1.Clinical Study on Congestive Heart Failure.

An analytical observation was carried out on clinical symptoms, physical findings, etiology, precipitating factors, pulse rate, and blood pressure manifested by 115 patients admitted to the Kyung hee university Hospital under the diagnosis of congestive heart failure for a period of October 1971 to September 1974. The results were as follows: 1. The disease affected female 1.4 times more frequently than male and occured most frequently in the 6th decade (26.1%), and its incidence decreased in the 7th decade (22.6%). 2. The important causes of congestive heart failure were hypertensive heart disease (39.7%), rheumatic heart disease (26.1%), cor pulmonale (13.9%), atherosclerotic heart disease (6.1%), postpartum heart failure (6.1%) and pericardial disease (3.5%). The unknown etiology was 3.5% of all cases. 3. The most common precipitatiog factors of the 106 patients of congestive heart failure were infections (59.5%), especially in the upper respiratory tract infection (37.8%). The physical exertion, pregnancy and labor, psychic stress, angina, myocardial infarction, and discontinuation of Tapazol and digitalis were followed. 4. The most common symptoms and signs were the dyspnea (93%), and the next common were pulmonary rales (65.2%), hepatomegaly (59.1%), distention of cervical vein (55.7%), pitting edema (44.3%) and cardiac murmur (38.3%) on admission. 5. The increased pulse rate over 101/min. was about 38.3%, but less than 60/min. was found in 4.3% only. 6. The electrocardiographic findings were abnormal except one case out of 107 cases. The left ventricular hypertrophy (48.1%), atrial fibrillation (24.5%), and bundle branch block (21.7%) were frequently observed. 7. The range of systolic blood pressure measured in 43 patients who had been suffered from hypertensive heart disease were disclosed that 221-240 mmHg (23.3%), 161-180mmHg (20.9%), and 181-200mmHg (20.9%) in order.

3.Neuroprotective Effect of Growth Hormone in Neonatal Rat with Hypoxic Ischemic Brain Injury.

PURPOSE: To investigate the neuroprotective effect of growth hormone(GH) on neuronal cell necrosis and apoptosis at 1 week and 3 weeks after hypoxic ischemic brain injury. METHODS: Sprague-Dawley rats, seven-day-old, were used. Rats were anesthetized with ether less than 5 minutes. The right carotid artery was cut between double ligation. And then, rats were allowed to recover for 30 minutes followed by exposure to 8% oxygen at 37 degrees C for 2 hours for hypoxic ischemic insult. The study group was divided into 2 groups, control group(N=3) and GH treated group(N=3). GH treated group received intraperitoneal injection of GH 1 IU 2 hours after hypoxic ischemic insult following daily adminstration as same dose for 5 days. Rats were decapitated at 1 week and 3 weeks after hypoxic ischemic brain injury. After then, right hippocampal CA1 and CA3 neurons of rat brains were examined. RESULTS: Necrosis was significantly less in GH treated group than control group, and was more prominent at 3 weeks in both groups. The apoptosis was not found in GH treated and control group. CONCLUSION: GH has a neuroprotective effect on neuronal cell deaths(especially necrosis) from 1 week to 3 week after hypoxic ischemic insult in neonatal rat.

4.Embryonic Developmental Capacity and Pregnancy Rates of Fertilized Oocytes in IVF, ICSI and TESE-ICSI Cycles.

OBJECTIVE: This study was performed to evaluate and compare the embryonic developmental capacity and pregnancy rates in conventional in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) with ejaculated sperm or testicular sperm cycles. MATERIALS AND METHODS: Fertilization was examined in the following morning after IVF (group I), ICSI (group II) or TESE-ICSI cycles (group III). Fertilized oocytes were co-cultured with Vero cells until embryo transfer (ET). On day 2 and 5~7, grades of embryos (<4- or > or =4-cell) and blastocysts (BG1, 2, 3 or early) were evaluated. Clinical pregnancy rate was determined by detecting G-sac with transvaginal ultrasonogram. We analyzed the results bychi2 and Student's t-test and considered statistically significant when P value was less than 0.05. RESULTS: Fertilization rate was significantly higher (p<0.05) in group I (79.0+/-21.2%) than in group II and III (56.8+/-21.6% and 36.7+/-25.3%). Cleavage and blastulation rate of group I (95.8+/-13.8% and 59.5+/-25.3%) were significantly higher (p<0.05) than those of group III (83.4+/-18.6% and 40.4+/- 36.5%). Clinical pregnancy rate was significantly higher (p<0.05) in group I and II (40.7% and 41.7%) than that in group III (12.5%). No differences were found in the rates of multiple pregnancy and abortion among three groups. Embryonic implantation rate was higher in group I (15.1+/-20.2%, p<0.05) and II (14.7+/-20.6%, NS) than that in group III (5.1+/-15.6%). However, embryonic implantation rate was increased in ET with blastocyst(s) among three groups. CONCLUSIONS: Fertilized oocytes obtained from TESE-ICSI were harder to be successfully cultured to blastocyst stage for 5~7 days than that from IVF cycles. However, all blastocyst(s) ET increased the embryonic implantation rate equally in IVF, ICSI and TESE-ICSI cycles.

5.Effect of Eicosapentaenioc acid and Butyrated hydroxyanisole on Hypercholestrolemic Diet Induced Atherogenesis in Rabbit.

