3.Enzyme Histochemical Study for the Estimation of the Lapse of Time in Brain Injury.

This experiment was designed for the evaluation of the usefulness of enzyme histochemistry in the determination of the lapse of time in brain wound, and also for the establishment of medicolegal 'biological time table' on brain wound. Brain injury was made by contusion and laceration of meninges and brain itself in rats. The results were as follows; 1) By routine histological technique, estimation of the lapse of time in brain wound could be possible 4 hours after the infliction of wound. 2) The earliest change of enzyme activities was recognizable by the decreased activities of ATPase and succinic dehydrogenase 30 minutes after the injury. These decreased enzyme activities were not recovered up to the 4th day after the brain injury. 3) Increased acid phosphatase activity was noticed 1 hour, and beta-glucuronidase, 2 hours after the injury in a mild degree. Both increased activities were pronounced following the lapse of time in brain wound. 4) No significant change was seen in alkaline phosphatase, monoamine oxidase, non-specific esterase and leucine aminopeptidase activities throughout the experimental period up to the 4th day. So the enzyme histochemistry of these enzymes seemed to be little valuable for the study on the timing of wound in brain injury. In the light of these results it appeared that the enzyme histochemistry, in particular of ATPase, succinic dehydrogenase, and acid phosphatase, for the estimation of timing of brain wound not only shortened the histological "lag period" up to 30 minutes after the injury, but also provided a useful information in determining the biological time table following the brain injury.

4.Studies of Relationship between P-A Cephalometric Measurements and Vidual Facial Asymmetry.

The degree of facial asymmetry in P -A cephalogram may differ from that we felt in clinical examination. The purpose of this study was to find out the P -A cephalometric measurements affecting the visual facial asymmetry. Fifty four adult patients who had been diagnosed as the facial asymmetry in clinical examination were selected for this study. Through the analysis of facial photograph and P -A cephalogram, the following results were obtained: 1. In linear measurements, Me (Menton) to MSR (Mid -sagittal reference line), Cd (Condylion) to Me, Ag (Antegonion) to Me, and Ag to MSR were significantly related with facial asymmetric index. 2. In angular measurements, angle Cg -Me plane to MSR, angle ANS -Me plane to MSR, and angle Cd - Ag -Me were significantly related with facial asymmetric index. Especially, angle Cg -Me plane to MSR, angle ANS -Me plane to MSR were highly related. 3. In surface measurements, Cg -Ag -Ag ' and Cd -Ag -Me -Cd ' were significantly related with facial asymmetric index. The results of this study suggested that menton point was the most affected landmark in visual facial asymmetry.

5.The Perception of Facial Profile in Korean.

This study was performed to compare the attractiveness of facial profile among the patients, parents and orthodontists. Six untreated patients between the ages 15 ~25 with Angle 's Class I, Class II, Class III malocclusion were chosen from the Chonnam National University hospital orthodontic department. The images were altered with of the computer to simulate varying degrees of mandibular retrusion and protrusion. The results of this study were obtained as follows: 1. No significant differences in attractiveness of facial profile among the patients, parents and orthodontists. 2. More protrusive lip position was preferred in female than male. 3. More protrusive chin was preferred in orthodontist group than patients, and parents, but there were no significant differences.

7.A Case of B-Cell Lymphoma Combined with Sinusitis in AIDS Patient.

We report an unusual case of primary extranodal non-Hodgkin's lymphoma combined with sinusitis in an AIDS patient. A 39-year-old male presented with sinusitis as initial manifestation. A nasal endoscopy revealed nasal polyposis with mucopurulent discharge in both middle meatus and paranasal sinus (PNS) CT scans demonstrated pansinusitis without lamina papyracea or skull base defect. He received antiretroviral and antibiotics therapy due to low CD4 count representing immunocompromised state. The patient was not followed up for observation and he returned with reduced right visual acuity and diplopia, and proptosis after 6 months. A PNS CT scan showed bilateral sinonasal polyposis with pansinusitis and soft tissue density in the right orbit and frontal lobe through bone defect of lamina papyracea and skull base. Endoscopic sinus surgery for orbital decompression and biopsy of intraorbital mass was performed under general anesthesia. The mass was infiltrated into orbital contents and was confirmed as an diffused large B-cell lymphoma with anaplastic variant. The patient received radiotherapy because his general condition was poor. Proptosis and ocular pain were decreased; however, he complained of a persistent headache. Brain CT and MRI showed diffused parenchymal edema around brain abscess in the right frontal lobe. Stereotactic evacuation of the right brain abscess was performed. He has no specific complaints for the last 14 months and antiretroviral therapy is concluded at the present.

8.The size of upper airway of the snorer in upright and supine position.

The purpose of this study was to compare and evaluate the upper airway structure between the snorers and asymptomatic control subjects depending on the positional change. Lateral cephalograms in the upright and supine position were taken in 25 female snorers and 20 female asymptomatic control subjects, The length and the area of the soft palate, tongue and airway were measured and evaluated statistically. The results obtained were as follows : 1. The snorers showed longer and higher tongue, narrower and longer airway, inferiorly positioned hyoid bone, longer and broader soft palate and narrower hypopharynx than the control subjects both in the upright and supine position. In addition, the snorers showed broader tongue area and narrower oropharynx area than the control subjects in supine position. 2. Depending on the positional change from upright to supine position, the controls and the snorers showed decreased airway length and superior positioned the hyoid bone. In addition, the snorers showed decreased tongue length and height, airway length and thickness and oropharynx area, but increased tongue area and soft palate area.

10.Association of Coronary Artery Disease and Osteoporotic Vertebral Fracture in Korean Men and Women.

BACKGROUND: The association of osteoporotic vertebral fracture or osteoporosis with coronary artery disease (CAD) was investigated in Korean men and women. METHODS: Four hundred consecutive postmenopausal women and men aged 50 years and older, undergoing coronary angiography, were enrolled for the evaluation of established or suspected coronary artery disease. CAD was diagnosed if there was narrowing of > 50% diameter in one or more major coronary artery. Morphometric vertebral fracture was assessed using lateral thoracic and lumbar spine radiographs. Bone mineral density was performed using dual-energy x-ray absorptiometry. RESULTS: Of the 400 subjects in the study (mean age of 61.9 +/- 11.6 years), 256 patients had CAD. Vertebral fracture was observed in 94 (23.5%) patients. There was no difference in vertebral fracture according to the presence or absence of CAD. In logistic regression analysis, vertebral fracture was not significantly associated with CAD after adjustment for multiple risk factors. Although women had lower BMD at any given site than men, BMD was not associated with the presence or absence of CAD among 191 patients. CONCLUSION: Our study demonstrated that osteoporotic vertebral fracture or osteoporosis was not associated with coronary artery disease in Korean men and women.
