3.A Recent 10 Year's Clinical Observation on Disease of Pediatric Urology.

A recent 10 year's clinical observation was made on pediatric patients who had been admitted to the department of urology, Kyungpook National University Hospital during the period from Jan. 1. 1973, to Dec. 31. 1982. The results Were as follows: 1. Of 2,252 total urologic inpatients, 301 cases were pediatric urologic inpatients giving a rate of 13.4%. 2. Annual distribution revealed the tendency that the pediatric urologic patients had been increased yearly and especially after the popularization of medical insurance. 3. The proportion of male to female was 5.8: 1. The school aged children (6 to 12 years of age) were most frequently seen and giving a rate of 40.9% of pediatric inpatients. 4. The most common disease group was genitourinary anomaly as 129 Cases and followed by injury 58 cases, hydrocele .42 cases, tumor 27 cases. The most common disease was cryptorchism as 65 cases and followed by hydrocele 42 cases, hypospadias 37 cases. 5. Two hundred fifty four operations were performed in 242 of 301 pediatric inpatients. Orchiopexy was most frequently performed as 62 cases, hydrocelectomy 43 cases, urethroplasty 34 cases and nephrectomy 20-cases followed in order.

5.Ultrasonographic findings of appendiceal mucocele

The authors analyse the ultrasonographic findings of 10 cases of appendiceal mucocele retrospectively. In atotal of 10 cases, 6 cases showed only cystic masses with posterior reinforcement. The remaining 4 cases showedinternal echoes in the cystic masses. Differential diagnosis of mucocele from periappendiceal abscess is notpossible in ultrasound. The possibility of mucocele shold be considered if cystic mass in right lower quadrantabdomen is observed in ultrasound.

6.Peroneal Nerve Entrapment Caused by an Atypical Popliteal Cyst in a Child: A case report.

Baker's cyst is commonly associated with intraarticular pathology such as degenerative arthritis. Few cases of neural compression by a Baker's cyst in the popliteal fossa have been reported with intraarticular pathology. We report a case of peroneal nerve compression by an atypical Baker's cyst in a child without intraarticular pathology. A 10-year-old boy had 6-month history of the left foot drop without a known trauma. There was no swelling or pain in the knee. The electrodiagnostic study demonstrated a profound lesion of the deep peroneal nerve and a mild denervation of the superfical peroneal nerve. Magnetic resonance images displayed an atypical Baker's cyst originating from the popliteal fossa and extending to the posterolateral side of the fibular head. Clinical and electrophysiological findings improved after aspiration of the cyst.

7.Adult Intussusception Caused by an Appendiceal Mucocele and Reduced by Colonoscopy.

Appendiceal intussusception is a very rare disease that is found in only 0.01% of patients who have undergone an appendectomy. Clinical symptoms vary but include acute appendicitis symptoms such as right lower quadrant abdominal pain or repetitive right lower quadrant crampy pain. Some patients are asymptomatic. Operative treatment is necessary to reduce an appendiceal intussusception in adults, but there is a debate about how to perform the reduction. Successful colonoscopic reductions have been recently reported for some cases. We report a case of appendiceal intussusception that was diagnosed, reduced by colonoscopy, and histologically confirmed as a mucinous cystadenoma after the operation.

BACKGROUND: In postmenopausal women, progestogen should be added to protect the endometrium from hyperplasia or carcinoma induced by the unopposed estrogen. However, the effects of progestogen on bone mineral densities and serum lipodproteins have not been precisely evaluated in Korean postmenopausal women. METHODS: To evaluate the effects of progestogen on bane mineral densities and serum lipoprotein in estrogen rephcement therapy, we canducted a 2-year trial of long conjugated equine estrogen(conjugated estrogen 0.625mg/day) with or without cyclic progestogen(MPA 5mg/day for 12 days) in 120 postmenopausal women. In all subjects, bone mineral density was measured in lumbar vertebra(L2-L4) and femur neck using dual energy X-ray absorptiometry(DEXA) and serum lipoprotein was measured from the beginning of the treatment, 12 manths, and 24 manths later, respectively. RESULTS: BMD of femur neck in both groups increased but not significantly compared to basal level at 12 months and/or 24 months of treatment. As for BMD of lumbar spine, it increased significantly in both groups. Both groups showed a significant decrease in the levels LDL cholesterol, but there was no statistical significance in serum triglycerids. Conjugated estrogen plus MPA group in constrast to conjugated estrogen only group showed a significant decrease in total cholesterol levels. CONCLUSIONS: These results suggest that the addition of MPA of the daily of 5mg for 12 days cyclically in estrogen replacement treatment appear to be effective in postmenopausal women with protection on bone mineral density and maintenance of long-term favorable effects on serum lipoprotein.

