1.COMPARISON OF THE ACCURACY OF STONE CASTS MADE FROM ALGINATE IMPRESSION MATERIAL BY MIXING METHODS AND APPLICATION OF TRAY ADHESIVE.

The use of alginate impression materials today is prevalent because of its efficiency and simplicity in clinical settings. Unfortunately, the simplicity of the procedure tends to lull the dentist into a sense of well-being, and lead him into using careless or sloppy technique. Alginate impression materials are used to fabricate diagnostic and preliminary casts, and the final cast. Incorrect use of this material is known to affect the accuracy of the final prosthesis. The purpose of this study was to compare the effect of different mixing methods of alginate impression material and tray adhesive on the accuracy of the stone cast produced by each method. A total of 30 stone casts were produced by using 3 different types of mixing methods (10 stone cast for each mixing method, respectively) The first method utilized an automatic-mixing machine to mix alginate while the second method was carried out manually, strictly following manufacturer's instructions. The third method also involved manual mixing, but did not follow the manufacturer's instructions and was done in a random fashion. Also, 20 additional stone casts were produced by using alginate with or without tray adhesives were included in the study to evaluate effects of tray adhesives on the accuracy of alginate impression. 10 stone casts were produced by adding tray adhesives to the interior surface of the impression tray prior to taking the impression. The other 10 excluded this step. A total of 50 stone casts were analyzed by the three-dimensional measuring machine to measure and compare the dimensional changes of the impression material of each group. The results are as follows. 1. No significant difference was found between the automatic mixing group and the manually-mixing group(p>0.05). 2. For the group that followed manufacturer's instructions, less dimensional changes were recorded than the group that didn't in measuring distance d4(p<0.05) 3. The group that used tray adhesives showed less dimensional changes(p<0.05). The findings revealed that mechanical methods of mixing alginate impression materials had little influence on dimensional changes. However, it is proven that following manufacturers instructions in alginate impression taking is an important step in acquiring accurate impressions and tray adhesives may play an important role in enhancing the results.

2.Hrombosed Aortic Dissections and Aortic Aneurysms: MRI Findings and Differential Diagnosis.

PURPOSE: MRI is known to be an effective imaging modality of the aorta and its role is steadily increasing in the evaluation of acquired aortic diseases including aortic dissections and aortic aneurysms. However, differentiation of the aortic dissections with thrombosed false lumen from the aortic aneurysm with mural thrombus in MRI has not been easy. Therefore, the authors tried to find the characteristic MR featrses which would to differentiate the two diseases. MATERIALS AND METHODS: MR images of 6 patients with thrombosed aortic dissection and 7 patients with thrombosed aortic aneurysms were reviewed retrospectively and compared in regand to shape and extent of thrombus, dimension of aorta, and luminal flow signal. RESULTS: Thrombosed aortic dissections showed sharply demarcated crescent shaped aortic wall thickening of even thickness involving long segment of the aorta, whereas thrombosed aortic aneurysms showed irregular aortic wall thickening of uneven thickness localized in the short dilated segment of the aorta. Characteristically aortic aneurysm with mural thrombus showed eccentric intraluminal slow flow signal. In contrast to the signal void of the true lumen in aortic dissections, the residual lumen of the aortic aneurysm with mural thrombus revealed intraluminal signal due to slow flow. CONCLUSION: Familiarity to these MRI findings of thrombosed aortic dissections and aortic aneurysms may lead to the accurate differential diagnosis in majority of cases.

3.The Effects of Healon(R), Amvisc(R) and Methylcellulose as Viscoelastic Materials on Rabbit Corneal Endothelium and Intraocular Pressure.

To compare the corneal reaction of the widely used viscoelastic substance such as Healon(R), Amvisc(R) and newly developed and less expensive viscoelastic substance(2% methylcellulose), 0.2ml each of various viscous solutions were injected into the anterior chamber of rabbit eyes and their reactions were observed. BSS(Balanced salt solution) was used as a control. 1. Intraocular pressure was elevated to the peak in 3 hours after injection of Healon(R)(44.0 +/- 14.4mmHg), methylcellulose(32.0 +/- 4.0mmHg), Amvisc(R)(33.7 +/- 14.5mmHg) and control BSS(25.8 +/- 4.6mmHg). Intraocular pressure returned to normal at 9 hours after injection of methlycellulose group, while Healon(R) and Amvisc(R) group returned to normal at 24 hours. 2. Central corneal thickness was increased to the peak 12 hours after injection in all groups; Healon(R) 475 +/- 71.9 micrometer, methylcellulose 454 +/- 55.0 micrometer, Amvisc(R) 489 +/- 90.9 micrometer and control BSS 403 +/- 14.0 micrometer. Corneal thickness returned to normal after 2 days in Healon(R) and control groups but it took 6 days for methyclellulose and Amvisc(R) groups. 3. The endothelial cell density 2 weeks after injection were Healon(R) 2,280 +/- 125.0 cells/mm2, methylcellulose 2,187 +/- 120.0 cells/mm2, amvisc(R) and BSS 2,338 +/- 74.0 cells/mm2. The endothelial cell reduction rate was Healon(R) 7%, methylcellulose 6%, Amvisc(R) 14.5% and BSS 3.0%, respectively. 4. In all groups except control ESS group, the endothelial cells under the scanning electron microscope showed decreased microvilli and enlargement of intercellular space. Scanning electron micrograph 2 weeks after injection of Amvisc(R) showed the findings of more edematous endothelial cells compared with those of other groups.

