3.Experimental Studies on Pathogenicity of the Zoophilic Dermatophytes.

Zoophiilc dermatophytes are basically animal pathogens, but all can cause dermatophytoses in man. Accordingly animals with dermatophytes are an important source of human dermatophytoses, since zoophilic dermatophytes are frequently transmitted directly or indirectly from domestic or wild animals to man through the use of the se animal as pets, or through contact in rural araes. With the recent increased incidence of zoophilic dermatophytoses worldwide,it is consided to be an important public health problem in view of its epidemiologie aspect i.e. its simultaneous occurence in families or groups. To this time, Microsporum canis in cats has only been sporadically reported throughout the country in the late 1970s and very little is additionally known of the zoopbilic species in animal populations and reservoir or host animals in Korea. Therefore, the authors performed a mycological study by culturing hair and skin lesions of 100 dornestic and 64 zoo animals fortnightly for the purgose of isolating zoophilic derrnatophytes and thereby recognizing the reservoir animals. We also performed an experimental derrnatophyte inoculation in human, mice and rabbits by using Microsporum canis, Trichopyton mentagrophyte vagranulare and Microsporum gypseum isolated from animals and patients lesions from Jan. l980 through Jun. 1981, for the comparison of pathogenicity of these dermatophytes species on human and animals and also for the comparison of pathogenicity of strains isolated from patients lesions and animals and thereby considered different from each other. Microsporum canis was isolated from 11 cats. Trichophyton mentagrophyte var granulare from 3 cats, and Microsporum gypseum from 2 Korean racoon dogs.

4.Cord Blood Leptin of Newborns with Normal Intrauterine Growth in 30-42 Weeks of Gestation.

PURPOSE: To evaluate leptin values in placental cord blood of newborns with normal intruterine growth after 30 weeks of gestation and to study the relationship of leptin with gestational age, birth size, ponderal index, BMI, placental weight, gender difference, Insulin-like Growth Factor-I(IGF-I) and Insulin-like Growth Factor binding protein-3(IGFBP-3). METHODS: Ninety healthy newborns(49 males and 41 females) with normal intrauterine growth were studied. They were classified into 4 groups according to the gestational age(GA):group I(30-33 weeks GA, n=17), group II(34-36 weeks GA, n= 24), group III(37-39 weeks GA, n=32) and group IV(40-42 weeks GA, n=17). At the time of delivery, newborn infant' weight, length, head circumference and placental weight were measured and BMI, ponderal index were calculated. A venous cord blood sample was collected at the time of delivery and serum leptin(RIA), IGF-I (IRMA) and IGFBP-3(IRMA) were measured. RESULTS: Cord blood leptin correlated positively with gestational age(r=0.730, P>0.01), body weight(r=0.686, P<0.01), length(r=0.419, P<0.01), head circumference(r= 0.498, P<0.01), BMI(r=0.750, P<0.01), ponderal index(r=0.704, P<0.01), placental weight(r=0.536, P<0.01), IGF-I(r=307, P<0.01) and IGFBP-3(r=0.736, P<0.01). Multiple linear regression analysis showed BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased progressively throughout gestation. Mean leptin values of group II(3.90+/-.28ng/mL) were significantly higher (P<0.01) than those of group I(1.92+/-.09ng/mL) and also values of group III(5.37+/-.52ng/mL) were higher than those of group II(P<0.05). But there was no significant difference between group III and group IV(6.45+/-.36ng/mL). Mean leptin values in females(5.07+/-.54ng/mL, n=41) were significantly higher than in males(4.07+/-.76 ng/mL, n=49) but there was no inter-gender difference in body weight, BMI, ponderal index, IGF-I, IGFBP-3. In addition, mean leptin values in females(7.45+/-.07 ng/mL) were significantly higher than in males(5.02+/-.09ng/mL) after 40 weeks of gestation. CONCLUSION: BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased significantly after 34 weeks of gestation and females had higher leptin values than males, especially in fullterm newborns. Leptin values in newborns had higher correlations with IGFBP-3 than with IGF-I as gestational age increased.

5.Singificance of Bone Scan in the Early Diagnosis of Acute Osteomyelitis

Bone imagings have played important role in early detection of acute osteomyelitis. In acute osteomylitis, bone scan findings precede the appearance of bone change on radiograph. Also, recent studies have reported accuracies of bone scan in diagnosis of osteomyelitis ranging from 84% to 100%. But, in above datas, bone scan reading were only qualitative studies. So we have reviewed Technetium-99m labelled methylen dipho sphonate(99mTc-MDP) scan on 21 patients who had been diagnosed acute osteomyelitis by clinical symptoms, blood culture, pus culture and operative findings from January, 1984 to September, 1985. The purpose of this study is to establish the quantitative study of bone scan using computed pixel counting method. The 21 cases were classified into group I and group II according to initial radiographic findihgs. The group I showed normal radiographic finding or soft tissue swelling without osseous change. The group II showed osseous change. The results obtained were as follows: 1. We obtained positive bone scans in the all cases which were confirmed acute osteomyelitis. 2. We experienced diagnosis of acute osteomyelitis by bone scan which was confirmed as early as 48 hours after onset of clinical symptom. 3, In group I, the average uptake ratio of 99mTc-MDP was 3.22±0.82 ranging from 2.20 to 4.80. 4. In group II, the average uptake ratio of 99mTc-MDT was 6.20±2.27 ranging from 4.00 to 9.40. (p <0.001).

7.Unknow Primary Melanoma.

Unknown primary melanoma shows only metastatic malignant malanoma without obvious primary tumor. These unusual instances of unknown primary melanoma have reported incidences of 1.0~8.7%. This 51 year old patient had many intra-abdominal and mesenteric masses without visible primary tumor. The most commonly accepted explanation for unknown primary melanoma is spontaneous regression of a primary skin lesion.

9.Expression of ras Oncogene in Tumor Cells of Epidermis.

DNA isolated from human tumor cells can induce malignant transfarmation of tissue culture cells. The DNA is then called an oncogene. Its protein produets have been detected in animal and human tumors and are considered to play a significant role in carcinogenesis. In order to evaluate whether the oncogenes are involved in development of tumors of epidermis and whether they could be used as tumor markers, immunoperoxidase staining was performed for the ras product in sections of squamous cell carcinoma, Bowen's disease, actinic keratosis, keratoacanthoma and seborrheic keratosis. Three cases of sgamous cell carcinoma showed 10~20 positive cells per high power field(HPF). Three cases of Bowen's disease revealed 1-9 positive cells per HPF, whereas the actinic keratosis 1~9 or no positive cells per 10 HPF in all three cases. The keratoacanthoma and seborrheic keratosis showed 1~9 or no positive cells in all observed cases. The positive staining was observed in the cytoplasm. The increasing positivity in parallel with the increase of malignant potential strongly suggests that the ras oncogene is closely related to development of epidermal malignancy and also point out the possibility of ras as a cancer marker.
