1.Aggressive Angiomyxoma of the Scrotum: A case report.

Aggressive angiomyxoma is a rare, locally aggressive tumor and occurs exclusively in the pelvic and perineal regions of women. But this tumor can occur in men, and the ratio of man to woman is about 1:6. We report a case of aggressive angiomyxoma of the scrotum in a 75-year-old man. The resected mass measured 23 21 10 cm and weighed 1,150 g, and was covered with thick but intact scrotal skin. The cut surface of the mass showed pale yellow to grayish white loose and edematous fibrous tissue with focal cystic degeneration. Microscopically, the mass was composed of loosely arranged bland-looking spindle or stellate cells and variable sized blood vessels with thickened and hyalinized wall. The spindle cells were strong positive for vimentin, focal, weak positive for desmin and smooth muscle actin but negative for S-100 protein. The spindle cells were also positive for estrogen and progesterone receptors.

4.The Effect of Long-term Steroed Therapy combined with Azathioprine in Henoch-Schonlein Nephritis.

We describe cytologic findings of two cases of pilomatrixoma which had been cytologically misdiagnosed as suspicious malignant and malignant lesions in 35 year-old and 22 year-old females who presented a palpable neck mass. The cytologic smears showed many basaloid cells with a high nuclear-cytoplasmic ratio, nuclear hyperchromatism and prominent nucleoli, keratinized squamous cells with pyknotic nuclei, foreign body giant cells, and chronic inflammatory cells in necrotic background. Retrospective view of this aspiration smear revealed that these findings were characteristic features of pilomatrixoma, and the evenly distributed chromatin pattern as well as the lack of nuclear pleomorphism were considered to be the differential points from malignant neoplasm. Pilomatrixoma is a benign neoplasm which should be included in differential diagnosis if the fine needle aspiration cytologic smear of a neck mass or subcutaneous mass of any site showed these features.

5.A Study of As is Patch Test in Cosmetic Contact Dermatitis.

BACKGROUND: Cosmetics are one of the common causes of contact dermatitis and many new cosmetic products are being introduced rapidly into our market. OBJECTIVE: Our purpose was to investigate the as is patch test result by several factors including age, cosmetic product and atopic diseases, and to compare as is patch result with those of standard and cosmetic patch tests. METHODS: The records of 240 patients were reviewed who had been patch tested with their own cosmetics and toiletries with a clinical impression of cosmetic contact dermatitis at the patch clinic of Kangnam St. Mary's Hospital in the period of 1991-1995. RESULTS: Of the 95 patients who were tested with European standard series 54 (56.8%) showed a positive reaction and nickel sulfate was the most common allergen. Eighteen (38.3%) of the 47 patients tested with a cosmetic series revealed a positive reaction and imidazolidinyl urea and dodecyl gallate were the most common allergens. In as is patch test, 99 (41.3%) of the 240 patients showed a positive reaction to 248 (7.3%) of the 3403 cosmetics. Common cosmetic products showing positive reactions were skin care products (26.2%), face make-ups (19.8%), and hair preparations (16.9%). There was no statistically significant difference in the positive reaction rate between atopy and non-atopy patients. The positive correlation rate of as is test with standard and cosmetic series was relatively low. CONCLUSION: Commercial standard patch test series including cosmetic series are not enough to detect causative allergen in cosmetic contact dermatitis. As is patch test should be encouraged to detect new allergens, because many cosmetic ingredients are developed and introduced in market.

6.Pathologic Correlations of Various Indications for Large Loop Excision of Transformation Zone(LLETZ) in Patients with Cervical Lesion.

OBJECTIVE: This study was performed to evaluate the effects of mutiple indications and age on the likelihood of finding cervical lesion in large loop excision of transformation zone(LLETZ). METHOD: We reviewed the medical records of 471 evaluable LLETZ of cervix for cervical neoplasia at the Yonsei University College of Medicine, from April 1996 to August l998. The indications for the LLETZ were grouped as being for treatment(biopsy-proven disease)(indication A), significant discrepancy among cytologic and histologic results(indication B), unsatisfactory colposcopic finding(transformation zone not fully visualized)(indication C), possible microinva- sion(indication D), and combinations of the above. RESULTS: The prevalence rates of cervical lesion in performed LLETZ for indication A alone, for indication B alone, for a combination of indication A and C, for a combination B and C, and for indication D alone were 85.7%, 56.5%, 82.3%, 42.6%, 95%, respectively. The data were then reanalyzed to determine the likelihood of finding high-grade disease on LLETZ specimen. Overall, those with preoperative high-grade cytologic or histologic characteristics were much more likely to have high grade disease(78.1%) than were those with preoperative low-grade cytologic or histologic characteristics(27.2%)(p<0.001). The influence of age on the likelihood of identifying diasease on the LLETZ is undertermined. CONCLUSION: Neither age nor the preoperative grade of disease are good discriminators of the likelihood that disease will be found on a LLETZ specimen, However, patients with preoperative high-grade disease are much more likely than those with preoperative low-grade disease to have high-grade dysplasia or invasive cancer on LLETZ.

