2.Sebaceous Carcinoma Occurred in a Patient with Recurrent Bowen's Disease.

Sebaceous carcinoma is a rare malignant epithelial tumor and has diverse clinical presentations as well as a variety of histologic patterns. Although the overwhelming majority of sebaceous carcinomas occur on the eyelid, they may also arise where sebaceous gland and hair are found together, but they favor the head and neck. We report a case of sebaceous carcinoma of the umbilicus in a 68-year-old man who had suffered from a long standing recurrent Bowen's disease of 23 years. The recurrent Bowen's disease in this patient appeared to be closely related to multiple arsenic medication 49 years and 39 years earlier. The excised specimen of umbilicus revealed a well demarcated, 1.5x1 cm sized tumor with grey-yellow hue. The tumor was identified as sebaceous carcinoma, which was characterized by lobular and comedo-carcinoma patterns. The tumor cells showed cytoplasmic foamy vacuolation, nuclear pleomorphism, and high mitotic activity. On an Oil-Red-O stain, the tumor cells were found to contain fine lipid droplets. The intracytoplasmic lipid vacuoles were demonstrated by an electron microscopic examination.

3.Radiologic evaluation of cervical spine fractures

The radiological findings of various cervical spine fractures were analized on the basis of J.Harrisclassification. It appears to be important for the radiologist to be familiar with radiographic findings ofcervical spine fractures, particularly those of unstable fractures which can result in serious medical problems ifimproperly handled in the department of radiology. 68 cases of cervial spine fracures were analized. The resultsare as follows; 1. Stable and unstable fracures were about equal in incedence. 2. Anterior subluxation accountsfor 43.4% of stable fractures, 19% of all fractures. 3. Bilateral interfacetal dislocation accounts for 51.1% ofunstable fractures, 255 of all fractures. 4. Associated fracture were involved in skull, scapula, mandible andfemur. 5. About 80% of the patients is in third through fifth decade.

4.Multiple Sclerosis in Korea: Its Clinical Features and Estimated Prevalence Rate.

It is well-known that multiple sclerosis (MS) has a higher prevalence rate in occidental countries than in Asia and Africa. In addition to their lower prevalence rate of MS, oriental countries may have different clinical characteristics. It has been shown in the previous studies performed in Japan that there is more frequent involvement of optic nerve and spinal cord during the course of disease. These results were also confirmed by the study of MS in Hawaii which compared orientals with caucasians in their clinical features. In Korea, there has been no estimate for clinical features and prevalence rate of MS except two studies. We performed this study to evaluate the clinical features and estimated prevalence rate based on a series of 50 consecutive cases of MS seen over 8 and a half year-period in the Seoul National University Hospital.

7.A Clinical Study of 21 Patients with Lateral Medullary Syndrome.

We reviewed clinicoradiologic findings of twenty-one patients with lateral medullary(Wallenberg) syndrome who admitted at Seoul National University Hospital from March, 1983 to February, 1989. The results are as follows: 1. Lateral medullary syndrome was 14.6% of ischemic stroke in vertebrobasilar territory during the same period. The ages ranged from 35 to 69 years and most of them were in 6th or 7th decades. Z. Detected risk factors were hypertension in 17, previous stroke in 8, hyperlipidemia in 7, and diabetes mellitus in 4. 3. The interval which developed maximal characteristic symptoms after onset were within 5 days in all patients and 2 days in 13 patients. 4. Frequent complaints were dizziness in 21, vertigo in 17, nausea or vomiting in 18, swallowrng difficulty in 14, dysarthria in 12, headache in 11, hoarseness in 7, hiccup in 7, and facial pain in 3, 5. Frequent signs were Horner's syndrome in 21, ataxia in 21, crossed sensory change in 19, soft palate sagging in 15, ipsilateral facial weakness in 9, DTR increase in the same side in 7, tongue deviation in 6, transient diplopia in 5, and transient altered consciousness in 4, 6. Disease courses were considerable improvement in 18, recurrence in 1, and discharge against medical advice in 1. Frequent disturbing complaints during the follow-up period were paresthesia or sensory loss in 16 and ataxia in 11. 7 Among the radiologic studies, MRI could define the lesion at corresponding site in 8 of 9, CT in 3 of 21, and TFCA in 3 of 6. These results suggest that MRI is the best rdiologic study to yeild the lesion in lateral medullary syndrome.

