1.Relationships Among Pubertal Development, Anthropometric Measurement, Bone Mineral Density in Males and Females 7-23 Years of Age

Background: Maximizing peak bone mass is advocated as a way to prevent osteoporosis. As a prerequisite to the elaboration of any preventive program aimed at rnaximizing peak bone mass, it is important to determine how the rate of skeletal growth at elinically relevant sites, such as lumbar spine(LS), femoral neck(FN), proceeds in relation to age and pubertal stages in both sexes. The present study was performed to measure bone mineral density(BMD) of Korean children and adolescents and to assess the influence of age, sex, puberty and body size on bone mineral density (BMD) during the period of bone growth. Method: Lumbar spine(LS), femoral neck(FN) BMD were measured in 199 healthy males and fernales 7~23 years of age using dual energy x-ray absorptiometry(DEXA), Tanner staging(TS) was used to assess stage of puberty. Results: Anthropometric measurements generally increased with tanner stage in both sexes. Sex differences were observed. In males, compared to females there were significantly higher in height(TS1, TS3, TS4, TS5), weight(TS1, TS3, TS5), dorsal hand skinfold thickness(TS1, TS5), and lean body mass(TS1, TS3, TS4, TS5): while in females, compared to males there were significantly higher in BMI(TS4), skinfold thickness except dorsal hand and fat %(TS3-TS5), and total fat mass(TS3, TS4)(p
2.Morphological analysis of the congenital heart disease associated with infrahepatic interruption of IVC with azygos continuation.

Infrahepatic interruption of the inferior vena cava with azygos continuation is relatively infrequent cardiovascular developmental anomaly occurring both in association with congenital heart disease and as an isolated anomaly of no hemodynamic importance. During the past 5 years, we observed 38 cases of infrahepatic interruption of IVC with azygos continuation out of 2,397 cases of congenital heart disease catheterized at Yonsei Cardiovascular Center. We conducted the study with a view point of position of the heart and abdominal organs and segmental analysis of the underlying congenital heart disease. We also analysed the associated exracardiac vascular anomalies The following results wer obtained: 1) The incidence of this anomaly among congenital heart disease was 1.7% and the sex ratio 1.4:1. Twenty four cases(63.1%) was below 5 years of age and 30 cases (79.0%) had cyanosis. 2) We observed 21 cases (55.2%) with the malposition of the heart and 17 cases (44.8%) with malposition of the abdominal organs. The ventricular loops revealed D-loop in 25 cases, L-loop in 5 cases and in the remaining 7 cases, it was uncertain. Eighteen showed normally related great arteries and malposition was present in another 18 cases. 3) Associatcd cardiovascular anomalics wcre as follows: right-sidcd aortic arch: 12 cases, patentductus arteriosus: 10 cases, pulmonic stenosis: 19 cases and bilateral superior vena cava: 9 cases. In conclusion, the presence of this anomaly should alert one to seek the severeintracardiac anomalies which are usually associated with it. Also, it is essential that, if any patient with this anomaly should come to thoracotomy, the azygos vein be not sacrificed.

3.Clinical Studies on Idiopathic Myocardiopathy.

Twenty-two cases of idiopathic myocardiopathy were observed during the period of 1962 to 1971 and follow up clinical study was made in 5 cases. The criteria of diagnosis was based mainly on exclusive diagnosis in etiology unknown marked cardiomegaly. 1) Male and female ratio of idiopathic myocardiopathy was 1.1:1 and age of onset were distributed from first decade to fifth decade with similar number of cases. 2) Cardinal symptoms were dyspnea, palpitation, chest pain and cough. Common physical findings were protodiastolic gallop (in 2 cases), moist rales (in 3 cases) and hepatomegaly(in 3 cases). 3) Laboratory findings were normal except elevated T.T.T. in one case. 4) Electrocardiogram were abnormal in all cases. There were 3 cases of left ventricular hypertrophy, 3 cases of non-specific ST-T changes and one case of first degree A-V block, intraventricular conduction defect, abnormal Q wave and low valtage, respectively. 5) After medical treatment, symptoms were improved in 4 cases but heart size was reduced in only one case and E.C.G. abnormalities were unchanged in all cases.

4.Assessment of Right Ventricular Pressure by Two-Dimensional Echocardiography in Congenital Heart Disease.

The pressure and resistance of the pulmonary artery are the most important factors when evaluating the congenital heart disease. They are easily measured by the cardiac catheterization, but this procedure is a invasive technique, and difficult to perform repeatedly. There were some reports to estimate the pulmory artery pressure by using non-invasive techniques, such as the echocardiography. The accuracy and the application of the estimate derived from this technique is still a matter of controversy. We estimate the right ventricular pressure by the real time two dimensional echocardiography. The way to measure accuracy for this technique is by of camparing the estimate the right ventricular pressure derived by the echocardiograpy against the right ventricular pressure measured by the cardiac catheterization. The diameter of the left ventricle in the plane of the papillary muscles in measured by short axis view from parasternal positions at the end-systolic phase. The diameter between the interventricular sulcus[B] and the longest vertical from to the interventricular septum[C] are measured. The relationship between the ration, C/B by the two-dimensional echocardiography and the RVP/LVP by the cardiac catheterization is relatively well correlated(r=-0.88). Thus we may conclude that measuring the right ventricular pressure by using the two-dimensional echocardiography is relatively accurate and should be used for evaluating the congenital heart disease.

6.A Case of Ovarian Mullerian Mucinous Papillary Cystadenoma of Borderline Malignancy.

