1.A Case of Giant Sertoli-Leydig Cell Tumor of the Ovary with Masculinization.

Sertoli-Leydig cell tumor is a rare sex-cord stromal tumor of the ovary and accounts for less than 0.5% of all ovarian tumors. It is among the most fascinating from pathologic and clinical viewpoints in masculinization. We experience an unusual case of a poorly differentiated Sertoli-Leydig cell tumor in 60-year-old woman who showed masculinization and present it with brief review of literature.

3.Clinical Experience of Etafenone (Baxacor(R)) in the Treatment of Angina Pectoris.

The clinical effect of etafenone on 12 cases of angina pectoris(9 males and 3 females with average age of 58.3 years) were studied and following results were obtained: 1. Anginal chest pains were improved after average 9 days (3~15 days) of medication and the dreug was very effective in 41.7%, moderately effective in 33.3% and slightly effective in 25.0% of cases. 2. The ST segment depression of E.C.G. were reduced in 50.0%, improvement of T waves inversion or enlargement of T waves were observed in 66.7% and E.C.G. improvement either ST segment or T wave observed in 75.0% of cases. 3. No side reactions such as hypotension, palpitation, dizziness, headache or gastrointestinal disturbances were seen during the treatement with etafenone.

5.Expression of ras Oncogene in Tumor Cells of Epidermis.

DNA isolated from human tumor cells can induce malignant transfarmation of tissue culture cells. The DNA is then called an oncogene. Its protein produets have been detected in animal and human tumors and are considered to play a significant role in carcinogenesis. In order to evaluate whether the oncogenes are involved in development of tumors of epidermis and whether they could be used as tumor markers, immunoperoxidase staining was performed for the ras product in sections of squamous cell carcinoma, Bowen's disease, actinic keratosis, keratoacanthoma and seborrheic keratosis. Three cases of sgamous cell carcinoma showed 10~20 positive cells per high power field(HPF). Three cases of Bowen's disease revealed 1-9 positive cells per HPF, whereas the actinic keratosis 1~9 or no positive cells per 10 HPF in all three cases. The keratoacanthoma and seborrheic keratosis showed 1~9 or no positive cells in all observed cases. The positive staining was observed in the cytoplasm. The increasing positivity in parallel with the increase of malignant potential strongly suggests that the ras oncogene is closely related to development of epidermal malignancy and also point out the possibility of ras as a cancer marker.

7.The impact of beam angle configuration of intensity-modulated radiotherapy in the hepatocellular carcinoma.

PURPOSE: This treatment planning study was undertaken to evaluate the impact of beam angle configuration of intensity-modulated radiotherapy (IMRT) on the dose of the normal liver in hepatocellular carcinoma (HCC). MATERIALS AND METHODS: The computed tomography datasets of 25 patients treated with IMRT for HCC were selected. Two IMRT plans using five beams were made in each patient; beams with equidistance of 72degrees (Plan I), and beams with a 30degrees angle of separation entering the body near the tumor (Plan II). Both plans were generated using the same constraints in each patient. Conformity index (CI), homogeneity index (HI), gamma index, mean dose of the normal liver (Dmean_NL), Dmean_NL difference between the two plans, and percentage normal liver volumes receiving at least 10, 20, and 30 Gy (V10, V20, and V30) were evaluated and compared. RESULTS: Dmean_NL, V10, and V20 were significantly better for Plan II. The Dmean_NL was significantly lower for peripheral (p = 0.001) and central tumors (p = 0.034). Dmean_NL differences between the two plans increased in proportion to gross tumor volume to normal liver volume ratios (p = 0.002). CI, HI, and gamma indices were not significantly different for the two plans. CONCLUSION: The IMRT plan based on beams with narrow separations reduced the irradiated dose of the normal liver, which would allow radiation dose escalation for HCC.

8.Comparative Analysis of Trauma Outcomes.

