3.The Examination of PUVA-Induced Dermatologic and Ophthalmologic Manifestations in Systemic PUVA Therapy.

BACKGROUND: Systemic PUVA therapy is a widely used method for treatment of vitiligo and psoriasis. However, there have been few studies concerning complications of systemic PUVA therapy. OBJECTIVE: We examined the dermatologic and ophthalmologic manifestations in patients receiving PUVA therapy. METHOD: We studied 42 patients who received systemic PUVA therapy for more than six months in our department. RESULTS: The results are summarized as follows. 1. Among the 42 patients, the number of male and female patients were 18(42.9%) and 24(57.1%) respectively. The mean age at the start of PUVA was 42 years and mean cumulative UVA dase was 996 J/cm2. 2. Among the 42 patients, hyperpigmentation found in 9(21.4%), PUVA lentigo in 8(19.0%), pruritus in 5(11.9%) and burn in 3(7.1%). 3. The ophthalmologic manifestations were conjunctivitis(13 cases, 31.0%), pteygium(7 cases, 16.7%), pingueculum(6 cases, 14.3%) and cataract(3 cases, 7.1%). CONCLUSION: Long term exposure to PUVA causes chronic clinical side effects of PUVA. Therefore careful follow-up of patients who receive long term PUVA therapy is necessary.

4.A Case of Primary Malignant Lymphoma of the Conjunctiva.

Involvement of the conjunctiva with malignant lympoma is very rare. Primary malignant lymphoma of the conjunctiva equally affects men and women over age of 40 years and has a predilection for inferior conjunctival fornices. Most lesions occur unilaterUy, involving right and left sides equally frequently. Periorbital swelling is the most common presenting symptom followed by proptosis, ptosis, pain and poor or blurred vision. Radiation therapy appears to be treatment of choice for malignant lymphoma involving the conjunctiva Surgical treatment alone is less likely to remove all the tumor and should be conjoined with a wide field of radiotherapy. The prognosis of the case depends upon whether or not the disease is generalized. Although in Korea a few cases of secondary involvement of the conjunctiva with generalized malignant lymphoma have been reported, no primary case has yet been reported. The authors reported here a case of primary malignant lymphoma of the conjunctiva in a 48 year-old female patient with a review of the literatures.

5.A Clinical Study on Hypertensive Encephalopathy.

Hypertensive encephalopathy is an acute clinical syndrome that shows central nerve dysfunction with sudden and marked elevation in blood pressure. But its pathophysiologic mechanisms, clinical courses and prognosis are still not clear. In order to study clinical manifestations and response to treatment in patients with hypertensive encephalopathy, we reviewed 45 patients with hypertensive encephalopathy who were admitted in Dept. of Internal Medicine, National Medical Center, from January 1975 to December 1984. The following results were obtained: 1) The ratio of male to female was 1.1:1. The peak age of incidence was in the 6th and 7th decade with mean age of 57.5 years. 2) Among 45 patients, only 29 had known history of hypertension and the average duration of hypertension was 8.1+/-3.6 years. 3) The most common sympotm was severe headache (68.9%). And altered consciousness, nausea and/or vomiting, focal neurologic signs and visual disturbance were also common symptoms in decreasing order of frequency. 4) Funduscopic examination showed hypertensive retinopathy in 20 of 24 (91.7%) patients and lumbar puncture revealed increased CSF pressure in 12 of 20 (60%) patients. 5) In most patients, the mean interval to symptomatic improvement was 2.1 days after administration of anti hypertensive agents, but in 6 patients with initial mean arterial blood pressure above 170mmHg, 4 patients showed delayed response and 2 patients were expired.

6.Effects of gender and gonadectomy on growth and plasma cholesterol levels in pigs.

We conducted two studies to determine the effect of gender, gonadectomy (GDX) on growth and plasma cholesterol levels in pigs. In experiment 1, five sham-operated and five GDX female Landrace pigs (26 kg) were allowed to have free access to water and feed up to market weight (approximately 100 kg). Body weight and feed consumption were recorded biweekly, and daily body weight gain, daily feed intake and feed efficiency (gain/feed) were calculated during the feeding period. In experiment 2, 10 male (26 kg) and 10 female (26 kg) Landrace pigs were used; five male and five female pigs were assigned to sham-operated or GDX. Pigs were allowed to have free access to water and a diet without added cholesterol (Table 1) until they were 6 months old (male 104 and female 98 kg) and thereafter they were fed a hypercholesterolemic diet (Table 1) containing 0.5% cholesterol and 0.1% cholate for 10 days. GDX of female pigs increased average daily gain (P<0.05), compared with their sham-operated counterparts during the growing-finishing period, but had no effect (P>0.05) on feed efficiency. Plasma cholesterol levels in pigs fed a hypercholesterolemic diet for 10 days were much higher (P<0.05) in females than in males (161 vs 104 mg/100 mL plasma), and were increased by GDX only in male pigs. HDL-cholesterol/LDL+VLDL-cholesterol ratio appeared to be higher in males than in females, and was not influenced by GDX in either sex. Results suggested that the lower growth rate of female pigs than their male counterparts is attributable to the ovarian activity, and the lower plasma cholesterol level in male than in female pigs fed a hypercholesterolemic diet is due to the testicular activity.

