1.Percutaneous Drainage and Irrigation in Pyogenic Vertebral Osteomyelitis.

STUDY DESIGN: The authors analysed the recovery of clinical symptoms after percutaneous drainage and irrigation in pyogenic vertebral osteomyelitis. OBJECTIVES: To determine the role of percutaneous drainage and irrigation in pyogenic vertebral osteomyelitis. SUMMARY OF LITERATURE REVIEW: Pyogenic vertebral osteomyelitis is a relatively rare condition but its incidence is recently increasing due to common use of intravenous access devices and resultant nosocomial bacteremia. Management of the disease is not uniform and is controversial regarding the role of surgery and duration of use of antibiotics. At present, the role of percutaneous drainage of involved spines in treatment of pyogenic vertebral osteomyelitis is not well-known. MATERIALS AND METHODS: We report 3 cases of pyogenic vertebral osteomyelitis which were treated by continuous drainage with automated percutaneous lumbar discectomy device(APLD device) and draining tube after they haute failed to conservative treatment. APLD device, fluoroscopic guide, contrast media and continuous percutaneous draining tubes were used for this procedure. The percutaneous draining tubes were maintained for 7 to 11 days and each case was followed up for 6 to 10 weeks under admission. RESULTS: Symptoms were relieved 3 to 5 weeks after this procedure. The patients were followed up to 12 to 39 months and experienced no recurrence. CONCLUSIONS: Percutaneous drainage and irrigation with APLD device brought prompt and marked clinical recovery. It is indicated as a supplementary method to conservative treatment or in case of high operative risk patient due to poor general conditions. It has minimal invasiveness, short clinical course and low recurrence rate.

2.Systolic Time Intervals and Hemodynamics in Anemia.

The systolic time intervals and hemodynamics were measured in 10 cases of acute anemia and 28 cases of chronic anemia. The measurment was done by non invasive technique, i.e., simultaneous recording of ECG, PCG and carotid and femoral pulse tracing with paper speed 100mm/sec. Compared with healthy persons chronic anemia showed significant reduction of the systolic time intervals except QS1, but the systolic time intervals were unaltered when they were corrected by pulse rate, diastolic pressure and stroke volume. Acute anemia showed decreased of the systolic time intervals except QS1 also the decrease of isovolumic contraction time(ICT) and QS2 when corrected as above. The preejection period(PEP)/left ventricular ejection time(LVET) ratio were not altered. ICT, PEP and PEP/LVET ratio decreased in proportion to the levels of hemoglobin. The heart rate, stroke volume and cardiac output showed significant increase in chronic anemia but mild incease in acute one. The decrease of diastolic pressures and peripheral resistances were more pronounced in chronic anemia than in acute one.

Mullerian Mucinous papillary Cyatadenernas of Borderline tumor(MMBT) is lined by mucinous epithelium of endocervical type and is characterized by papillae architecturally similer to those of serous horderline tumors, It has been described rarely in the literature, Thia case was reported with a brief review of the concerened literatures. It has important clinical and pathological diBerences from mucinous birderline tumors with intestinal differentiation, but has many similatities to mixed epithelial borderline tumora of Mulierian type. Recently, a case of MMBT in a 22 years old woman was experienced at our department. We presented this case with a brief review of literature.

6.Interferon-alpha2a, 13-cis-retinoic Acid and Radiotherapy for Locally Advanced Squamous Carcinoma of the uterine Cervix: Preliminary Study.

Locally advanced cancer of the uterine cervix is a major cause of death worldwide. Standard treatment with rdiolherepy for locally advanced cancer of the uterine cnvix has a response rate of less than 50%. Resently concurrent chcmoradirothcrpay has been introduced but with some contvovesy. Interferon and retinoic acid are inducible proteins which posses many hiologic activities such as, antiproliferative, immunomodulatory and antineoplastic properties. Combination of interferon and retinoic acid has produced high response rates especially for patients with squamous cell carcinoma . And they may potentiate the radiation cytotoxicity as adiosensitizer. This study was underaken to assess the clinical efficacy of combination regimen with interferon-alpha2a, 13-cis-retinoic acid and radiotherapy from Dec. 1988 to Dex. 1994 at Severance hospital Yonsei uniersity. Twenty seven patients of locally advanced squamous cell carcionma of the uterine cervix enrolled in this study are evaluated for response and toxicity. The results were as follow: 1. Preliminary results of interferon-alpha2a 13-cis-retinoic acid and radiotherapy are 46.7% of response rate(33.3% complete response)and those of concurrent chemoradiotherapy are 41.7% of response rate(46.7% complete response). 2. Major toxicity of interferon-alpha2a, 13-cis-retinoic acid and radiotherapy is fever(60.0%), and only case of grade 2 anemia and one case of grade 1 AST/ALT elevation was noted. There was no grade 3 or 4 toxicity. Systemic interferon-alpha2a, 13-cis-retinoc acid and radiotherapy is highly active, well tolerated therapy for locally advanced cervical cancer.

10.Central Trichoptilosis Associated with Trichorrhexis Nodosa and Pili Torti.

A 12-year-old girl presented with a 1-week duration of hair loss associated with splitting of the hair ends and whitish dots on the occipital hairs. On microscopic examination, a longitudinal splitting of the hair shaft with reconstitution of the normal hair distal to the fracture, nodular swellings, with the appearance of broomsticks pushed into one another, at the site of whitish swellings, and the flattening and twisting of the hair shaft around the long axis were demonstrated. Minor trauma to injury-prone hair is a common cause of hair shaft defects, however the reports with the combined conditions are insufficient in the literature. We describe a patient with central trichoptilosis associated with localized trichorrhexis nodosa and pili torti.