8.A study on enamel thickness of maxillary incisors using X-ray micro computed tomography.

PURPOSE: The objectives of the current study are to assess the accuracy of X-Ray Micro Computed Tomography (microCT) in measuring enamel thickness and to evaluate enamel thickness in maxillary incisors of Koreans. MATERIAL AND METHODS: Five maxillary incisors were embedded in resin block. These teeth were longitudinally sectioned labiolingually through the medial axis. After polishing, the teeth were scanned using a microCT (X-EYE SYSTEM; DRGEM, Seoul, Korea). On a scanning electron microscope (S-4300; Hitachi, Tokyo, Japan) (x20) and a microCT, nearly identical planes were reconstructed. In each tooth, the thickness of labial enamel was measured 1, 3 and 5 mm above the cementoenamel junction (CEJ). Thus, the accuracy of the microCT was evaluated. In addition, using 26 maxillary central incisors and 11 maxillary lateral incisors, in the medial axis and 2 mm remote areas mesially and distally from the medial axis, the thickness of labial enamel was measured 1, 3 and 5 mm above the CEJ along the long axis of the teeth RESULTS: Measurements from nearly identical planes in physical and microCT sections differed by 3.81%. An independent t-test was performed and this showed that there were no significant differences in the measurements between the two methods. Mean values of labial enamel thickness in maxillary central incisors 1, 3 and 5 mm above the CEJ were 0.32 +/- 0.01, 0.50 +/- 0.02 and 0.70 +/- 0.02 mm, respectively. Mean values of labial enamel thickness in maxillary lateral incisors 1, 3 and 5 mm above the CEJ were 0.30 +/- 0.01, 0.55 +/- 0.03 and 0.80 +/- 0.02 mm, respectively. CONCLUSION: In measuring enamel thickness, microCT is one of useful way of measurement. So according to the results of this research, when restoring a porcelain laminate veneer on maxillary incisors in Koreans, careful consideration is needed in the amount of enamel reduction.

10.Compliance and Impact of an ID Physician's Advisory Consults on the Use of Restricted Antimicrobial Agents.

BACKGROUND: Many strategies have been developed to guide more appropriate and cost-effective antibiotic use in hospitals. This study was done to evaluate compliance and short-term impact of targeted antibiotic advisory consult implemented in our hospital since October 2002. Materials and METHODS: A total of 339 prescriptions in 187 patients were retrospectively reviewed and antimicrobial utilization density was compared quarterly from January 2002 through June 2003. RESULTS: Overall, 85.6% of prescriptions were inappropriate, with 73.7% being inappropriate for therapeutic regimens and 100% for prophylactic regimens. Overall compliance was 46.5%. The compliance was higher in the medical services than in the surgical services (64.2% vs. 43.1%, P=0.005) and for therapeutic use than for prophylactic use (54.7% vs. 36.5%, P=0.001). From 1Q 2002 to 2Q 2003, use of aminoglycosides was reduced from 269.1 AUD to 171.8 AUD, and that for antipseudomonal cephalosporins from 112.1 AUD to 64.5 AUD (both, P<0.01). Overall, inpatient use of parenteral antibiotics was decreased from 770.8 AUD to 626.8 AUD (P<0.01). CONCLUSION: Targeted antibiotic advisory consult resulted in substantial reduction in antibiotic use despite relatively poor compliance. This type of antibiotic control program can be an alternative for hospitals with limited human resources trying to implement an antibiotic policy.
