4.A Case of Livedoid Vasculitis.

A 47-year-old male patient presented us with erythematous, telangiectatic, purpuric or hyperpigmented patqhes, recurrent painful ulceration and ivory white atrophic scars on the lower portions of his legs, ankles and dorsal surfaces of both feet exacerbating on every summer for 5 years. The histopathalogical findings of an atrophic patch showed endothelial proliforetiion, partial occlusion of the dermal capillaries and fibrinoid material) on the superficial blood vessels, This patient was treated with aspirin and dipyriclamole for 4 months, and the skin lesions were improved significantly.

5.Light and Electron Microscopical Studies on the Stroma of Hydatidiform Mole.

Many investigators were interested in the pathogenesis and the relationship between microscopical features and clinical behavior of hydatidiform mole. Trophoblastic cells in the trophoblastic disease were intensively examined histologically, ultrastructurally, immunohistochemically, and with hormone assay method, etc. But ultrastructural study on the stroma of hydatidiform mole was scarcely reported. In this paper, hydatidiform mole was examined at light and electron microscopic levels, with emphasis on the stroma. The results were as follows: 1) Hydropic degeneration of H-mole is more severe in the center of stroma and is not related with the degree of trophoblastic proliferation. Hofbauer cell and vascular structure are extremely rarely observed in the periphery of stroma which has relatively preserved cellular components. 2) Basement membrane is sometimes separated from trophoblastic layer. Degenerated cells in the stroma contain vacuoles, autophagosomes, and lipid droplets. Collagen is abundant in the loose interstitium. Hofbauer cells have no lysosome or phagosome. Vascular lumen is patient and endothelial cells are degenerated. From the above results, H-mole may be produced due to abnormal changes of trophoblasts and stromal changes may be a secondary process, so called autolysis. Hofbauer cells are not engaged in the stromal degeneration and may be different from usual tissue macrophages.
6.Wells' Syndrome with Bullous and Vegetative Lesions.

We report a case of Wells' syndrome consisting chiefly of papular, vesicular, pustular and vegetative lesions on the trunk, lower extremities and palms in a 48-year-old female, though characteristic lesions of this syndrome are known to be cellulitis or urticaria like plaques. Only a few cases with such lesions as ours have been reported until now. Histologic finding showed spongiotic vesicles in the epidermis and diffuse dense infiltration of eosinophils with characteristic flame figures in the dermis. In addition, marked blood eosinophilia was noted. We tried oral prednisolone for 3 weeks, and the skin lesions improved almost completely.

7.Composite Carcinoma-Carcinoid Tumor of Stomach: Report of a case.

The concurrence of typical carcinoid admixed with adenocarcinoma of the gastrointestinal tract is a well known but unusual phenomenom that has been reported in the esophagus, stomach, gall bladder, colon and appendix. The amine precursor uptake and decarboxylation (APUD) concept included the idea that neuroendocrine cells of the gastrointestinal tract were dervied from the neural crest by a process of migration during embryogenesis. However the intimate admixture of cells which have morphologic and immunocytochemical properties of both carcinoid tumors and adenocarcinoma suggests that these tumors are derived from a multipotential stem cell showing bidirectional differentiation rather than from unrelated cell lines. The term composite carcinoma-carcinoid tumor is therefore an appropriate designation for these neoplasms. We report a case of composite carcinoma-carcinoid tumor of the stomach in a 38 year-old female with brief review of literature.