7.The Radiotherapeutic Significance of Serum NSE Level in Non-Small Cell Lung Cancers (NSCLC).

From December 1989 to February 1993, 108 patients with Non-Small Cell Lung Cancers(NSCLC) were studied retrospectively to evaluate radiotherapeutic significance of serum levels of NSE. We considered elevated serum neuron specific pathologic evaluation revealed 86 squamous cell carcinomas, 11 adenocarcinomas, 3 large cell carcinomas, 3 mucoepidermoid carcinomas, and 5 unknown pathology. Eight patients had stageI, 40 stage III A, and 60 stageIII B. S-NSE level greater than 15 ng/ml was considered as elevated, and below this considered as normal. All patients received radiotherapy as primary treatment modality. The responders to radiotherapy had significantly higher mean S-NSE level than on-responders (28.5 ng/ml vs 20 ng/ml, p=0.01). Overall 2-year survival rate (YSR) was 23.6%. According to radiotherapy response, 2 YSR for patients with CR, PR, and NR were 39.2%, 28.6%, and 6.2% respectively (p=0.001). 2 YSR for patients with elevated and normal S-NSE were 14.6% and 31.7%(p=0.02). The patients with NR showed no difference in survival according to S-NSE level. When we considered all patients, S-NSE level showed no significant impact on response. But for squamous cell cardinomas alone, patients with elevated S-NSE had more patients with higher nodal stage. Based on our and other data, NSCLSC with neuroendocrine features have different response to treatment and clinical behavior compared to other NSCLSC. Thus, this subgroup may need different treatment modality, and S-NSE level may have prognostic significance.

8.Evaluation of the radiopacity and cytotoxicity of resinous root canal sealers.

The aim of this study was to evaluate the radiopacity and cytotoxicity of three resin-based (AH 26, EZ fill and AD Seal), a zinc oxide-eugenol-based (ZOB Seal), and a calcium hydroxide-based (Sealapex) root canal sealers. Specimens, 10 mm in diameter and 1 mm in thickness, were radiographed simultaneously with an aluminum step wedge using occlusal films, according to ISO 6876/2001 standards. Radiographs were digitized, and the radiopacity of sealers was compared to the different thicknesses of the aluminum step wedge, using the Scion image software. Using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, the cytotoxicity of each material was determined in immortalized human periodontal ligament (IPDL) cells. The results demonstrated that EZ fill was the most radiopaque sealer, while Sealapex was the least radiopaque (p < 0.05). AH 26, AD Seal and ZOB Seal presented intermediate radiopacity values. All the materials evaluated, except for Sealapex, presented the minimum radiopacity required by ISO standards. The cell viabilities of resin-based root canal sealers were statistically higher than that of other type of root canal sealers through the all experimental time. Further, EZ fill showed statistically lower cell viability in 24 and 48 hours compared to AD Seal and in 72 hours compared to all other resin-based root canal sealers. However, there was no correlation between the radiopacity and cytotoxicity of three resin-based root canals sealers (p > 0.05). These results indicate that resin-based root canal sealer is more biocompatible and has advantage in terms of radiopacity.

9.Hybrid Squamous-Verrucous Carcinoma of Penis.

We report a rare case of hybrid squamous-verrucous carcinoma of penis that grew extensively in the glans and prepuce of a 68-year-old man. He presented himself with penile discharge and pain upon voiding. The penile glans and prepuce were eroded and replaced by a large, fungating, cauliflower-like ulcerative mass. There was no inguinal or pelvic lymph node enlargement. Partial penectomy was done and histologically, the superficial spreading type, well differentiated squamous cell carcinoma was seen with only minimal invasion of the background verrucous carcinoma with a pushing margin. There were no complications and recurrence of the tumor at 12 months after partial penectomy.