9.The Measurement of Cardiac Troponin T(cTnT) and Troponin I(cTnI) in Maintenance Dialysis Patients.

BACKGROUND: Despite of the high prevalence of cardiac disease, diagnosing and predicting myocardiac infarction in maintenance dialysis patients may be difficult. The availability of cardiac troponin T (cTnT) and cardiac troponin I(cTnI) offered promise for improving the accuracy of myocardial injuries in the ESRD patients. We hypothesized that cardiac troponin T and I might be useful markers of myocardial injury for patients with dialysis. METHODS: We examined several clinical parameters to identify factors that determine cTnT and cTnI in dialysis patients. The study included 63 patients who had been on dialysis without evidence of myocardial damage for minimum 3 months. Patients were assessed using demographic, cardiac and atherogenic indices including CPK, CK-MB, CK index, LDH, homocysteine, total cholesterol, triglyceride and serum albumin level. All patients were undertaken EKG and echocardiography. Serum levels of cTnT and cTnI were measured by electrochemiluminescence immunoassay(ECL) and chemilumino immunoassay(CLIA) method. We estimated the correlation between cTnT and cTnI with several parameters by using Pearson correlation, linear correlation and Chi- square analysis. RESULTS: The results are summarized as follows : The positivity of serum cTnT was higher in HD patients than CAPD(32% vs. 18%). But there was no significance statistically. Serum cTnI was elevated(>or=0.1 ng/mL) in only one HD patient. Serum cTnT inversely correlated with serum albumin levels (p < 0.05). The positivity of serum cTnT showed statistically significant correlation with underlying diabetic patients than non-diabetic patients(39% vs. 8%, p < 0.05). Serum cTnI was elevated(>or=0.1 ng/mL) in only one diabetic patient. We could not find the significant correlation between serum cTnT and cTnI with age, sex, dialysis mode, duration of dialysis, serum CPK, CK-MB, CK index, LDH, homocysteine, total cholesterol and triglyceride. CONCLUSION: Because cTnT revealed significant correlation with serum albumin and underlying diabetes mellitus. So we speculated that cTnT possibly might be a helpful marker as a predictor of myocardial event and mortality in maintenance dialysis patients. But we can not exclude false positivity of cTnT. And we are currently working to define that cTnI might be more specific marker of myocardial damage than cTnT and the development of cardiac event in cTnT-positive patients.

10.A Case of Pancreatic Metastasis From a Papillary Thyroid Carcinoma Mimicking Pancreatic Cancer.

Papillary thyroid cancer (PTC) has a good prognosis and a low incidence of distant metastases. It is extremely rare for PTC to metastasize to the pancreas. Only five cases have been previously reported worldwide. Most cases are discovered incidentally by abdominal computed tomography (CT) or positron emission tomography-CT (PET-CT) during follow-up studies after thyroidectomies. Pancreatic metastasis of PTC is usually unidentifiable by a whole-body I131 scan, a common follow-up modality. When a pancreatic mass is found in patients with PTC, it must be differentiated from pancreatic cancer. In previous reports, patients with pancreatic metastases of PTC underwent operations for therapeutic diagnosis or underwent fine needle aspiration biopsies (FNAB). However, it is unclear whether the benefit of an operation outweighs the risk. We experienced a case of PTC with pancreatic metastasis that was found on PET-CT. Contrast-enhanced endoscopic ultrasonography (EUS) was performed to evaluate the characteristics of the pancreatic mass and pathological confirmation was obtained cytologically via EUS-FNA.
