3.Cranio-orbital Zygomatic Approach for Skull Base Lesion.

Radical removal of extensive tumors in the parasellar region, interpeduncular fossa and cavernous sinus, or the clipping of the distal posterior circulation is very difficult via conventional approach. Seven patients with tumors in the parasella, cavernous sinus and subtemporal area, and six patients with aneurysms of distal posterior circulation were treated by using cranio-orbital zygomatic approach, with good results. The advantages of this approach are minimal brain retraction, exposure of lesion in shortest distance, multidirectional viewing of the lesion and a single cranial bone flap with relative good cosmetic result. Our cases treated via this approach are yet insufficient to compare this approach with other conventional operative approach in definitive advantages but we suggest that this approach is one of the best suitable approach for deep large lesions in the anterior and middle cranial fossae.

4.Urinary Hippuric Acid Excretion in Toluene Exposed Workers.

In order to prepare the fundamental data for preventing the hazardous effects at toluene exposure in many kinds of industry, the authors determined the level of urinary hippuric acid on 592 toluene exposed women(exposed group) and 102 unexposed women(control group) in Pusan area, from April 1 to October 31, 1986. Hippuric acid was measured by high performance liquid chromatography. The results were summarized as follows; 1. The mean value of urinary hippuric acid concentration of the control group was 0.44+/-0.21g/l(0.11-0.89g/l). The distribution of urinary hippuric acid concentration of the control group was not fit to the normal distribution. 2. The mean value of urinary hippuric acid concentration of the exposed group was 1.56+/-0.95g/l(0.44-4.57g/l). The distribution of urinary hippuric acid concentration of control group was not fit to the normal distribution. 3. The urinary hippuric acid concentration by age group was not statistically significant in the control group, but in the exposed the urinary hippuric acid concentration was highest in women between 20-29 years old(1.71+/-0.95g/l) and was statistically significant(p<0.01). 4. The urinary hippuric acid concentration by duration of working hours was not statistically significant(p>0.1).

Methyl bromide is a commonly used fumigant to disinfect soil, grains, and warehouses. The accumulation of danger-ous concentrations can produce various neurotoxic effects in humans. A 45-year-old man, who had worked in the fumi-gation business, presented with polyneuropathy, optic neuropathy, and cerebellar sign, and serially sampled blood, urine and cerebrospinal fluid showed significant elevation of methyl bromide concentration. Regular education to workers and strict adherence to the safety precautions are needed to avoid the risk of toxicity associated with methyl bromide fumigation.

Angiodysplasia of the small bowel is an uncommon disorder in Korea, and is often found to be the cause of unexplained gastrointestinal bleeding. A 53-year-old male patient visited an emergeney room because of repeated melena and dizziness, He underwent selective superior mesenteric arterial angiography which revealed abnormal clusters of small arteries and early opacification of veins draining a segment of lower jejunum. An exploratory laparotomy was performed under the impression of angiodysplasia. An ulceration on the lower jejunum was found, and was resected. Subsequent jejunojejunostomy was performed. Histology of the resected specimen showed an irregularly dilated vessels in mucosa and submucosa.

8.Porencephalic Cyst Associated with Shunt Malfunction.

When hydrocephalus develops, excessive cerebrospinal fluid accumulation usually occurs within the ventricular system, leading to a ventriculomegaly caused by increased intracranial pressure. Shunt malfunction, a common complication after a shunt operation, usually occurs, together with recurring ventricular dilatation. The authors report a case of shunt malfunction which manifested as a porencephalic cyst along the proximal catheter. The cyst subsided after the replacement of the shunt system, and this led to the patient's clinical improvement. We postulate that although the region around a catheter is an unusual site for cerebrospinal fluid collection, this finding should be considered as a shunt obstruction. The possible mechanism of reversible porencephaly is discussed and the literature is briefly reviewed.

Carcinoid tumor of the ampulla of Vater comprises about 2.0-8.9% of the gastrointestinal earcinoid tumors and 3-5.5% of the duodenal tumor. The climcal manifestations of the carcinoid tumor of the ampulla of Vater are jaundice, hemorrhage, obstruction of duodenurn, or carcinoid syndrome. But the symptoms of carcinoid tumor are nonspecific and diagnosis is delayed until far advanced state, usually. Recently we experienced one case of carcinoid tienor in a 33-year-old woman who had epigastric pain and intermittent melena, so we report it with a review of the literature
