1.A Study on Physical Measurement of Officials in Jeonbug Province.

A study was conducted on 2,210 officials in Jeonbug Province to check out their physical growth, development and nutritional state during the period June 1st 1975 to August 31, 1975. As the results of this study, the following conclusions were reached: 1. Physical measurement. a. Body height: The mean body height of male was 166.8+/-4.6 cm and that of female was 156.3+/-4.9 cm. The highest value of mean body height was 167.1+/-3.9cm, showing by male of administrative job. The lowest value of that was 165.7+/-5.2cm, showing by the male functional job, while the difference of body height was not significant in female. b. Body weight: The mean body weight of male was 61.5+/-4.6kg and that of female was 51.2+/-3.9kg. The administrative job heavier than functional job of male by type of job, while that of female of functional job heavier than functional job of male by type of job, while that of female of functional job heavier than administrative job. c. Chest-girth: The mean body chest-girth of male was 90.8+/-3.2cm and that of female was 83.6+/-3.6cm. 2. Physical growth and developmental state. a. The relative body weight was normal in male except the young man under 25 years, while that of female was under the level of standard. b. The relative chest-girth was showed standard values by all age group of officials. c. The Rogrer and Kaup indices were showed standard values except under 19 years officials and 30 to 34 years female officials. 3. Vervaeck index for nutritional status was appeared normal in male but under the values of standard in female.

3.A Case of Cutis Mqrmorata Telangiectatica Congenita.

Cutis marmorata telangiectatica congenita is a very rare congenital vascular anomaly, characterized by vascular mottling of the skin, telangiectasia, hemangioma and skin atropy at birth. The mottling may be generalized or localized in distribution and tends to fade spontaneously with age. A case of CMCT occuring in a 5 months old girl shows localized, reticuhted mottling of the skin and telangiectasia from the left buttock to the leg and s!in atrophy on the left knee. There was no other anomaly except the finger-tip sized capillary hemangioma on the letf scapular area and no change in the course for 8 months.

4.Quantitative Measurement of Membrane Na+-K+ ATPase Activity using Thallium-201: Comparison with Rubidium-86.

PURPOSE: Na+-K+ ATPase Activity has beem estimated by the degree of inhibition of cation transport by cardiac glycosides (ouabain) using Rb-86 as a substrate. The biological characterist-Isc of T1-201 is known to be simiIar to those of potassium as a transport substrate in the presence of glucose, insulin or phobol myristate acetate (PMA). The purpose of this study was to measure ouabain sensitive Na+-K+ ATPase activity using T1-201 and compare with that using Rb-86. MATERIALS AND METHODS: Smooth muscle cells isolated from rat aorta or human placental umbilical artery were cultured, and used to measure cellular Na+-K+ ATPase activity. Na+-K+ ATPase activity was measured as a percentage decrease in cellular uptake of T1-201 or Rb-86 by ouabain under the presence of glucose, insulin or PMA in media. RESULTS: Na+-K+ ATPase ase activity measured with T1-201, as a transport substrate, was not different from those measured with Rb-86 in rat or human smooth muscle cell preparation. Incubation with high concentration glucose resulted in about 30% decrease in enzyme activity. In contrast, insulin or PMA resulted in 50-70% or 28% increases from baseline activity, respectively. CONCLUSION: These results suggests that T1-201 could replace Rb-86 in measurement of ouabain sensititive Na+-K+ ATPase activity in vitro. High level of glucose concentration decreased cellular Na+-K+ ATPase activity, but insulin or PMA increased it.

