4.Correlation of Nitric Oxide Concentrations in Follicular Fluid with the Ovarian Response as well as the Age in Women Undergoing Controlled Ovarian Hyperstimulation.

OBJECTIVE: To elucidate the correlation of nitric oxide (NO) concentrations in follicular fluid (FF) with the ovarian response as well as the age in women undergoing controlled ovarian hyperstimulation (COH). METHODS: This study was performed in 72 cycles of whom underwent COH for in vitro fertilization at infertility clinic of Changwon Hanmaeum Hospital and Pusan National University Hospital from Aug., 2003 to Dec., 2003. Each FF was collected at the oocyte retrieval and NO concentrations in FF were assessed. We analyzed the correlation of NO concentrations in FF with the ovarian response as well as the age in women undergoing COH. RESULTS: Among 72 cycles, the clinical pregnancy rate was 37.5% (27cycles). There were no significant differences in patient's age, ampules of gonadotropins used, estradiol level on the day of hCG administration, the number of oocytes retrieved, and fertilization rate between pregnant and nonpregnant groups. However, negative correlation was found between the follicular size and NO concentrations in FF. No significant correlation was found between age and NO concentrations in FF. Comparing NO concentrations in FF between pregnant and nonpregnant women by age, NO concentrations in FF were higher at nonpregnant women, significantly those of them more than 40 years old. CONCLUSION: These results suggest that NO concentrations in FF cannot be a marker for the ovarian response, and have no correlation with all parameters of COH including patients' age with exception for the follicular size.

Partial Trisomy of 3p (Trisomy of 3p2, dup (3) (p23-->pter)) is a characteristic syndrome of chromosomal duplication of distal part of 3p, but breakpoints seem to vary in location. This syndrome shows multiple congenital anomalies with severe mental retardation, characteristic craniofacial change and absence of other gross external abnormalities. The craniofacial dysmorphism includes frontal bossing and temporal indentation, square face, marked hypertelorism, thick and short nose, full lips and a large mouth with downturned corners. Congenital heart defect, most frequently ASD and VSD, are found in most patients. In the majority of patients, the 3p2 duplication is the unbalanced product of a parental autosomal translocation involving 3p2 and another chromosome. We report a case of female baby who has facial dysmorphism, ASD and hyptonia and was found to have 3p2 duplidation (46XX-9, +der(9)t (3:9)(p23:p24)) by chromosomal analysis. Also we found her father was a carrier of blanced translocation of 3p2 and chromosome 9p (46XY, t(3:9)(p23:p24)).

6.Role of Leukemia Inhibitory Factor in the Effect of Co-Culture on Preimplantation Embryo Develpement.

OBJECTIVE: To assess the effect of recombinant human leukemia inhibitory factor on in vitro development of 1-cell ICR mouse embryo. MATERIALS AND METHOD: ICR mice were superovulated with PMSG/hCG and 1-cell stage mouse embryos were recruited. 1-cell mouse embryo were cocultured on human oviductal cells in a CO2 incubator (coculture group) and were cultured on 0.4% BSA+HTF media (control group). And anti-hLIF Ab was added the cocultured group in a different concentration (1pg, 10pg, 100pg, 1ng) and developmental rate was compaired to the control group, and rhLIF was added to the preincubated 0.4% BSA+HTF media in a different concentration (2000U, 1000U, 100U, 10U) and its developmental rate was compaired to group which was cultured on 0.4% BSA+HTF media only. RESULT: 1. The cleavage rate of 2-cell mouse embryo co-cultured with human tubal epithelial cell was significantly higher than that of cultured with media alone (HTF with 0.4% BSA) (p<0.05). 2. When LIF antibody was added to the medium with human tubal epitherlial cell, the mouse embryo could not cleave more than 2-cell in 1 ng of LIF antibody, and less than 1 ng, the cleavage rate was lower than cultured without LIF antibody group(p<0.05). 3. Two cell blocked ICR mouse embryos were developed into four cells under LIF(p<0.05), but no further development was observed. CONCLUSIONS: These results shows that LIF enhances the development of preimplantation embryo, and when rhLIF is applicated in vitro, it has positive effects on the development of early mouse embryo and can help overcoming the two-cell block.

7.Application of Cumulus Cells as Factors to Predict the Outcome of IVF-ET.

