1.A Case of Hemorrhagic Renal Cyst.

We have experienced an unusual case of hemorrhagic renal cyst in the rt. kidney at 58 years old woman. The Rt. kidney was enlarged, and showed a few subcapsular ischemic areas The color of kidney was dark brown. As the cyst at lower pole of rt. kidney was dissected, hemorrhagic fluid visualized in the cystic cavity. The final pathologic finding was renal cyst associated with malignant change and renal cell carcinoma at another renal parenchyma. Herein, hemorrhagic renal cyst, a rare clinical entity, was reported with review of literatures.

3.The Emerging Crisis of Stakeholders in Implant-based Augmentation Mammaplasty in Korea

Background:
Korea is no longer safe from the risk of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL); the first reported case was a Korean woman in her 40s who had a 7-year-history of receiving an implant-based augmentation mammaplasty using a textured implant. We conducted this study to discuss the emerging crisis of stakeholders in implant-based augmentation mammaplasty and to propose a multi-disciplinary approach to early detection of its complications.
Methods:
We analyzed medical examination data that was collected from patients who visited us between August 12 and September 27, 2019. We evaluated a total of 114 women (n = 114) in the current study. They were evaluated for whether they were in healthy condition. Moreover, their baseline characteristics were also examined; these included age, gender, height (cm), weight (kg), duration since surgery (years), possession of a breast implant card, the site of surgical incision, side of symptoms and reasons for outpatient visit. Furthermore, the patients were also evaluated for their subjective awareness of the manufacturer, surface and shape of the breast implant. Potential complications include malrotation, folding, seroma, capsule thickening, upside-down rotation, rupture, capsule mass and breast mass.
Results:
A majority of the patients had a past history of receiving textured implants. The corresponding percentage was 78.95% (90/114) and 85.09% (97/114) based on their subjective awareness of a breast implant and sonographic findings, respectively. That is, it was slightly increased with the use of a breast ultrasound.
Conclusion
Here, we propose the following approaches. First, patient data should be prospectively collected. By tracking outcomes and complications of an implant-based augmentation mammaplasty, both high-quality care and patient safety can be ensured. Second, stakeholders in implant-based augmentation mammaplasty should collaborate with customers and regulatory authorities. Third, surgeons should consider applying imaging modalities for early detection of postoperative complications.
4.A Case of Acute Transverse Myelitis Complicating Diffuse Spinal Cord Atrophy and Syrinx Formation.

A 4-year-old girl developed acute motor, sensory impairment, lower abdominal pain and urinary dysfunction after URI. In acute stage of disease, MRI showed diffuse spinal cord swelling and high signal intensity on T2WI below T2 body level. The patient did not improved and one month later, follow-up MRI showed diffuse spinal cord atrophy and syrinx formation as sequelae of ATM on T1WI below T9 level. To our knowledge, this is the first case showing the sequelae of acute transverse myelitis seen in Korea.

5.Cytopathologic Observation of Primary Malignant Melanoma of the Lung: A case report.

The pulmonary cytology has reached a high level of accuracy. By the examination of the sputum and/or bronchial brushings, it is now possible to make a diagnosis in 70% to 90% of patients with cancer. Primary melanoma of the lung is very rare and there have been reported about 20 cases in the world literature. We present a case of primary malignant melanoma of the lung in a 61-year-old male diagnosed by cytologic examination of sputum, bronchial brushing and aspirated pleural fluid. Histologic examination of bronchoscopic biopsy and examination of the skin and other primary sites confirmed the diagnosis.

7.A Modified Surgical Technique for 2 Cases of Conjunctivochalasis Near the Lower Punctum.

Conjunctivochalasis is an isolated bilateral condition in which redundant conjunctival tissue overlies the lower eyelid margin or covers the lower pmctum. It produces tearing by mechanical obstruction of the lower punctum and/or mechanical disruption of normal outflow of the lacrimal film. A modified surgical technique, simple local excision of redundant conjuctiva in the crescent shape along the plica semilunaris and the lower bulbar conjunctiva between limbus and lower fornix, relieved epiphora in two male patients with redundant conjunctiva near the lower punctum. Surgery was done under the local anesthesia without postoperattve complications. Authers emphasize the importance of careful examination of conjunctivochalasis during ocular examination of patient complaining tearing.

8.A Case of Silicone Granuloma After Liquid Silicone Injection.

We report a case of silicone granuloma of the breast in a 38-year-old female patient who had been injected with liquid silicone by an unauthorized practitioner for augmentation of her breast. On histopathological examination, we could observe numerous variable sized ovoid or round cavities that gave the section a ""Swiss cheese"" appearance in the dermis and subcutaneous fat. She was treated initially with oral antibiotics and corticosteroids with no significant improvement, and she was referred to the Plastic Surgery Department for surgical removal of the silicone granuloma.

9.A Clinical Review of Gallbladder and Biliary Tract Stones.

In Korea, the incidence and modality of gallstones have changed, since meal have become westernized and the socioeconomic state has improved. Choledocholithiasis, which has a high incidence rate in Koreans is difficulty to treat as a result of retained stones. This report deals with a clinical analysis of recent trends in gallstone disease and approaches to ideal method of treatment. A retrospective analysis was done on 192 gallstone patients who had undergone an operations at the Department of Surgery, Seoul Red Cross Hospital, from January 1992 to December 1996. The results are as follows: 1) The male to female ratio was 1:1.7, and the most common age group was the 6th and 7th decade (50.0%). 2) The most common symptom and physical finding were RUQ pain (83.9%) and RUQ tenderness (73.3%). 3) The most common positive laboratory finding was leukocytosis (40.4%). 4) Bile culture tests were done in 117 cases and were positive in 55 cases (47.0%). The most common bacteria was E.coli in 35 cases (29.9%). 5) The locations of stone were the GB in 146 cases (76.0%), the GB and CBD in 26 cases (13.5%), the GB and CBD and IHD in 14 cases (7.3%), the CBD only in 5 cases (2.6%), and the GB and IHD in 1 case (1.3%). 6) The incidence of retained stones was 15.1% (29 out of 192 cases) and the prevalent site was the CBD in 11 out of 29 cases (37.9%). 7) A complete choledochoscopic stone removal of retained stones was possible in 19 out of 25 cases, showing a 76.0% success rate. 8) The most common postoperative complication was wound infection (13.5%), and the postoperative mortality rate was 1.0%.