It has been suggested that the fish oil can reduce atherogenesis in humans and animals, and that peroxidation of lipoproteins may be a major factor causing atherosclerotic lesions. We tested these posibilities in rabbits fed an atherogenic diet by comparing the effect of a eicosapentaenoic acid(EPA: a major component of fish oil)supplement and a butyrated hydroxyanisole(BHA: antioxidant)diet supplement. Tweenty-eight young male New Zealand White rabbits were used in this study. The animals were divided by control, cholesterol fed only, cholesterol + EPA, and cholesterol + BHA groups. The experimental course lasted 12 weeks and animals were sacrificed periodically(2, 5, 8, 12weeks)for quantitative studies of aortic atherosclerosis using light and electron microscopy. Plasma cholesterol levels were determined and lipopreteins were separated periodically. The cholesterol fed only group showed an increased serum cholseterol level and atherosclerotic lesions from 5 weeks of experiments. The EPA supplement resulted in similiar serum cholesterol levels with cholesterol fed only group, but greater lesion than cholesterol fed only group. The BHA supplement resulted in higher serum cholesterol levels except VLDL-cholesterol than EPA supplement group. However, the atherosclerotic lesion was not increased. Our studies support the theory that oxidative modification of lipoproteins is important for the atherogenesis and antioxidant may have a protective effect. However, it failed to show antiatherogenesis effect of fish oil.

6.Cross-reactivity between sera from dogs experimentally infected with Dirofilaria immitis and crude extract of Toxocara canis.

This study was performed to investigate whethere there is cross-reactivity between Dirofilaria immitis and three intestinal nematodes of dogs. In ELISA, D. immitis-infected dog sera obtained at the 4th molting stage (9-11 weeks) and microfilaremic stage (25-30 weeks) were shown to be highly reactive with crude extract of T. canis. In immunoblotting, some antigenic fractions, 44, 57, 88, 100 kDa of crude extract of T. canis, were found to be positive reaction with sera of dogs infected with D. immitis. However, little or no cross-reaction were observed between sera of D. immitis-infected dogs and crude extract antigen of T. vulpis or A. caninum. These result suggest that there are partial cross reaction between sera of D. immitis-infected dogs and the antigen of T. canis.

7.A Case of Sinus Histiocytosis with Massive Lymphadenopathy.

Sinus histiocytosis with massive lymphadenopathy(SHML) is a benign, generally selflimited pseudolymphomatous disease that typically appears with cervical massive lymphaclenopathy. Extranodal involvement including skin occurs in the 28% of the cases. We report a case af SHML in 51 year-old male who had several, prominent firm masses ranging from 1-10cm in the cervical, axillary, inguinal areas and multiple, plum colored nodules and plaques in the face, trunk for about 10 years. The histopathological findings of cervical lymph node, facial nodule showed dense heavy infiltration of large histiocytes with abundant pale eosinophilic cytoplasm in the subcapsular and medullary sinuses of lymph node and dermis of skin. No atypical cells suggesting malignancy is seen in the infiltrates. The patient had been treated with combination of prednisolone and vinblasstine, but he expired 1 month later.

8.Increased expression of the epidermal growth factor receptor gene in psoriasis.

Psoriasis is characterized by disregulation of keratinocyte growth with profound epidermal hyperplasia. Keratinocyte hyperplasia in psoriasis may be expained in part by overproduction of growth factor, and by altered metabolism of the epidemal growth factor receptors (EGFR) in affected skin. The expression of epidermal growth f ictor receptor was investigated by Northern blot and slot-blot analysis of total RNA extrated from biopsies of normal skin and psoriatic lesions. In Northern blot analysis, EGFR-specific mRNA transcripts from psoriatic tissues demonstrated the specificity of hybridizarion with a EGFR mDNA probe. The size of EGFR mRNA transcript was 6.7kb in psoriasis lesions which showed no change of quality. In slot-blot analysis, the levels of EGFR mRNA in poriasis revealed a 1.2 fold to 4.1 fold elevation when compared to normal skin. EGFR were present in all epidermal layers by immunoperoxidase staining, whereas in normal skin they were primarily present in the stratum basalis. These results indicate that the increased expresion of the EGFR gene may be, in part, responsible for the hyperproliferation of the epider nis and that retained EGFR may reflect incomplet; abnormal differentiation in active porasis. This altered process of EGFR metabolism may be involved in the pathogenesis of psoriasis.

9.Malignant Fibrous Histiocytoma of the Heart: A case report.

A case of a 58-year-old man with malignant fibrous histiocytoma (MFH) of the right ventricle is reported light and electron microscopically. This is the first case of MFH of the heart in the Korean literature. A tendency for malignant fibrous histiocytoma of the heart to occur in the left atrium of young women is suggested; this sarcoma's usual location is in the soft tissue of elderly men. The tumor consisted of spindle cells arranged in a focal storiform patterns, clusters or sheets of histocyte-like cells, benign and malignant giant cells, inflammatory cells, scattered mitotic figures and anaplasia of stromal cells. In ultrastructure the constituent of cells of the tumor are primitive mesenchymal cells, histiocytoid cells and fibroblast like cells including giant cells.

10.Pleomorphic Xanthoastrocytoma: A Case Report.

Pleomorphic xanthoastrocytoma is histologically characterized by marked cellular pleomorphism of lipid-laden neoplastic astrocytes and bizarre giant cells showing mitotic figures and high cellularity. Inspite of its ominous-looking microscopic features, howerver, the prognosis is usually favorable. This tumor develops mainly in the supratentorial area of young people and frequently involves the leptomeninges. We experienced a case of pleomorphic xanthoastrocytoma in 18 year-old-male. In addition to the cellular pleomophism, the prominent reticulin fibers surround the individual tumor cells or the tumor cells nests. Immunohistochemical staining and electron microscopy revealed glial fibrillary acidic protein(GFAP) expression and pericytoplasmic basal lamina in the tumor cells.