5.Clinical Evaluation of Degenerative Spondylolisthesis

The instability of the degenerative spondylolisthetic lumbar spine was clinically evaluated by patients symptoms and serial radiograms. With X-ray films of patients lumbar spine, the lumbosacral angle, the sacral angle, the vertebral height difference between anterior and posterior surface of the 5th lumbar vertebra and the level of intercristal line were analysed. And the results of treatment were analysed too. The Results were as follows: 1) In degenerative spondylolisthetic L3 and L4 spines, facets were arranged in sagittal plane and sacrum was vertically arranged, but in L5 facets were coronally arranged and sacrum and in horizontal plane. 2) The lumbosacral angle was 149.9 and the average difference between anterior and posterior height of L5 vertebral body was 3mm. These results revealed that sacrum was more vertical and the 5th vertebral body was more rectangular, so lumbar lordosis was decreased as a whole. 3) The intercristal line was passed at IA or L4–5 interspace in 72.1% of cases(normal:95.8%), so L4 vertebra was placed in higher level. 4) In group A posterolateral fusion was done in 2 cases. Among group B and C, operation was done in 9 cases;4 cases with laminectomy and 5 cases with laminectomy and posterolateral fusion. Two years after surgery, slipping and instability were far advanced in 2 laminectomized cases for whom posterolateral fusion was followed later. 5) Unstable group of degenerative spondylolisthesis with severe neurological symptoms and claudication must be treated by decompressive laminectomy and posterolateral fusion, especially in patients under 60 years of age. 6) Young woman having flattened lumbar curve without slipping, narrowed disc space of L4–5, high-seated L4 and vertical sacrum is thought to be in prespondylolisthetic stage of degenerative origin; therefore, intensive back muscle exercise to prevent the slipping should be performed and periodic X-ray examinations should be followed up to detect the progress of the disease.

6.The Treatment of Recurrent Anterior Shoulder Dislocation with Modified Bristow Operation

A review of 15 cases of the modified Bristow operation for recurrent anterior shoulder dislocation from Jan. 1983 to Dec. 1988 was presented. Using arthroscopy, we could examined the pathologic feature of the recurrent anterior dislocation of the shoulder. The results are as following. 1. The ages at the initial dislocation were ranged from 15 to 26 years and operated from 19 to 61 years. 2. The most common injury mechanism of the dislocation was sports injury (5 cases) and followings were traffic accident, military training, hanging with one hand. 3. The 10 cases were dislocated above 10 times before operation and 3 were dislocated above 10 times per year. 4. Bony lesions were detected on routine X-ray at 8, that is A-P and lateral view, Hill-sach's view, Stryker-notch view and West point view, Hermoddson view, and apical obique view. 5. Arthroscopic examination was done at 7 cases and could detect the pathologic lesion more exactly. 6. All were treated with modified Bristow method 7. Mean follow up periods were 2 year and 4 monthes. 8. The range of motion was not limited but external rotation was done about 18. 9. Excellent results were obtained by Rowe's grading system, mean 88.8.

7.Study on the lead exposure of workers in a litharge making industry.

In order to investigate the level of lead exposure of workers in litharge making industry and to evaluate how lead exposure, personal habit such as smoking and drinking affect the prevalence of lead related symptoms and other study variables, we investigate 114 workers(24 office workers and 90 lead exposed workers) in a litharge making industry. Study variables chosen were blood lead(PbB), zinc protoporphyrin in whole blood(ZPP), Hemoglobin(Hb), hematocrit (Hct), SGOT and SGPT. symptom questionnaires which had 15 lead exposure related symptoms were provided to all workers and filled up by themselves and reconfirmed by physician. The results obtained were as follows; 1. The mean value of PbB, ZPP and SGOT in lead exposed group were higher than those of non-exposed group, and there were no differences of means in other study variables. 2. The smoking and drinking rate of study subjects were 65.8% and 71.0% as a whole. Smoking rates were lower in non-exposed group than exposed group, but drinking rate were not. 3. There were no differences of mean values of study variables between smoker and non-smoker in non-exposed and exposed group, but there was a difference of mean value of SGOT between drinker and non-drinker in lead exposed group. 4. while the symptom prevalence of lead exposed group were higher in neuromuscular category than non-exposed group, those of non-exposed group were higher or same with exposed group in gastrointestinal and general symptom category. 5. The symptom prevalence of smoker were higher than non-smoker regardless of exposure. 6. The symptom prevalence of drinker were only higher in gastrointestinal symptom category than non-drinker. 7. In multiple stepwise regression analysis of lead related symptoms as dependent variable and blood lead, smoking habit, drinking habit and work duration as independent variables, drinking habit contributed to the gastrointestinal symptom category, whereas blood lead and smoking contributed to the neuromuscular symptom category. For the total symptoms work duration and smoking habit contributed significantly.