4.Two Cases of Congenital Chylothorax Diagnosed by Prenatal Ultrasonography.

Congenital chylothorax is a rare disorder and can be diagnosed by prenatal ultrasonography recently. Most cases of congenital chylothorax were characterized by different clinical courses of respiratory distress. We describe two female cases with congenital chylothorax observed by ultra- sonography prenatally. In the first case, left-sided pleural effusion was noted by prenatal ultrasonography taken at 34 weeks of gestation, and then pleural fluid was extracted by intrauterine thoracentesis under sonography guidance. After birth, this patient was managed by TPN(total parentral nutrition) and intermittent thoracentesis without surgical treatment. But, pleural fluid was accumulated recurrently and respiratory distress was aggravated. At 15th hospital day, shock state was developed and patient died. In the second case, bilateral pleural effusion and ascites were noted by prenatal ultrasonography, and then patient was delivered immediately without intrauterine thoracentesis. After birth, the second case received conservative therapy including mechanical ventilation, TPN, intermittent thoracentesis and paracentesis. The patient was discharged with complete regression of chylothorax. We report the two cases with brief review of related literatures.

6.Assessment of Irritancy to Sodium Lauryl Sulfate in Man by Laser Doppler Velocimetry.

It is desirable to use more safe, objective, non-invasive method than visual scoring for the evaluation of irritation reaction and determine the optimum eoncentration of sodium lauryl sulfate(SLS) as a positive irritant control in routine patch test. In this study, SLS was applied to the volar forearm of ll non-atopic, healthy volunteers in concentrations of 0.5, 1.0, 2.0, and 5.0% under occlusion for 24h. The blood flow in test sites exposed to SLS was tecorded by laser Doppler velocimetry (LDV). The test sites were scored visually and the skin blood flow was measured at 24h, 48h, and 72h after the application of FiLS. We obtained the following results,' 1. There was a good correlation between visual scoring and skin blood flow using LDV(p = 0.4547). 2. 5.0% SLS could be used as a positive irritant control in routine patch test (p<0,05).

7.Radiation Exposure of Operator during Various Interventional Procedures.

PURPOSE: To investigate the levels of radiation exposure of an operator which may be influenced by the wearing an apron, type of procedure, duration of fluoroscopy and operator's skill during various interventional procedures MATERIALS AND METHODS: Radiation doses were measured both inside and outside the apron(0.5mm lead equivalent) of the operator by a film badge monitoring method and the duration of fluoroscopy was measured in 96 procedures prospectively. The procedures were 30 transcatheter arterial embolizations (TAE), 25 percutaneous transhepatic biliary drainages (PTBD), 16 stone removals (SR), 15 percutaneous needle aspirations (PCNA) and 10 percutaneous nephrostomies(PCN). To assess the difference of exposure by the operator's skill, the procedures of TAE and PTBD were done separately by groups of staffs and residents. RESULTS: Average protective effect of the apron was 72.8%. Average radiation exposure(unit:micro Sv/procedure) was 23.3 in PTBD by residents, 10.0 in PTBD by staffs, 10.0 in SR, 8.7 in TAE by residents, 7.3 in TAE by staffs, 9.0 in PCN and 6.0 in PCNA. Average radiation exposure of residents were 1.9 times greater than those of staffs. CONCLUSION: Radiation exposure was not proportionally related to the duration of fiuoroscopy, but influenced by wearing an apron, various types o[procedure and operator's skills.

8.The Calretinin Immunoreactive Ganglion Cell Postsynaptic to the ON-Cholinergic Amacrine Cell in the Guinea Pig.

PURPOSE: To demonstrate the characterization calretinin-immunoreactive displaced amacrine cells in the ganglion cell layer using immunohistochemistry and electron microscopy. METHODS: For immunohistochemistry, sections from guinea pig retina were incubated with mouse monoclonal antibody directed against calretinin. For double label studies, sections were incuated in mixture of mouse monoclonal anti-calretinin or rabbit polyclonal anti-calretinin with following antibodies: goat polyclonal anti-ChAT, rabbit polyclonal anti-GABA, mouse monoclonal anti-GABAA receptor alpha1, beta2/3. Sections were analyzed using Bio-rad Radiance Plus confocal scanning microscope. Stained sections from three guinea pig were observed with transmission electron microscope. RESULTS: Calretinin immunoreactivity was present in displaced amacrine cells and ganglion cells gaving rise to processes ramified in the inner part of the inner plexiform layer in stratum 4. The same stratum was also occupied by the dendrites of ON-cholinergic amacrine cells. Double-labeling demonstrated that dendrites and cell bodies of displaced amacrine cells colocalized with ON-cholinergic amacrine cells and dendrites of ganglion cells directly overlapped with dendrites of ON-cholinergic amacrine cells. The synaptic connectivity was identified by electron microscopy. Ganglion cell dendrites received synaptic input from ON-cholinergic amacrine cell. GABAA receptor beta2/3 subunit bands cofaciculates the dendrites of displaced amacrine cell and ganglion cell that are juxtapose to the alpha1 subunit of GABAA receptor. CONCLUSIONS: These results indicate that ON-cholinergic amacrine cells modulate calretinin-labeled ganglion cell via GABAA receptor beta2/3 in the guinea pig retina.
