1.A Comparative Study of Cytotoxicity of Topical Antimicrobials to Cultured Human Keratinocytes and Fibroblasts.

The clinical use of topical antimicrobial substances is common in clean wounds as well as contaminated and chronic wounds. This use is based on the observation that the reduction of superficial bacterial contamination promotes wound healing. While the antibacterial efficacy of topical antimicrobials has been well documented, their effects on living tissue and the process of wound healing remain controversial. Recently, there have been several trials to detect the toxicity of various antimicrobial materials by using cultured fibroblasts and keratinocytes. In this study the cytotoxicity of several commonly used antimicrobials and the cytotoxic concentrations of antimicrobials on cultured human keratinocytes and fibroblasts were compared using 3 different METHODS: 1) the MTT method showing the ability of the cells to clear the cytotoxic agents, 2) LDH release indicating damage of cell membrane, and 3) tritiated thymidine incorporation measuring DNA synthesis. In the MTT method all topical antimicrobials in their usual clinical concentration showed cytotoxicity on both keratinocytes and fibroblasts by suppressing cell proliferation. Keratinocytes were more sensitive to certain antibiotics. When the 50% inhibition concentration (IC50) of topical antimicrobials were compared, the cytotoxicity to keratinocyte was, in the decreasing order, chlorhexidine, hydrogen peroxide, povidone iodine, sulfadiazine, fusidic acid, ethanol. Gentamicin and bacitracin. The cytotoxicity to fibroblasts was, in decreasing order, chiorhexidine, hydrogen peroxide, povidone iodine, fusidic acid, ethanol, sulfadiazine, bacitracin, and gentamicin. Most of the antimicrobials tested increased LDH release by keratinocyte in IC50.Chlorhexidine, hydrogen peroxide and povidone iodine showed statistically significant increase in LDH release. The tritiated thymidine incorporation study showed the order of cytotoxicity to keratinocyte in IC50 of each antimicrobials was similar to that of the MTT method. Tritiated thymidined incorporation, however, appeared to be a more sensitive method in detecting cytotoxicity than MTT because the IC50 was lower in the former. In conclusion, all topical antimicrobials are cytotoxic to keratinocytes and fibroblast in their usual concentration. Because of their cytotoxicity it is suggested that one should use topical antimicrobials in a dilution of up to 1 : 1,000 of commonly used concentration for a short period only especially when applied directly on a skin graft or wounds without a protective barrier. Bacitracin or gentamicin appear to be less cytotoxic among all the antimicrobials tested.

3.Effect of Serotonin on Gastric Secretion in the Dog.

Heidenhain pouch secretion in response to small dose of serotonin was studied in conscious dogs. A single subcutaneous injection of 0.5 to 2.0 mg of serotonin produced no changes in spontaneous fasting secretion; however, the milk-induced secretion was greatly inhibited by the same dose. This inhibition was abolished by treatment of dibenzyline or LSD(d-lysergic acid die- thylamide). LSD alone enhanced the response of gastric secretion to milk. Constant intravenous infusion of serotonin, at levels of 3 to 10 microgram/kg/min was associated with a significant increase in the volume of gastric juice aspirated from three anesthetized dogs, but the acidity of juice varied very slightly. However, when histamine is given as much as 0.8 to 3 microgram/kg/min, a marked increase in both the volume and acidity was observed. A significant elevation of mucin content in the juice obtained from the Heidenhain pouch was seen in dogs receiving a single subcutaneous injection of 1.0 mg of serotonin. In case of histamine, the mucin content of pouch juice was not relatively increased and merely an increase in the total amount of mucin secondary to the volume increase was seen. The observed increase in mucin by serotonin was inhibited by LSD, BOL (2-bromo-d-lysergic acid diethylamide) or dibenzyline, and mildly by morphine. Atropine or hexamethonium did not block the response of mucin production to serotonin. The gastrointestinal motility elicited by serotonin was not affected by these agents. It is felt that the receptor(s) responsible for the mucin production in the dog belongs to the D-receptor types postulated by Gaddum and Picarelli.

4.Detection of Methicillin Resistance in Staphylococcus aureus by 2% NaCl Mueller-Hinton Agar and Mannitol Salt Agar.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) has a heteroresistant nature, so methicillin resistance is influenced by various culture conditions, such as temperature, incubation time, and NaCl content in the medium. Mueller Hinton (MH) agar containing 2% NaCl and mannitol salt agar (MSA) with oxacillin disk were evaluated for the detection of methicillin resistance. METHODS: Disk diffusion test on plain Mueller- Hinton (MH) agar, 2% NaCl MH agar, and MSA with 1 microgram oxacillin disk was performed in 70 Stap hylococcus aureus isolates. Oxacillin MIC was determined by E-test. As a gold standard of methicillin resistance, mecA gene was amplified by PCR and detected by agarose gel electrophoresis. RESULTS: Plain MH agar could not detect heterogeneous resistance in 12 S. aureus isolates (18%), but 2% NaCl MH agar and MSA could correctly detect homogeneous and heterogeneous resistance. S. aureus isolates from stool have as much as 48% heterogeneous resistance, while those from non-stool specimen have 5%. CONCLUSION: 2% NaCl and MSA can be used reliably for accurate susceptibility testing of methicillin resistance in routine laboratory.