OBJECTIVE: To establish the evaluation system of the quality of oocytes on the basis of the incidence of cumulus cells apoptosis, to investigate the relationships between the incidence of cumulus cells and the outcomes of IVF-ET. METHOD: Thirth-four cycles undergoing controlled ovarian hypersimulation for IVF-ET with tubal infertility (23 cycles) or unexplained infertility (11 cycles) were included in this study. Cumulus cell masses surrounding mature oocyte and co-culture of embryos with autologous cumulus cells during IVF-ET process. The incidence of apoptosis in cumulus cells was assessed by apoptosis detection kit fluorescein. The effect of co-culture using cumulus cells and the incidence of cumulus cells apoptosis. RESULTS: The results were as follows: 1. The incidence of apoptosis in cumulus cells markedly increased in patients aged 40 or over, while the fertilization rate was greatly decreased in those age group. 2. Apoptosis in cumulus cells was found in both the fertilized oocytes and unfertilized oocytes, but the incidence of apoptosis was higher in unfertilized oocytes. 3. There is no clear correlation between apoptosis in cumulus cells and the number of oocytes retrieved. However, the incidence of apoptosis was increased when the number of oocytes retrieved was 5 and fewer in comparison with 6~10. 4. Embryo grade was significantly affected by the incidence of apoptosis in cumulus cells. 5. Pregnancy rate of IVF-ET per cycle was 29.4%, and the pregnant group had the higher fertilization rate and a significantly lower incidence of apoptosis in cumulus cells compared with the nonpregnant group. 6. When cumulus cells were used as helper cells in the co-culture of the embryo, in vitro activity of cumulus cells based on morphological change and proliferation did not influence the quality of embryo, but was closely associated with the implantation rate and pregnancy rate, which was enhanced when morphological changes and proliferation of cumulus cells was more active. 7. This difference in the outcome of IVF-ET according to in vitro activity of cumulus cells used for co-cultue was not associated with the incidence of apoptosis in cumulus cells, but rather had likely relations with the different secretion pattern of protein, which may be an embryotrophic factor by cumulus cells. CONCLUSION: These results suggest that the incidence of apoptosis in cumulus cells can be used in predicting oocyte qualities and the outcomes of IVF-ET. And the effect of co-culture largely depends on the in vitro activity of cumulus cells as well.

8.Clinical and Cytogenetic Cytogenetic Studies in Primary Amenorrhea.

The purpose of this study was to determine the frequency of chromosomal or genetic causes of primary amenorrhea, and was made to assess the etiology of disorders in those patients whose chromosome appeared normal. Sixty eight patients with primary amenorrhea were evaluated clinically and cytogenetically, which were refered to our Cytogenetic Laboratory in Department of Obstetrics and Gynecology, Pusan National University Hospital, from Aug. 1988 to Dec. 1996. The results were as follows. l. Out of 68 cases with primary amenorrhea, 40 cases (58.9%) had the normal chromosome constitutions and 28 cases (41.1%) had the abnormal chromosome constitutions including 46, XY. 2. Turner's syndrome was found in 25 cases (36.7%), consisting of 11 cases (16.1%) of 45, X, 3 cases (4.3%) of 46, X, i (Xq), 1 case (1.5%) of 46, X, inv (X), 1 case (1.5%) of 46, X, del (Xq), 1 case (1.5%) of 46, X, del (Xp), 1 case (1.5%) of 46, X, tel (Xq), 1 case (1.5%) of 45, X/46, XX, 1 case (1.5%) of 45, X/46, XY, 1 case (1.5%) of 45, X/47, XXX, 2 cases (2.9%) of 45, X/46, X, del (Xq), I case (1.5%) of 45, X/46, X, del (Xq), 1 case (1.5%) of 45, X/46, X, r (X). 3. 3 cases (4.3%) had the 46, XY chromosome constitution consisting of 2 cases (2.9%) of testicular feminization syndrome and 1 case (1.5%) of pure gonadal dysgenesis. 4. Among 40 patients whose chromosome are normal, the etiologies of amenorrhea were assumed to be caused by 11 cases (27.5%) of hypogonadotropic hypogonadism (idiopathic), 10 cases (25.0%) of congenital absence of vagina, 5 cases (12.5%) of pure gonadal dysgenesis in order of frequency.

9.Comparison of Pregnancy Outcome According to the Types of Tuboplastic Surgery for Correction of Tubaland Peritoneal Infertility.

