1.A Case of Erythrodermic Form of Mycosis Fungoides.

We report a case of an erythrodermic form of mycosis fungoides in a 68-year-old male, who showed generalized erythroderma with scales for 3 months. Lymphadenopathies in the inguinal and neck areas were present. Histopathological findings showed epidermotropism, perivascular atypical lymphocyte infilteration in the upper dermis, and dermatopathic lymphadenopathy in the inguinal lymph node. T cell markers were positively stained in the immunohistochemical study. These clinicopathological features were consistent with an erythrodermic form of mycosis fungoides(TNM IIl). The patient was treated with PUVA therapy and low dose chemotherapy.

2.Bone Marrow Necrosis in CD7 positive Acute Myeloid Leukemia.

Bone marrow necrosis is infrequently diagnosed during life, and its presence often signifies a poor prognosis. It has been associated with a variety of disease, including acute and chronic leukemia, carcinoma, malignant lymph oma, infection and sickle cell disease. About 5-26% of acute myeloid leukemia has been reported to express lymphoid differentiation markers, of which CD7 is ex pressed very early during T-cell ontogeny. A 46-year-old male complaining severe bone pain had pancytopenia, leukoerythroblastosis and bone marrow necrosis. Peripheral blood immature cells expressed CD7 as well as myeloid markers such as CD13 and CD33 on immunophenotypic studies. We report a case of CD7 positive acute myeloid leu kemia associated with bone marrow necrosis, confirmed by bone marrow biopsy and immunophenotypic study.

3.Clinical Nurses' lived Experience of Interpersonal Relations in the Ward Setting of the hospital.

PURPOSE: The purpose of the study was to describe the essential structure of the lived experience of clinical nurses' interpersonal relations among nurses, patients, and others in the ward setting of the hospital. METHOD: Six nurses who have experienced from 4 to 7 years on the same ward setting, were interviewed. The data were collected from September, 2000 to May, 2001 and analyzed using Colaizzi's (1978) method of phenomenology. RESULT: In this study, 7 themes were extracted: difficulty of interpersonal relations after being familiar with work, developing good relations with doctors, patients, and their significant others as experience increased, generation gap among individual nurses, evaluating other nursing colleagues on their past experience in ward settings, avoiding nurses with whom one was in conflict, sometimes, resolving conflict through getting together with colleagues informally, having a limited interpersonal network, experiencing becoming mature through struggling with the difficulty of interpersonal relations. CONCLUSION: Nurse managers need to provide resources, opportunities, and information to clinical nurses through fully understanding the characteristics of nurses' interpersonal relations. In addition, they should minimize the factors which intervene with good interpersonal relations among clinical nurses.

4.Distal Radius Fractures Using Acu-Loc Volar Plate.

OBJECTIVE: We determined the radiographic outcome of distal radius fractures with Acu-loc volar plate. METHODS: Forty-one patients were recruited between August 2009 and September 2010. There were 10 males and 31 females, with a mean age 61.2. Fractures were radiologically classification the Frykman. Fifteen fractures were group 8, eight were group 7, ten were group 6, four were group 5 and four were group 3. Distal part of the fractures was dorsally inclined in 37 wrists and inclined to volar side in four wrists. Lateral and anteroposterior radiographs taken after operated day, the fracture were compared with radiographs of the injured wrist and the differences in palmar tilt, ulnar variance; radial height; radial shift and radial inclination were measured. RESULTS: There was significant improvement in the measurements of radial height, radial inclination, volar tilt, ulnar variance and radial shift postoperatively. The radial height improved from an average of 8.5 mm (range, 3 to 15 mm) to 11.0 mm (range, 8 to 15 mm), the radial inclination improved from an average of 21.5 degree (range, 10 to 40 degree) to 28.1 degree (range, 19 to 44 degree), the palma tilt improved from an average of 12.9 degree (range, 6 to 22 degree) to 17.2 degree (range, 7 to 27 degree), the ulnar variance improved from an average of -2.3 mm (range, -6 to 4 mm) to 1 mm (range, -3 to 7 mm) and the radial shift improved from an average of 18.7 mm (range, 15 to 26 mm) to 17.3 mm (range, 12 to 21 mm). CONCLUSION: Acu-loc volar plate is a safe and effective device.

5.Leukocytoclastic Vasculitis of the Breast: A Case Report

Leukocytoclastic vasculitis is a rare disease characterized by neutrophil and immune-complex deposition in the small vessel walls. We report a 47-year-old female patient with leukocytoclastic vasculitis of the breast, presenting as breast edema on mammography, irregular hypoechoic lesion with central necrosis on ultrasonography and regionally distributed heterogeneous non-mass enhancement on magnetic resonance imaging.
6.Papillary Apocrine Metaplasia of the Breast Mimicking Papillary Neoplasm: A Case Report

Apocrine metaplasia is a benign fibrocystic change characterized by dilated acini lined by columnar cells with apocrine features. These apocrine-like cells form papillary clumps of cells extending into the cystic space called papillary apocrine metaplasia. The researchers report a case of a 48-year-old woman who presented with a palpable mass in the right breast. Breast ultrasonography and MRI showed a large complex cystic and solid mass. A differential diagnosis included intraductal papilloma or intracystic papillary carcinoma. She underwent surgery and the final diagnosis was papillary apocrine metaplasia.

High load is concentrated on Satba wearing area of Ssireum athletes during the tournament. Muscular injuries in this area may seriously affect the athletic performance. We report a case of a 21-year-old Ssireum athlete who was experiencing pain in his left thigh (Satba wearing area) during a tournament due to the migrating mass effect of a partial ruptured semitendinosus muscle occurred approximately 6 months before. Herein is described the clinical results after surgical intervention with an added review of the relevant literature.

8.Diagnostic Experience in the 3 Human Brucellosis Cases by the Microbiologic, Serologic and Gene Tests.

Brucellosis is a zoonosis caused by Brucella species. B. melitensis, B. suis, B. abortus and B. canis can infect humans. Recently, as the cases of bovine brucellosis have increased every year in Korea, the cases of human brucellosis have also increased among livestock workers and veterinarians in rural areas, since the first human case was reported in 2003. Because clinical manifestations of the disease are nonspecific and may be very atypical, clinicians and laboratory persons need to be active in using diagnostic tools including polymerase chain reaction in addition to the ordinary culture and serologic tests, and taking an appropriate measure to prevent intralaboratory infection. We report herein our experience in three human brucellosis cases diagnosed by cultures, serologic tests and gene detection.