5.A Case of Cavernous Hemangioma of Stomach.

A 44-year-old woman who comlpained of dizziness and generalized weakness was admitted. The hemoglobin was 6.6g/dL, hematocrit 25.5%, and serum ferritin 2.14 ng/mL Stool occult blood was positive and microcytic hypochromic anemia was found on periyheral blood smear. Gastroscopic examination showed about 2 x 1 cm sized hemispherical sebmucosal tumor on antrum. The patient underwent operatioh for confirmatory diagnosis and treatment. The final pathologic diagnosis of the resected lesion was hemangioma of stomach. Cavernous hemangioma of stomach is a rare disease.Mostly, it has a benign course clinically, but early diagnosis is important because massive hemorrhage and anemia by chronic blood loss can occur. We report a case of hemangioma of stomach with review of literature.

6.Expression of CD133, CD24, CD44 in Cutaneous Malignant Tumors.

BACKGROUND: Based on the unlimited proliferative and self-renewel properties of cancer cells similar to those of stem cells, the idea that cancer may originate from stem cells has been suggested in many different studies and has given rise to cancer stem cell hypothesis. CD133, being normally expressed on the surface of hematopoietic stem cells, has recently been suggested as a marker of cancer stem cells in several malignancies. CD24 and CD44 are membrane proteins reported as markers of various neoplasms. OBJECTIVE: This study was designed to investigate the immunohistochemical expression of the CD24, CD44 and CD133 in squamous cell carcinoma (SCC), basal cell carcinoma (BCC) and malignant melanoma (MM). METHODS: We performed immunohistochemical staining for CD24, CD44 and CD133 using 18 skin cancer tissue samples, including 6 SCCs, 6 BCCs and 6 MMs. The expression of each marker was standardized by the histochemical score (HSCORE). RESULTS: The expression of CD24 showed positive in 1 case of 6 SCCs (mean HSCORE, H; 0.02) and showed negative in 6 BCCs (H; 0.00), 6 MMs (H; 0.00). The expression of CD44 was not observed in 6 SCCs (H; 0.00) but observed in 1 case of 6 BCCs (H; 0.04) and 1 case of 6 MMs (H; 0.03). The expression of CD133 showed positive in 2 cases of 6 SCCs (H; 1.21) and 1 case of 6 BCCs (H; 0.05) and 6 cases of 6 MMs (H; 2.78). CONCLUSION: Our data suggest that CD133 may be a reliable marker of which the higher expression is observed in the more invasive skin cancers and that the existence of cancer stem cells may enhance tumorigenic potential in cutaneous malignant tumors.

7.Empyema Thoracis Resulting from Migrated Acupuncture Needle: Two Cases.

We experienced two cases of empyema thoracis which were caused by migrated acupuncture needle. Two patients were transferred from oriental hospitals because of dyspnea and high fever. Simple chest radiography and computed tomography(CT) of the two cases on admission showed haziness of the lower lung and acupuncture needles in the lung and pleural cavity on admission. Removal of acupuncture needles and decortication were performed.

Nail-patella syndrome is a relatively rare autosomal dominant disorder characterized by dysplastic nail, hypoplastic or absent patella, and dislocation of radial head and iliac horns. In addition, renal abnormalities have been reported. The usual clinical signs of the renal involvement are asymptomatic proteinuria, microscopic hematuria, and in some cases progression to end stage renal disease. We present the case of adult with nail-patella syndrome, who developed proteinuria. Electron microscopy revealed irregular thickening of the glomerular basement membrane with areas of rarefaction, giving rise to a pathognomonic "moth-eaten" appearance.

Docetaxel is a new taxoid antineoplastic drug widely used for advanced breast cancer , ovarian cancer . Skin and nail toxicity are one of the more frequent nonhematologic adverse reactions. Especially, nail changes include dark pigmentation, subungual hemorrahage, acute paronychia, onycholysis, Beau s line etc. We have observed a striking example of multiple Beau s lines from all fingernails and toenails in a man receiving a docetaxel infusion for treatment of metastatic cancer with unknown-origin.