From January, 1988 to December, 1994, 336 cases of tuboplastic surgery were performedfor the purpose of restoration of fertility at the Department of Obstetrics and Gynecology,Pusan National University Hospital. Of these 336 cases, 269 cases were followed-up for 36months postoperatively and reviewed for evaluation and comparison of pregnancy rate andfactors influencing pregnancy outcome according to the types of surgical procedure performed.Six types of surgical procedures were performed : salpingolysis, tubal reanastomosis, tubalimplantation, fimbrioplasty, neosalpingostomy, and combined procedure.The results for the 269 cases were as follows:1. The distribution of tuboplastic surgery was 38 cases(14.1%) for salpingolysis, 142(52.85) for tubal reanastomosis, 5(1.9%) for tubal implantation, 15(5.6%) for fimbrioplasty,45(16.7%) for neosalpingostomy, and 24(8.9%) for combined procedure.2. One hundred forty-eight(55.0%) of 269 patients who underwent tuboplastic surgeryconceived postoperatively. The pregnancy rate was 44.7%(17/38) for salpingolysis, 71.8%(102/142) for tubal reanastomosis, 20.0%(1/5) for tubal implantation, 26.7%(4/15) for fimbrioplasty,24.4%(11/45) for neosalpingostomy, and 54.2%(13/24) for combined procedure.3. The term pregnancy rate was 37.5%, the ectopic pregnancy rate was 5.6%, and thespontaneous abortion rate was 8.6% in patients who underwent tuboplastic surgery.4. Among one hundred forty-eight conceived cases, one hundred twenty-six pregnancies(85.1%) occurred during the first 21 months after surgery, and almost all pregnancies wereachieved before 24 months postoperatively. The mean interval from surgery to conception was11.1 months.5. On tubal reanastomosis, 1) Concerning the site of anastomosis, the pregnancy rate was higher in the isthmic-isth-mic(87.5%) than in other groups. 2) In patients whose postoperative tubal length was less than 3cm, pregnancy was notachieved. 3) Statistically significant differences were not found in the pregnancy rates followingmicro-and macro-surgery for tubal reanastomosis(p > 0.05). 4) There were no statistically significant differences between the pregnancy ratesfollowing unilateral and bilateral surgical procedures for tubal reanastomosis(p > 0.05). 5) The pregnancy rate was statistically higher in patients who underwent tubal ligationwith ring than with electrocautery(p < 0.05). 6) There were no statistically significant differences between the pregnancy ratesaccording to the duration of sterilization(p > 0.05).6. Distal tubal obstruction treated by neosalpingostomy resulted in pregnancy rates thatwere higher with mild disease than with moderate or severe disease(p < 0.05).

10.The Outcomes of Proximal Humerus Fractures with Medial Metaphyseal Disruption Treated with Fibular Allograft Augmentation and Locking Plate.

BACKGROUND: Proximal humerus fracture is considered to be the third most common fracture for patients aged 65 years or older. Conservative treatment has been known to treat most of humerus fracture. However, fractures with severe displacement or dislocation may require surgical treatment. Intramedullary fibular allograft with a locking plate is frequently used in patients accompanying medial metaphyseal disruption. In this study, author intends to evaluate clinical and imaging results based on patients who underwent surgical treatment using fibular allograft with a locking plate. METHODS: This study is conducted prospectively at Wonju Severance Christian Hospital, targeting patients who previously underwent surgical treatment using open reduction and intramedullary fibular allograft with a locking plate between 2011 and 2015. A total of 26 patients were evaluated on the following: postoperational clinical assessment measuring Constant score, American Shoulder and Elbow Society (ASES) score, and the Disabilities of the Arm, Shoulder and Hand (DASH) score. Postoperational imaging assessments are evaluated via measuring the neck-shaft angle. The study subject were Neer classification type 3, 4 proximal humerus fracture cases with disrupted medial hinge and having cortical comminution in the region of the surgical neck. RESULTS: The average period of progression was 22.5 months, and the average age of patients was 72.6 years. At the final follow-up, the average Constant, average ASES, and average DASH scores were 80.1, 78.5, and 20.6 respectively. The average neck-shaft angle was 127.5°. CONCLUSIONS: In conclusion, fibular allograft augmentation with a locking plate showed satisfying results in both clinical and imaging studies.