As the productive activities have vastly increased following industrialization and urbanization in the modem society, the resulting high mobility of people and goods have caused a sharp increase in the accidents in the work places as well as traffic accidents. In particular, deaths caused by injuries are generally concentrated in the economically active young peoples, producing incalculable losses to the society and nation as a whole. Advanced nations with superior medical care systems have succeeded in reducing incidents of such deaths by operating trauma centers. Especially noteworthy is the case of the United States where such specialized trauma centers have greatly contributed to reducing deaths from injuries in the non-urban areas with less access to medical facilities. At present Korea has no medical centers specializing in injuries. In large cities, the injuries are being treated tertiary medical facilities while in the provinces they are referred to small and medium-sized hospitals that constitute secondary medical facilities. Currently in Korea the Trauma patients are treated at general hospitals that consist of 726 secondary medical facilities and 40 tertiary medical centers nationwide. The secondary medical facilities which tend to take most responsibility for the treatment of trauma are generally deficient in medical staff and facilities (including operating and intensive care facilities). Despite such deficiency and limitations, no regulations exist regarding treatment or transportation of trauma patients. This article reports the outcome of a comparative analysis of the results of trauma treatments among different types of medical facilities based on objective data in the hope that such study would facilitate a comparison with the treatment systems of advanced countries and thereby contribute to a precise formulation of problems that must be addressed in this area.

9.Clinical Analysis of Domestic Violence in Emergency Department.

BACKGROUND: Domestic violence once considered an infrequently occurring event existing only in deviant relationships, it has been more recently recognized as a widely distributed phenomenon resulting from multiple cultural, social, psycological factors. According to declining world economics, family violence rises with a simultaneous curtailment of agencies available to assist battered persons and there is increasing pressure on emergency departments to serve those who might otherwise have sought help from public agency. In Korea, we don't have any clinical data about domestic violence, and authors analysed clinical pictures of domestic violence victims who came to the emergency department of Korean University medical center. METHOD: We reviewed the victims of the domestic violence retrospectively, who visited in ED, Korean Univ. Hospital from Jan. 1996 to the Dec. 1996. We Analysed data to Age, sex, Injury severity score, location of injury, mechanism, perpetrator. RESULTS: During the study period, total 134 domestic violence victims were identified. The young, females were predominantly victimized. And married women were more commonly violated. (about 70%). ISS was relatively mild (2.491), face and head were mainly injured by punching and objects throwing. The Prevalence of domestic violence victims almost presented at Emergency department between 6:00/P.M. and 8:00 A.M.. CONCLUSION: Emergency physicians suspect domestic violence victims who injured at face and head by punching or hitted would. In these patients, we need more detail history and physical exam. And in case of repeated domestic violence, refer to neuropychatry dept and social workers with perpetratrs, and need mere concerte social programs for domestic violence.

10.A Decreased Growth Rate of the Great Toe Nail Observed in Patients with Distal Subungual Onychomycosis.

BACKGROUND: There are prerequisites for fungal nail infection: fungal organism, susceptible host, and environments with damaged nail. OBJECTIVE: The purpose of this study was to evaluate the growth rate of the great toe nail in distal subungual onychomycosis(DSO). MATERIALS AND METHODS:: The subjects are 60 adult patients with DSO and 19 age- and sex-matched controls with tinea pedis but no DSO. The growth rate of the great toe nail was measured. Data were evaluated with Mann-Whitney U-test and regression analysis using the statistical program for PC. RESULTS: The growth rate of the patients group showed a decrease against that of the controls. The equation for expected nail growth rate of specific age and sex was as follows: the expected nail growth rate(mm/day) = 0.07382 + (0.01498 × G) - (0.00033 × Age) - (0.00669 × S): if a person has DSO, G is 1, and if he has no DSO, G is 2; if a person is male, S is 1, and, female, 2. CONCLUSION: Patients with DSO often have a decreased growth rate of the nail compared with that of the controls. The authors propose the equation of expected nail growth rate in persons with or without DSO, and think that the growth rate of the nail is one of factors in the pathogenesis of onychomycosis.
