2.Development of the Somatic Stress Response Scale and Its Application in Clinical Practice.

The objective of this study was to develop the Somatic Stress Response Scale (SSRS), and then to use the scale in clinical practice. A preliminary survey was conducted using 109 healthy adults to obtain somatic stress responses. Then, 215 healthy subjects completed a preliminary questionnaire. A comparison was made regarding the somatic stress responses among 191 patients (71 with anxiety disorders, 73 with depressive disorders and 47 with somatoform disorders) and 215 healthy subjects. Factor analysis of the SSRS yielded five subscales: the cardiorespiratory response, somatic sensitivity, gastrointestinal response, general somatic response and genitourinary response subscales. The test-retest reliability for the five subscales and the total score was significantly high, ranging from .86 to .94. The Cronbach's yen afor the five subscales ranged from .72 to .92, and was .95 for the total score. By correlating the five subscales and the total score of the SSRS with the somatization subscale scores of the Symptom Checklist-90-Revised (SCL-90-R), convergent validity was calculated. The correlations were all at significant levels. Each of the disorder groups was significantly higher in scores of the cardiorespiratory response, gastrointestinal response, general somatic response and genitourinary response subscale, and in the total SSRS score than the healthy group. Only the depressive disorder group scored significantly higher on the somatic sensitivity subscale than the healthy group, and they also scored significantly higher on the genitourinary response subscale than the anxiety disorder group did. These results suggest that the SSRS is highly reliable and valid, and that it can be effectively utilized as a measure for research of the somatic symptoms related to stress. It also implies that somatic sensitivity and genitourinary responses are associated with depressive disorders.

5.Prevalence, Incidence, and Risk Factors of Dementia and Depressive Disorders of the Elderly Residing in the Community: A Two Stage One-year Follow-up Study.

OBJECTIVES: This study is to evaluate the prevalence, incidence, and risk factors of cognitive impairment and depression in the elderly in community. METHODS: This study was a one-year follow-up of Yonchon cohort aged 65 years and over, selected initially in a prevalence study of dementia and depression. A sample of 790 elderly persons who completely interviewed in a prevalence study was re-interviewed with one year interval using the Korean version of Psychogeriatric Assessment Scale (K-PAS). RESULTS: A total of 631 elderly persons was completely interviewed in this follow-up study. The prevalence of cognitive impairment and depression were 21.4% (11.9% in males and 28.3% in females) and 8.4% (6.3% in males and 10.0% in females), respectively. The one-year incidence of cognitive impairment and depression were 11.4% (5.7% in males and 16.2% in females) and 5.1% (3.1% in males and 6.6% in females), respectively. The risk factors of the prevalence of cognitive impairment were education, age, sex, history of stroke, and economic status. The risk factors of the prevalence of depression were family history of depression, history of stroke, marital status, and economic status. The risk factors of the incidence of cognitive impairment were female gander, education, age, alcohol use disorder, marital status, and economic status. The risk factors of the incidence of depression were low education, disrupted marital status, and poor economic status. CONCLUSION: Cognitive impairment and depression were frequent in the elderly. Several risk factors for cognitive impairment and depression were identified. These results suggest that comprehensive health and social services were needed for the elderly.

6.Study of Exoparasites, Rickettsia and Hantaan virus in Bats.

To study the relation between ectoparasite, Rickettsia and hantaan virus in bats, four order of Athropoda were collected from Rhinolophus ferrumequinum captured in Cheju and Eptesicus serotinus captured in Chungnamin from July 1989 to March 1998. Also antibody of Rickettsia and hantaan virus were detected by immunofluoroscent antibody technique and RT-PCR. The results are as follows. 1. Five species of Acarina were identified from E. serotinus: Leptotrobidium subakamushi of Trombiculidae, Macronyssus coreanus, Steatonyssuss spinosuss and Steatonychus superans of Macronyssidae, Argas vespertilionis of Metastigmata. 2. Ischnopsyllus needhami of Siphonaptera and Cimex of Hemiptera were identified from E. serotinus. 3. Cyteribia uenoi and Brachytarsina kanoi of Diptera were identified from R. ferrumequinum. 4. The positive rate of rickettsial antibodies in E. serotinus were 17.58%, 15.15%, 22.22%, 52.73% against R. tsutsugamushi, R. typhi, R. sibirica and R. thai tick typhus, respectively. The high positive rate of antibody related to the high content of Arthropoda. 5. The Positive rate of hantaan virus IFA antibodies were 3.4% (27 of 802) and hanntan virus infection rate 36.7% (22 of 60) by RT-PCR in bats. According these result, we showed that certain species of Athropoda isolated play a role as vector of Rickettsia in E. serotinus. Also bats play a role as a reservoir of hantaan virus in nature.

