1.Atypically Large Calcific Tendinitis of the Shoulder: A Case Report.

Calcific tendinitis of the shoulder joint is common disease causing acute pain, mainly involving the supraspinatus or infraspinatus muscle, and less frequently the teres minor or subscapularis muscle. This study reports on the satisfactory arthroscopic removal of calcium deposits as well as infraspinatus and supraspinatus muscle repair without relapse via minimal incision using suture anchors. This was a case of atypically extensive calcific tendinitis involving the infraspinatus muscle, with a bursal side partial rupture of the supraspinatus muscle in a 61-year-old female whose chief complaint was chronic pain of the right shoulder exacerbated by limited movement.

159 histologically proved cases of lung cancer have been reviewed at the Department of Internal Medicine, Yeungnam University Medical School for the past two years and six months from January, 1984 to July 1986. 1. The age distribution ranged from 27 to 87 years and 69.2% of the patient were distributed between the ages of 51 and 70. 2. The ratio of male and female was 4.6 : 1 (131 males, 28 females) 3. Chief complains were in order of dyspnea, chest pain, cough, hemoptysis and weight loss. 4. Localization on chest film. Right was more than left (right 58.6%, left 36.2%) and the most frequent site is right upper lung field (33 cases, 21.7%) 6. 76.8% of case was diagnosed histologically under the bronchoscopic biopsy. 8. The most common treatment was conservative therapy In general. However chemotherapy was most common treatment of the small cell type carcinoma.

3.Total Knee Arthroplasty for Severe Flexion Contracture in Rheumatoid Arthritis Knees

Flexion contracture deformities, as well as severe varus and valgus deformities of the knee joint, accompany osteoarthritis or rheumatoid arthritis (RA). In particular, severe flexion contracture deformity of the knee joint is often found in patients with RA, which renders them nonambulatory. This report describes a 26-year-old female patient diagnosed with RA 10 years ago. She had chronic joint pain, severe flexion contracture, valgus deformity in both knees, and limited range of motion in both knees and became nonambulatory. She underwent a total knee arthroplasty (TKA) and serial casting and physical therapy to restore stable joint movement and correct knee joint deformity. Her pain was successfully relieved, and she was able to walk after surgery. Here, we report the excellent results of TKA in this RA patient with severe flexion contracture of both knees.

5.A Simple Prediction Method of Central Vertigo Among Patients with Dizziness in the Emergency Department.

PURPOSE: The symptoms of dizziness are common in the emergency department, accounting for 1~2% of all emergency department visits. Central vertigo, especially stroke is a leading concern when these symptoms occur in isolated condition. However, no method is available to discriminate isolated central vertigo from peripheral vertigo. The objectives of the present study were to evaluate the clinical usefulness of the simple prediction method of central vertigo among patients presenting with the symptoms of dizziness in the emergency department. METHODS: A three-year retrospective study with symptoms of dizziness was performed in a university teaching hospital between 1st January 2005 and 31st December 2007. Multivariate logistic regression analysis of all clinical parameters was performed for a model comprising 10 variables to form a St. Mary's dizziness prediction score. RESULTS: A total of 1058 patients were enrolled. Among central vertigo patients, 70 were ischemic stroke, 8 vertebrobasilar insufficiency, 3 hemorrhagic stroke and 1 had a brain tumor. The odds ratio of the predictor variables were (1) 2.13 for men, (2) 7.18 for disequilibrium, (3) 3.49 for headache, (4) 7.17 for falling tendency, (5) 2.96 for risk factor score 1, and (6) 8.71 for risk factor score 2. Each of these variables was assigned a score 1 or 2 to obtain a total of 8. The risk of central vertigo was 2.64%, 25.60%, and 84.21% for the low, intermediate, and high risk groups, respectively. CONCLUSION: St. Mary's dizziness prediction score is a simple and relatively reliable tool for accessing the symptoms of dizziness and diagnosing isolated central vertigo in the emergency department.

6.Thoracic Epidural Anesthesia and Analgesia (TEA) in Patients with Rib Fractures.

BACKGROUND: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. MATERIALS AND METHODS: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment of enteral nutrition, and ambulation. RESULTS: The indications of TEA were malunion or nonunion of fractured ribs in 29 (82.9%; first operation) and incompletely ribs under previous general anesthesia in 6 (17.1%; second operation). The average number of fixed ribs per patient was 1.7 (range: 1~3). As a combined operation for thoracic trauma, 17 patients (48.6%) underwent removal of intrathoracic hematomas, and we performed repair of lung parenchyma (2), wedge resection of lung (1) for accompanying lung injury and pericardiostomy (1) for delayed hemopericardium. No patient had any intraoperative cardiopulmonary event nor did any need to switch to general anesthesia. We experienced 3 postoperative complications (8.6%): 2 extrapleural hematomas that spontaneously resolved without treatment and 1 wound infection treated with secondary closure of the wound. All patients reestablished oral feeding immediately after awakening and resumed walking ambulation the day after operation. CONCLUSION: Thoracic epidural anesthesia and analgesia (TEA) may positively affect cardiopulmonary function in the perioperative period. Moreover, this technique leads to an earlier return of gastrointestinal function and early ambulation without severe postoperative complications, resulting in a shortened hospital stay and lowered costs.

7.Injury of the Superficial Femoral Artery Secondary to an Unstable Intertrochanteric Fracture: A Case Report.

Injury of the femoral artery with a femoral intertrochanteric fracture is rare, and usually occurs on the deep femoral artery during surgery. We experienced a case of preoperative injury of the superficial femoral artery by a lesser trochanteric fragment. We repaired the femoral artery through an anterior approach before internal fixation.

Vascular complication of hip arthroplasty is relatively rare, and usually involves iatrogenic injury or thrombus formation of main vessels. No case of vascular injury associated with closed suction drainage has been reported. The current report describes an injury of a branch from the lateral circumflex femoral artery caused by a trocar of closed suction drainage in a 72-year-old man who had been treated with bipolar hemiarthroplasty because of a femoral neck fracture. We report on this case with a review of the literature in order to avoid similar complications.

A 46-year-old man presented with a lateral thoracic meningocele associated with cutaneous neurofibromatosis type I and kyphoscoliosis of the thoracic spine upon medical examination. In the majority of such cases, these meningoceles remain asymptomatic, but surgery is indicated when giant or symptomatic cysts are present. The large thoracic meningocele was successfully extirpated through the transthoracic approach in combination with lumbar puncture and cerebrospinal drainage for decompression of the cyst.

10.Treatment of a Segmental Ulnar Shaft Fracture and an Olecranon Fracture.

We would like to report a case of treating a segmental fracture of ulnar shaft, with a concomitant olecranon fracture. The patient was treated using tension band wiring and intramedullary nailing procedures. Based on the nature of the olecranon, we believe this should be a recommended treatment method that minimizes complications for the patient that has multiple fractures and a dislocation of the forearm.