Mullerian Mucinous papillary Cyatadenernas of Borderline tumor(MMBT) is lined by mucinous epithelium of endocervical type and is characterized by papillae architecturally similer to those of serous horderline tumors, It has been described rarely in the literature, Thia case was reported with a brief review of the concerened literatures. It has important clinical and pathological diBerences from mucinous birderline tumors with intestinal differentiation, but has many similatities to mixed epithelial borderline tumora of Mulierian type. Recently, a case of MMBT in a 22 years old woman was experienced at our department. We presented this case with a brief review of literature.

7.Primary Intrapericardial Lipoma Simulating Pericardial Effusion -Report of A Case-.

Primary pericardial tumors are rare than those originating within the myocardium or endocardium and, moreover, primary benign pericardial tumors are much rare in occurrence. Fine(1986) was able to collect only 43 cases of lipoma. Most cases are diagnosed only at autopsy. One fourth of the cardiac lipoma arise subepicardially, where they suggest the presence of pericardial effusion with compression or displacement of the heart. A case of primary intrapericardial lipoma simulating pericardial effusion is presented. This tumor originated in left anterior aspect of pericardium. The tumor was measured 30x30x5cm in size and 2,200mg in weight. Total excision of the mass was accomplished by posterior lateral thoracotomy incision, resulting in complete cure. This is the first case of primary benign lipoma of the pericardium in literatures in our knowledge.

8.Evaluation of EL-ANA/6 Profiles for Specific Antinuclear Antibodies.

BACKGROUND: Identification of specific antinuclear antibodies is useful for the diagnosis, subclassification and determination of prognosis in autoimmune disorders. In many diseases, multiple autoantibodies are detected, and simultaneous detection of multiple autoantibodies has been shown to be useful. Recently, a commercial kit (EL-ANA/6 profiles, TheraTest Laboratories, USA) losing enzyme-linked immunosorbent assay (ELISA) method for detection of six specific autoantibodies is avallable. In this study, we attempted to compare the results of EL-ANA/6 profiles with those of routinely used methods and evaluated usefulness of EL-ANA/6 profiles. METHODS: EL-ANA/6 profiles were performed with 28 sera which were positive for fluorescent antinuclear antibody (FANA) Simultaneously we tested anti-dsDNA antibodies with immnofluorescent (If) method and anti-Sm, anti-RNP, anti-SSA and anti-SSB antibodies with double immunodiffusion (DID). To evaluate specificity, EL- ANA/6 profiles tests were performed on 10 sera from healthy blood donors. RESULTS: Ten sera of healthly blood donors were all negative for EL-ANA/6 pro biles. In the results of EL-ANA/6 profiles on sera positive for FANA, the concordance rate with IF method for the anti-dsDNA antibodies was 89.3% (25/28) and the con- cordance rates with DID for anti-Sm, anti-Sm/RNP, anti-SSA and anti-SSB antibodies were 85.7% (24/28), 82.1% (23/28), 92.9% (26/28) and 82.1% (23/28), respectively. In 16 discordant settings, thirteen (81.3%) were negative on DID and positive on EL-ANA/6 profiles. CONCLUSIONS: The results of the EL-ANA/6 profiles show good concordance rates with If and DID. EL-ANA/6 profiles showing quantitative profiles for multiple autoantibodies is useful for diagnosis and tool)ow-up of autoimmune disorders.

10.Validation of Nursing-Sensitive Patient Outcomes : Focused on Self Care Outcomes.

The purpose of this study was to validate self care outcomes included Nursing Outcomes Classification(NOC) developed by Johnson and Maas at the University of Iowa. A sample of 103 nurse experts working in university affiliated hospitals participated in this study. They were asked to rate indicators that examplified the outcomes on a scale of 1(indicator is not at all characteristic) to 5(indicator is very characteristic). A questionnaire with an adaptation of Fehring's methodology was used to establish the content validity of outcomes. The results were as follows: 1. All indicators were considered to be 'supporting' and no indicators were considered to be 'nonsupporting'. 2. 'Self care: Toileting' attained an OCV score of 0.884 and was the highest OCV score among self care outcomes. 3. 'Self care: Oral Hygiene' attained an OCV score of 0.756 and was the lowest OCV score among self care outcomes. 4. 'Self-care: Activities of Daily Living (ADL)' attained an OCV score of 0.845 and the highest indicator was 'eating'. 5. 'Self-care: Bathing' attained an OCV score of 0.810 and the highest indicator was 'washes body'. 6. 'Self-care: Dressing' attained an OCV score of 0.831 and the highest indicator was 'buttons clothing'. 7. 'Self-care: Eating' attained an OCV score of 0.815 and the highest indicator was 'chews food'. 8. 'Self-care: Grooming' attained an OCV score of 0.833 and the highest indicator was 'combs or brushes hair'. 9. 'Self-care: Hygiene' attained an OCV score of 0.823 and the highest indicator was 'washes hands'. 10. 'Self-care: Insrumental Activities of Daily Living(IADL)' attained an OCV score of 0.776 and the highest indicator was 'uses telephones'. 11. 'Self-care: Non-Parenteral Medication' attained an OCV score of 0.796 and the highest indicator was 'identifies medication'. 12. 'Self-care: Parenteral Medication attained an OCV score of 0.810 and the highest indicator were 'identifies medication' and 'administers medication correctly'. More outcomes need to be validated and outcomes sensitive to Korean culture need to be developed.