7.Prevalence of Diabetes Mellitus and Associated Diseases in Yeungnam Province Area.

To know the prevalence of the diabetes mellitus and associated diseases, we analyzed the date of the 3,088 subjects who were examined with the Computed Automated Medi-Screening Test System which consisted of 65 parameters including blood glucose determination fasting and one hour after 100 g of oral glucose load. We grouped the subjects by the modified criteria of National Diabetic Data Group. Followings are the results of the various analysis: 1. The prevalence of diabetes mellitus and impaired glucose tolerance in 2.27% and 18.26% respectively. 2. The prevalence of diabetes mellitus is 2.63% in male and 1.66% in female. There is no statistically significant difference between male and female. 3. There in tendency of increasing prevalence of diabetes mellitus as the age increases. From second to eighth decade, the prevalence of diabetes mellitus increases as 0.0, 0.45, 0.67, 2.28, 3.47, 5.36, 10.00% respectively. 4. There is no statistically significant difference of prevalence of obesity between normal and diabetes: that is, 18.03%, 22.86% respectively (P≥0.1). 5. There is no statistically significant difference of prevalence of impaired glucose tolerance and diabetes between non-obese and obses group (P≥0.1). 6. There is statistically significant increases of frequency of proteinuria, azotemia, hypertension as the glucose tolerance decreases (P≤0.05).

9.Experimental Study on the Effects of Suture and Limited Active Motion on Achilles Tendon Healing

The injured tendon is thought to be healed by, extrinsic healing from tendon sheath and adjacent connective tissue, and intrinsic healing from epitenon and endotenon. About the factors influencing the healing of injured tendon, many authors reported the effects of mobilization, electricity, ultrasound and various druge. Especially, active or passive mobilization had been known to promote the healing process and to reduce adhesion. To investigate the influences of active limited mobilization, intrinsic and extrinsic healing process, right Achilles tendons of 30 New Zealand white rabbits were grouped according to the types of treatments (Group I; tenotomy of Achilles tendon only, Group II; Tenotomy and long leg cast, Group III; surgical repair after tenotomy and long leg cast). The tendon sheath was repaired in all the groups, and the left Achilles tendons were used as control. At six weeks after operation, the tendons were removed and analysed biomechanically using Instron 1000, and histologically. The following results were obtained: 1. There were significant decreased of break strength, in order of Group III, Group I and Group II(p < 0.05). But, no difference was found between control group and Group III. 2. Extension length to the rupture, was decreased significantly in the experimental groups than control group. But there was no difference among the experimental groups. 3. Histologically, dense proliferation of fibroblasts and hypertrophy of tendon was observed in Group I; and matured fibricytic tendon tissues in Group II and III. In Group II, the amount of matured tendon tissues and degree of maturity were less than in Group III. In summary, suture and active limited motion were considered as promoting factors on the tendon healing.

10.A study of the "wits" appraisal of the Korean adults.

The purpose of this study was giving some helpful datas in analysing the degree of anteroposterior jaw disharmony with the "Wits" appraisal. The cephalograms of 105 Korean adults with normal occlusion rangig from 17 to 20 years of age ane 76 Koreans with various malocclusion ranging form 12 to 24 years of age were studied and analyzed statistically. The results were as follows; 1. The mean value of "Wits" appraisal in normal occlusion group wasa -0.82mm in males and -0.17mm in females. 2. There was no significant difference between the males and females in normal occlusion group. 3. The mean value of "Wits" appraisal in Angles class II malocclusion group was 2.67mm and higher than normal group. 4. The mean value of "Wits" apraisal in Angles class III malocclusion group was -8.11mm and lower than normal group.
