkin's Lymphoma.

PURPOSE: The prognosis of non-Hodgkin's lymphoma (NHL) is disappointing for patients who experience primary treatment failure or relapse after an initial response. Patients in relapse may respond again to chemotherapy, however the time to disease progression becomes shorter and eventually the disease becomes resistant. The aim of this study was to evaluate the efficacy and safety of the MINE regimen in the treatment of patients with relapsed or refractory NHL. Material and Methods: Forty-three pretreated patients with a median age of 56 years were enrolled into the study between October 1995 and June 2000. Most patients (60.5%) had a performance status of 0 to 1, and a diffuse large cell subtype (55.8%). Seventy-four percent of patients had stage III or IV disease at the start of MINE treatment. Eighteen (41.9%) patients had complete response, 5 (11.6%) had partial response, and 20 (46.5%) had failed to respond to prior therapy. Ifosfamide 4 g/m2 was divided over 3 days and administered IV over a 1 hour period. Mitoxantrone 8 mg/m2 was administered as a short IV infusion on day 1. Etoposide (65 mg/m2/day) was infused over 1 hour on days 1 to 3. A total of 144 cycles was administered, with a mean of 3.34 cycles per patient (range, 1-8). The mean relative dose intensity was 87.4%. RESULTS: 1) Nine patients achieved a complete response and nine patients achieved a partial response, resulting in an overall response rate of 43.8% of the 41 assessable patients. 2) The median survival time was 6 months (95% CI, 4 to 8 months), and the median time to failure was 5 months (95% CI, 3 to 7 months). 3) A statistically significant association with complete response rates was found for complete response to prior therapy (p=0.049). The significant factors for overall survival were a complete response after MINE chemotherapy and serum 2-microglobulin (p=0.003, p=0.012, respectively). The significant factors for time to treatment failure were a complete response after MINE chemotherapy and serum 2-microglobulin (p=0.003, p=0.044, respectively). 4) The main result of toxicity of MINE was bone marrow suppression. CONCLUSION: The response to MINE chemotherapy and serum 2-microglobulin were both independent prognostic factors for overall survival and time to treatment failure. As the median time to treatment failure for complete responses was 14 months, the best use of this regimen could be in a strategy that includes prompt consolidation of a complete response with intense chemotherapy, with or without hematopoietic stem cell rescue.

8.Effect of Additional Lidocaine on the Onset Time of Vecuronium.

BACKGROUND: Local anesthetics have been shown to interact with neuromuscular blockers. Most local anesthetics decrease neuromuscular transmission and potentiate neuromuscular block from muscle relaxants. The purpose of this study was to examine the effectiveness of lidocaine on the onset time of vecuronium and to compare that with other method such as simply increasing the dose of vecuronium. METHODS: Sixty patients of ASA physical status I or II were induced with thiopental (4-5 mg/kg) and maintained with O2-enflurane (2.5 vol%). They were randomly divided into four groups: Vecuronium (0.1 mg/kg) was administered intravenously in Group C (n = 15), additional lidocaine (1 mg/kg) was given intravenously 1 min prior to administration of vecuronium in Group L (n = 15), increased vecuronium (0.15 mg/kg) was given in Group V (n = 15) and succinylcholine was given in Group S (n = 15), respectively. Neuromuscular blockade was assessed by train-of-four (TOF) at the adductor pollicis muscle with supramaximal stimulation of ulnar nerve (2 Hz, 0.2 ms) every 12 sec. Endotracheal intubation was performed and intubating conditions were evaluated according to the standard scoring method after measuring the onset time (from the end of giving each muscle relaxants to the 90% suppression of the first twitch). RESULTS: The onset time of Group L (122.0+/-11.0 sec) and that of Group V (98.0+/-16.9 sec) were shorter than that of Group C (135.2+/-16.0 sec) (P<0.05), but these were not shorter than that of Group S (42.0+/-6.2 sec). There was no statistical difference between Group L and Group V. Intubating conditions were good or excellent in all groups. CONCLUSIONS: Additional lidocaine for attenuating sympathetic response could accelerate the onset of vecuronium. But the onset time of this method was not shorter than that of simply increasing the dose of vecuroium nor that of succinylcholine.

9.Functional Evaluation of Autonomic Nervous System in Patients with Chronic Renal Failure.

OBJECTIVE: To evaluate the autonomic nervous system function in chronic renal failure patients compared to normal control and to assess the effect of dialysis method and underlying diseases such as diabetes mellitus and hypertension, on autonomic nervous system function in chronic renal failure patients. METHOD: We checked palm and sole skin temperature with digital thermometer, sympathetic skin responses and heart rate variability in chronic renal failure patients (77 persons) and normal control group (77 persons). RESULTS: The amplitude of sympathetic skin response (SSR) and heart rate variability (RRIV) of patients group showed statistically significant difference compared to control group (p<0.05). The diabetic patient group with chronic renal failure showed prolonged latency of SSR in sole but significant differences were shown in amplitude and RRIV (p<0.05). The hypertensive group with chronic renal failure showed prolonged latency of SSR in both palm and sole (p<0.05) but the amplitude and RRIV of those didn,t show statistical difference (p>0.05). CRF without diabetes mellitus and hypertension showed significant difference on amplitude of SSR and RRIV (p<0.05) but autonomic nervous system function tests showed no difference (p>0.05) between hemodialysis and peritoneal dialysis groups. CONCLUSION: SSR test and RRIV could be valuable measure to evaluate autonomic nervous system functions in the patients with chronic renal failure.

10.The Effect of Postoperative Analgesia in Epidurally Administered Clonidine.

Epidurally administered morphine is reported to produce analgesia in human. Epidural and intrathecal clonidine is also reported to produce analgesia and to prolong analgesic action of local anesthetics. This study was undertaken to evaluate the analgesic effects of the combination of epidural morphine and clonidine versus epidural morphine alone in patients with postoperative pain. 20 patients were scheduled for postoperative pain relief by epidural block. Patients were received either epidural morphine and clonidine(clonidine group: n=10) or morphine alone(morphine group: n=10) before general anesthesia. To compare clonidine group and morphine group in postoperative analgesia, we used Visual analogue scale(VAS), time and dosage of additional analgesics required and change of forced vital capacity(FVC). 1) There were no statistically significant differences in hemodynamic values during operation between two groups. 2) Although there was no statistically significant difference in postoperative analgesic effect between two groups, clonidine group showed some trend of analgesic effect during postoperative 24 hours. 3) There were no statistically significant differences between two groups in side effects. Epidural clonidine enhances the analgesic effect of epidural morphine clinically without causing more side effects.
