3.A Clinical Study of 32 Patients with Sweet' s Syndrome.

BACKGROUND: The clinical study of Sweets syndrome has never been reported in Korea. OBJECTIVE: The purpose of this study was to elucidate the clinical observations and associated diseases of Sweets syndrome. METHODS: During a 5-year-period from Jan. 1990 to Dec. 1994, 32 patients were evaluated in regard to age incidence, sex ratio, location of skin lesions, and associated diseases. RESULTS & CONCLUSIONS : 1, The age distribution was in the range of 21-70 years. Most patients were between 31 to 40 years of age. There were 12 male patients and 20 female giving a sex distribution of 1.7: 1 ratio in favor of the fernale. 2. The most cornmon site of iavolvement was the arm(37.5%). 3. ESR was increased in 21 patients(65.6%) and leukocytosis was present in 13 patients(40.6) 4. The most common associated disease was Behcets disease(31.2%). 5. Steroids were used mos frequently for treatment of Sweets syndrome with 12 patients(37.5%). Recurrence occurred in 7 patients(21.9%). The results show that the iniidence of Sweets syndrome in men in Korea is not as rre as was previously thought and tfiat there are many associated diseases especially Behcets disease.

5.Comparison of Characteristics of Ganoderma lucidum According to Geographical Origins : Consideration of Morphological Characteristics(II).

Nine species of genus Ganoderma different in cultural characteristics each other were separated by histo-anatomical differences. Differences among the species and geographic distribution of G. lucidum were also analyzed to provide the criteria for the classification of Korean cultivation or wild type strains. Korean cultivation and wild type strains were quiet different from other species as well as Taiwan and North American G. lucidum strains in histo-anatomical traits of Ganoderma. Pore color of Korean G. lucidum strains was less brown than those of Taiwan and North American strains. Shapes of pores were round or circular in Korean strains but ellipsoidal or angular in Taiwan or North American strains. Pore numbers of Korean strains were more than those(4~6/mm) of Taiwan or North American strains. Hardness of the pileus of Korean strains was much more than that of Taiwan or North American strains. Such characteristics of Korean strains were different from those of other species of Ganoderma. Korean G. lucidum strains could be classified into the other group because they had many different traits in growth characteristics of fruiting bodies and histo-anatomical characteristics from those of Taiwan or North American G. lucidum and other species of G. lucidum complex.

7.Status of parasitic infection diagnosed by surgical biopsy in Kwangju and Chollanam-do.

: In order to know the species and frequency of human parasitic infection diagnosed by biopsy, 149 cases (0.18%) of parasitic infection were reviwed, which were selected from 80,947 biopsied materials submitted for routine histopathological examination during a period of 10 years from 1980 to 1989 at Department of Pathology, Chonnam National University Hospital. They consisted of 112 cases of cysticercosis, 17 paragonimiasis, 7 clonorchiasis, 4 amebiasis, 1 sparganosis, 1 enterbiasis, 1 aniskiasis, and 1 fascioliasis respectively. Based on morphological preservation of cysticercus, they could be divided into mild (20.2%), morderate (40.4%), and severe (39.4%) degeneration. Except 2 cases biopsied at the lungs, 15 cases of ectopic paragonimiasis were located at abdominal cavity (8 cases) and central nervous system (7 cases). One case of intrahepatic fascioliasis was observed. This is the 13th human fascioliasis reported in Korea. From the above results, the frequency of parasitic infections found in biopsied specimens was on the decrease as the cysticercosis and ectopic paragonimiasis.

8.Cementless Acetabular Revision using microporocoated Hemispherical Cup.

Late aseptic loosening of the acetabular component following total hip arthroplasty become increasing frequently after about 10 years and has become a more severe problem both in frequency and severity than femoral component loosening. The complexity of acetabular revision depends largely on the reconstruction required to restore normal anatomy due to acetabular bone loss. The clinical and radiologic results of acetabular revision using a porocoated acetabular component fixed to the pelvis with screws were studied in 36 patients (40 hips) who had moderate or severe acetabular loss. Acetabular revision in patients whose bone stock had already been destroyed provied more formidable problems at revision surgery. The mean Harris hip score was improved 46 to 84. Bone graft union was achieved by 7.4 months and incorporated by 16 months. The graft bone resorption was noted minor degree lateral to the cup. Of 40 cases, 2 cases required re-revision of acetabular cup for identifiable failure of fixation and one was showed probable loosening. The results of the present study suggest that revision of the acetabulum with use of a hemispherical cementless component stabilized with multiple screws and morselized bone grafts filling bone defects appears to be successful in restoring bone stock and providing a stable, pain-free reconstruction.

9.Effect of Radioactive Isotope 131I on Epiphyseal Plate of Tibia in Mice

The purpose of this investigation was to study the changes in the epiphyseal plate of growing mice by histological, histochemical and electron microscopic observation through the administration of radioactive isotope I which induced hypothyroidism. To confirm hypothyroidism induced, radioimmunoassay for triiodothyronine was carried out and the morphologic changes of thyroid gland were observed by a light microscope. Of the total of eighty ICR strain of mice, fifty received single intraperitoneal injection of 20ue/gm of radioactive isotope I which were dissolved in 0.2ml of saline. Thirty received single intraperitoneal injection of only 0.2ml of saline and these mice were used as control groups. At the end of first, second, third, fourth, sixth and eighth week after injection, both saline injected and I treated mice were sacrifised. The thyroid gland and epiphyseal plate being removed were stained with hematoxylin and eosin, alcian blue and with uranyl acetate lead citrate in each for histological, histochemical and electron microscopic studies, The results obtained are as follows: 1. The thickness of epiphyseal plate was narrowed in the I treated animals. In each zone of epiphyseal plate the cell size was reduced although the cell numbers of each cell column were not changed. This means that the reduction of thickness in the epiphyseal plate was due mainly to decreased cell size and scanty territorial matrix not to decreased cell number. 2. Alcianophility was markedly decreased ln proliferative zone, indicating that acid mucopolysaccharide diminished markedly or was absent in the proliferative zone. 3. In electron microscopic examination, the number of rough surfaced endoplasmic reticulum was decreased and their cisternae were narrowed. Decreasing tendency of dilatation and sacculation of cisternae, .reduced number of attached ribosomes on the endoplasmic reticulum, and atrophy of the Golgi apparatus with diminished number of its vesicles were noted, However glycogen particlea were accumulated. 4. The number and length of each trabeculae in the metaphysis just adjacent to the epiphyseal cartilage plate were reduced markedly, and occasionally chondrocytes which were not consumed by endochondral ossification were observed in the cartilage lattice.

10.Varus Osteotomy in Subluxated Hip as Sequellae of Healed Tuberculosis in Children: A Case Report

Subluxation of hip is a disease commonly experienced in orthopaedic field, which is classified into primary subluxation and secondary subluxation. Generally, secondary subluxation of the hip in children who once suffered from infected hips, Legg-Calve-Perths disease and also as sequellae of incompletely treated congenital dislocation of the hip can be often seen in Korea. Especially in Korea, subluxation of the hip with some destruction of the affected femoral head frequently arose from late complication of the tuberculous or septic hip in children. But, no definite treatment for the condition was established and no satisfactory results were obtained with various operative methods. Authors experienced satisfactory result by combining the intertrochanteric varus osteotomy and administration of crystalline glucosamine sulphate (Viatril, Rotta pharmaceuticals, Italy) in subluxated burnt-out tuberculous hip of 6 years old male.
