1.A case of Melkersson-Resenthal syndrome.

The Melkersson-Rosenthal syndrome consists of the associated of indings of facial nerve paralysis, facial swelling and scrotal tongue. We report a case of MRS where a 20 year old female had diffuse swelling on her face and lips for 10 years. This was also accompaiined with difficulty of facial expression and fissured tongue. The improved with intralesional injection: of triamcinolon acetonide 40mg for 3 consecutive weeks and 2 every other week.

3.Osteomalacia: Report of One Case

This presentation is a case of osteomalacia, the cause of which was a nutritional deficiency. The patient was a 42-year-old female who had pseudofractures of ribs, ulnae, pelvic raml, the right second metacarpai bone and both femoral necks. Following treatment by medication combined with internal fixation of the femoral necks, the result was good.

4.Association of HLA-DR and -DQ Genes with Familial Moyamoya Disease in Koreans.

OBJECTIVE: Moyamoya disease (MMD) is an uncommon cerebrovascular disorder, characterized by progressive occlusion at the terminal portion of the internal carotid artery. Incidence of the disease is high in East Asia and familial MMD accounts for about 15% of the disease. Although the pathogenesis is unknown, association of HLA class I or II alleles with MMD has been reported with conflicting results. We investigated whether there is a difference in HLA class II association between familial and non-familial forms of the disease. METHODS: A total of 70 Korean children with MMD, including 16 familial cases (10 probands), and 207 healthy controls were studied. Among familial cases, only 10 probands were used for the HLA frequency analysis. High resolution HLA-DRB1 and DQB1 genotyping was performed using polymerase chain reaction (PCR)-sequence specific oligonucleotide hybridization and PCR-single strand conformation polymorphism methods. RESULTS: The phenotype frequencies of HLA-DRB1*1302 (70.0%) and DQB1*0609 (40.0%) were significantly increased in familial MMD compared to both controls [vs. 15.5%, corrected p (pc) = 0.008, odds ratio (OR) = 12.76; vs. 4.3%, pc = 0.02, OR = 14.67] and non-familial MMD patients (vs. 14.8%, pc = 0.02, OR = 13.42; vs. 1.9%, pc = 0.02, OR = 35.33). The frequencies of DRB1 and DQB1 alleles in non-familial MMD patients were not significantly different from those in controls. CONCLUSION: Our findings suggest that the genetic polymorphism of HLA class II genes or other closely linked disease relevant gene(s) could be a genetic predisposing factor for familial MMD.

6.Objective Criteria for Radiologic Diagnosis of Epiglottitis in Korean Adults.

BACKGROUNDs: A retrospective study was performed to define objective radiologic parameters in diagnosing epiglottitis on soft-tissue lateral neck radiographic study. METHODS: Parameters of soft-tissue structures(epiglottis width, third vertebral body width, ratio of epiglottic width to third vertebral body width) in 30 adult patients compared with those of age and sex-matched controls with foreign body in throat whose radiographic reading was normal. RESULTS: Epiglottis width of more than 11mm, ratio of epiglottis width(EW) to the third vertebral body width(C3W) of more than 0.5 were, respectively, found to be 100% sensitive and specific in differentiating between patients with and without epiglottitis. CONCLUSION: These preliminary results suggest that EW/C3W ratio of more than 0.5, EW of more than 11mm, respectively, may be useful in the diagnosis of epiglottitis in adult patients.

7.The epidemiologic study on dental malocclusion and skeletal facial pattern.

The study was designed to examinate the discrimination rate of 4 antero-posterior cephalometric measurements from Angle's malocclusion groups. The material was 246 cephalometric radiographs taken from 42 Normal occlusions, 83 Class I malocclusions, 64 Class II Division I malocclusions, 57 Class III malocclusions. ANB angle, APDI, AB/OP angle, and Wits appraisal as measurements of antero-posterior skeletal relationship were measured on the cephalometric radiographs and statistically analyzed by the Canonical Discriminant Function. The results of this study were as follows 1. ANB angle, APDI, AB/OP angle, and Wits appraisal were clinically useful measurements for the evaluation of the antero-posterior skeletal relationship 2. The rates that discriminate actual malocclusion groups were 77.45% in Wits appraisal, 74.02% in AB/OP angle, 71.08% in ANB angle, and 70.59% in APDl. 3. The discrimination rate of actual Class III malocclusion show above 93%, but actual Class I and Class II division I malocclusions were relatively low.

9.A case of brain metastasis of papillary thyroid carcinoma.

Papillary thyroid carcinoma(PTC), the commonest thyroid malignancy, has a good prognosis and low incidence of distant metastasis. Brain metastasis from PTC are extremely rare. The frequency of it reported as 0.1% to 5% in several reported series. A 24 year-old man noted a mass in his neck at 19 years of age. The diagnosis of PTC was made by the biopsy of the mass. Total thyroidectomy, postoperative internal radiation and daily 400w thyroxine therapy was done. The tonic contraction of the right leg evolving to secondary generalized seizure was noted at the age of 24 years old. A brain MRI showed Ix1cm enhancing mass in the left frontal lobe without perilesional edema. The brain mass was removed by the CRW-guided stereotactic surgery. The brain metastasis of PTC was confirmed histopathologically.

10.The Prevalence of BPH in Busan City over Age 40.

PURPOSE: We studied 278 men who are over 40 years of age to educate BPH and to evaluate the prevalence of BPH in Busan city of Korea. MATERIALS AND METHODS: Between December 1997 and February 1998, 278 men in Busan city were investigated. The screening tests included urinary symptom questionnaire (IPSS), digital rectal examination (DRE), prostate specific antigen (PSA) and uroflometry. RESULTS: Based on the IPSS, 36.3%, 49.7% and 14.0% of the men were mild (1-7), moderately (8-19) and severely (20-35) symptomatic. The mean maximal flow rate and PSA for men 40 to 49, 50 to 59, 50 to 69 and over 70 years old were 18.4 ml/sec, 14.2 ml/sec, 12.9 ml/sec, 10.6 ml/sec and 1.081 ng/ml, 1.195 ng/ml, 1.527 ng/ml, 2.352 ng/ml respectively. CONCLUSIONS: The prevalence rate of clinical BPH(IPSS>7 and peak urinary flow rate<10 ml/sec) in community-based study of 278 men was 25.5%.