8.Effect of Tubercin - 3 on a Case of Chornic Dermatitis with Selective Hereditaty Complete IgA Deficiency and Paritial T - Lymphocyte Inactivation.

A case of a clinical syndrome of selective hereditary complete IgA deficiency with partial T-lymphocyte inactivation is reported. A 14 year old boy was admitted with chjef complaint of persistent wide shallow ulcerated non-vesicular patches on the skin folds of whole body, chronic upper respiatory infections and gastointestinal upsets. General appearance showed us as if the undemourished or malabsorption status had been accompanied. The duration of illness was about 10 years under the incorrect or mistaken diagnosis and skin lesions have incompletely responded to the treatment. Peripheral T-lymphocyte determination was carried out by the method of Wybran to be tumed out active T-cell 14%, total T-cell 48%. Tuberculin test showed negative with 5 TU. Immunoglobulin values were respectively as follows: IgG 325 mg%, IgM 60 mg%, and IgA trace. And we diagnosed this case as selective IgA defiency with partial T-lymphocyte inactivation. We started the immunochemotherapy with tuberculo-protein complex, Tubercin-3 and antibictics for 2 months. After 2 months with above regimen. the skin lesions were markedly improved. T-cell count also retumed to the normal limit(active 35%, total 59%) and levels of TgG and IgM raised(IgG 617.5 mg%, IgM 75mg%), but IgA still showed trace amount, Tuberculin test tumed out positive.

9.A case of Idiopathic Neonatal Chylothorax Treated with Thoracostomy and Medium Chain Triglyceride - Containing Diet.

Chylothorax is the most common cause of pleural effusion in the neonatal period and is defined as an effusion of lymph in the pleural cavity. We report a case of chylothorax in a 13-day-old male who was admitted due to respiratory difficulty. Chest AP showed pleural effusion of the left lung and milky yellow fluid was aspirated via thoracentesis upon which laboratory and lipoprotein electrophoresis of pleural fluid revealed findings compatible with that of chylothorax. Due to reaccumulation of chyle after daily thoracentesis, chest tube was inserted at the 4th hospital day, and MCT containing diet was given. He was discharged on the 21st hospital day in good health, and revealed no recurrence for 3 months during outpatient follow up visits.

10.Changes of Serum Androgen and Bone Mineral Density in Korean Aging Male.

PURPOSE: Osteoporosis is generally regarded as disease of women and there are extensive data about post-menopausal osteopotosis, but there is relatively little information concerning the osteoporosis in aging male. We aimed to determine the relationships between age ,androgen and bone mineral density (BMD) in elderly korean men. MATERIALS AND METHODS: We evaluated the sex hormones and BMD of 82 elderly men between May 1999 and April 2000. Serum free testosterone (FT), total testosterone (TT), DHT, LH, FSH and Prolactin were measured by RIA. BMD at anteroposterior L2-4, femoral neck, ward's triangle, femoral trochanter and femoral shaft were determined by dual-energy X-ray absorptiometry. We defined group I as free testosterone <10ng/dl and group II as 10ng/dl. RESULTS: Serum LH and FSH increased with age. There was a significant decrease in serum FT with age, but T concentrations remained unchanged. There were significant declines with age in BMD at femoral neck, Ward's triangle, femoral trochanter, femoral shaft. Serum FT were significantly correlated to BMD at anteroposterior L2-4, femoral neck, Ward's triangle, femoral trochanter, femoral shaft. After controlling for age, serum FT were significantly correlated to BMD at femoral trochanter, femoral shaft. Mean BMD was significantly lower in group I than group II at anteroposterior L2-4, femoral shaft. CONCLUSIONS: We suggest that FT is a male osteoporotic marker in man older than 50 years and bone mineral density should be considered for diagnosis of male osteoporosis associated with PADAM, especially when FT is lower than 10ng/dl.