7.COVID-19 Public Health Measures During National Assembly Elections of the Republic of Korea

The general elections for the 21st National Assembly in the Republic of Korea were scheduled for April 15th, 2020, which was during the novel coronavirus disease (COVID-19) outbreak. To ensure a safe election, the Korean Centers for Disease Control and Prevention (KCDC) recommended several public health measures. The KCDC developed key interventions after reviewing the general election strategy that targeted COVID-19 patients and individuals isolating at home. Four voters who participated in the election tested positive, but did not contract COVID-19 during voting. The results demonstrated that the KCDC minimized the spread of infection in the community during the election. The measures implemented by KCDC during the election held under a COVID-19 outbreak cannot be generalized to elections as a whole because cultural and national consciousness vary between countries. Nevertheless, it demonstrates that the systemic strategies and applications against the pandemic can minimize the possibility of viral spread.
8.Sequence Generation and Genotyping of 15 Autosomal STR Markers Using Next Generation Sequencing.

Recently, next generation sequencing (NGS) has received attention as the ultimate genotyping method to overcome the limitations of capillary electrophoresis (CE)-based short tandem repeat (STR) analysis, such as the limited number of STR loci that can be measured simultaneously using fluorescent-labeled primers and the maximum size of STR amplicons. In this study, we analyzed 15 autosomal STR markers via the NGS method and evaluated their effectiveness in STR analysis. Using male and female standard DNA as single-sources and their 1:1 mixture, we sequentially generated sample amplicons by the multiplex polymerase chain reaction (PCR) method, constructed DNA libraries by ligation of adapters with a multiplex identifier (MID), and sequenced DNA using the Roche GS Junior Platform. Sequencing data for each sample were analyzed via alignment with pre-built reference sequences. Most STR alleles could be determined by applying a coverage threshold of 20% for the two single-sources and 10% for the 1:1 mixture. The structure of the STR in each allele was accurately determined by examining the sequences of the target STR region. The mixture ratio of the mixed sample was estimated by analyzing the coverage ratios between assigned alleles at each locus and the reference/variant ratios from the observed sequence variations. In conclusion, the experimental method used in this study allowed the successful generation of NGS data. In addition, the NGS data analysis protocol enables accurate STR allele call and repeat structure determination at each locus. Therefore, this approach using the NGS system will be helpful to interpret and analysis the STR profiles from singe-source and even mixed samples in forensic investigation.

9.Antimicrobial Effects of Nano-Silver Gauze against Common Bacterial Isolates.

BACKGROUND: Silver has been used for disinfection and sterilization. We aimed to confirm the in-vitro antibacterial effects of nanocrystalline silver-coated gauze. METHODS: Fourteen clinical isolates each of Escherichia coli and Acinetobacter baumannii were used. Bacterial suspensions made in tryptic soy broth were exposed to Ordinary and silver-coated gauze. Bacteria were then harvested from the gauze immediately and after 24 h incubation, cultured on blood agar plates and eunmerated for viable counts. The number of colonies was converted into common logarithms for comparison. RESULTS: The number of colonies recovered from silver-coated gauze was significantly lower than those recovered from ordinary gauze when harvested immediately after exposure (E. coli, 3.06 vs 1.73; A. baumannii, 3.13 vs 1.98; P<0.001). After 24 h incubation of exposed gauze, silver-coated gauze produced less than 1 CFU/mL, whereas ordinary gauze produced a number of colonies significantly higher than it did immediately after exposure (E. coli, 4.13; A. baumannii, 4.46; P<0.001). Conclusion: Compared with ordinary gauze, silver-coated gauze was shown to have 99.99% antibacterial effect.

10.CT diagnosis of primary lung cancer coexisting with pulmonary tuberculosis.

When bronchogenic carcinoma is coexisting with pulmonary tuberculosis, it is difficult to differentiate bronchogenic carcinoma from pulmonary tuberculosis radiologically. Thus, the object of this study is to define differential diagnosis of bronchogenic carcinoma by computed tomography. We analized CT scans of 27 patients with radiologic findings of pulmonary tuberculosis and mass of which twelve cases were pulmonary tuberculosis and fifteen cases were primary lung cancer. The location of parenchymal infiltration and the mass was the same in 60%(9/15) of the primary lung cancer in cases and 83%(10/12) of the pulmonary tuberculosis cases. The common location of the mass was the both upper lobes in 92%(11/12) of the pulmonary tuberculosis cases and 53%(8/15) of the primary lung cancer cases. The common locations of the mediastinal lymphadenopathy were 4R, 2R of the pulmonary tuberculosis cases and 4R, 10R of the primary lung cancer cases. In the feature of post enhanced lymph nodes, homogenous increased density was more frequent in primary lung cancer. Measurements of the maximum thickness part of the cavity wall was not a reliable indication of malignancy.
