1.A Case of Allergic Contact Dermatitis to Laureth 9.

Laureth 9 is a nonionic surfactant and it is used as a sclerosing agen; in the treatment of esophageal and gastric varices and a local anesthetics. We report a 20-year-old male patient presenting with features of alergic contact dermatitis due to laureth 9 used in the oxoralen, ointment.

2.A clinical and histologic study of 71 cases of basal cell carcinoma.

A clinical and histologic survey was done on 71 cases of basal cel carcinoma(BCC) filed at Department of Clinical Fathology, Kang Nam, Han Kang, and Kang Dong Sacred Heart Hospital during past 13 year s from 1979 to 1991. The results were summerized as follows: 1. Of all malignant skin tumors BCC accounted for 31.6%. The frequency of BCC in the total number of outpatients visiting the Department of Dermatology was 0.06%. 2. The ratio of men to women with HCC was 1:1.03. 83.8% of the pat.ients with BCC were older than the age of l0 years with the mean age of 56.9 years. 3. 83.8% of the BCC appared in the face, especially on the nose(25.4%), eyelid(20.3%). 4. On the basis of classification of Lever et al, the solid type(66.7% ) was the most com mon histologic pattern, followed by the adenoid(8.8%), pigmented(8.8%), and etc. By the classification of Farmer et al, the nodulocystic type(35.1% ) was the most common histologic pattern followed by infiltrative(31.6%), adenoid(8.8%), pigmented(8.8%), and etc. By the classification on Sexton et al, in the order of decreasing frequency, liistologic subtypes were the nodular(43.9%), mixed (22.8%), infiltrative(19.3%), supeficial(3.5%), morpheic(3.5%), and micronodular(3.5%).

3.A case of Kikuchi's disease(subacute necrotizing lymphocytes).

Kikuchi's disease is a self-limiting lymphadenitis, predomin;intly of young women who present with cervical lymphadenopathy. We present a case of Kikuchis disease in a 18-year-old female, representing multiple tender subcutaneous mass on her neck, left eyelid and posteriarcuricle. Histopathologically, the biopsied mass was a lymph node showing architectural effacement by necrotic focicomposed of nucear karyorrhexis and mononucl ar cell proliferation.

4.Spectral Analysis of Blood Pressure and Heart Rate Variability during Hemorrhage after Losartan Administration into the Lateral Ventricle.

This study was aimed to elucidate the effect of intracerebroventricular losartan administration on arterial pressure regulation during hemorrhage in rats by power spectral analysis of blood pressure and heart rate variability. Nineteen male Sprague-Dawley rats weighing 240-300g were divided into losartan-administered(n=10) and control(n=9) groups. Hemorrhage was induced with a withdrawal pump from the femoral artery at 3ml/kg/min for 5min. Arterial presure was measured with a pressure transducer connected to the contralateral femoral artery for 5min before, during and after hemorrhage. The blood pressure signal digitized at 500 Hz through a data acquisition system was analyzed with fast Fourier transform algorithm to yield power spectra of systolic(SP) and diastolic(DP) blood pressure and instantaneous heart rate(HR). Powers of very low frequency (VLF, 0.02-0.26 Hz), low frequency(LF, 0.26-0.75 Hz) and high frequency(HF, 0.75-5.00 Hz) band were obtained. Basal systolic and diastolic blood pressure was 149+/-9 and 99+/-2mmHg, respectively, and was not changed by hemorrhage in control rats. Basal blood pressure in losartan group was 143+/-9 and 97+/-6mmHg and was significantly lowered to 116+/-13 and 77+/-9mmHg, respectively. HR was significantly increased during and after hemorrhage in both groups. Total power of SP variability in losartan group was 13.9+/-3.2mmHg2 before hemorrhage and was significantly increased to 66.6+/-25.3mmHg2 during hemorrhage. VLF, LF and HF powers of SP variability were 7.3+/-2.0, 3.8+/-1.1 and 2.8+/-0.7mmHg2, respectively, in losartan group and 5.5+/-1.4, 3.7+/-1.5 and 2.8+/-0.8mmHg2 in control rats. VLF and HF powers of SP were increased to 33.0+/-15.2 and 20.3+/-6.4mmHg2 in losartan group, and VLF power was increased to 7.9+/-1.5mmHg2 in control group. VLF power of DP variability increased from 3.3+/-0.9 before hemorrhage to 5.9+/-1.0mmHg2 during hemorrhage in control group. Powers of DP variability in losartan group and those of HR variability in both groups were not changed by hemorrhage. The above results suggested that losartan aggravated the arterial pressure fall during hemorrhage by impairing the sympathetic nerve activation by central angiotensin II.

5.Minimum Flexion Angle of the Knee Joint during Femoral Tunneling and Interference Screw Fixation in Endoscopic ACL Reconstrution.

Isometric positioning of the ACL graft is an important consideration in successful reconstruction of the ACL-deficient knee. The purpose of this study is to get a certain guideline in the endoscopic one-tunnel technique of anterior cruciate ligament reconstruction by measuring the skin angle and determine the degree of minimum flexion of the knee joint during femoral tunneling and interference screw fixations. To get the guide lines, first we get the tibial tunnel angle parallel to the Blumensaat's line from fully extended lateral knee joint radiography. Secondly measure the differences between angles of the femur-tibia shaft and anterior thigh-leg skin. Then measure the minimum femur-tibia flexion angle does not perforated the posterior cortex of the distal femur during femoral tunneling. Intraoperative measuring the angle between interference screw guide pin and tibial tunnel to get the parallelism of the femoral tunnel and interference screw. The results were as follows; The average femur-tibia shaft angle with 30degrees anterior thigh-leg skin angle was 30.2+/-1.75degrees, with 45degrees was 45.2+/-1.23degrees, with 60degrees was 61.9+/-4.23degrees, with 75degrees was 78.6+/-2.62degrees, with 90degrees was 97.8+/-3.96degrees. Predetermined sagittal tibial tunnel vector on the 0degrees extension knee joint lateral radiographs were applied to the several knee joint dynamograms. The mean minimum flexion angle of the femur-tibia shaft that doesn't perforate the posterior cortex of the femur was 45+/-1.58degrees (male), 44.5+/-4.97degrees (female). The average angle between interference screw guide pin and tibial tunnel was 23.0+/-2.23degrees. The findings of the present study suggest that anterior thigh-leg skin angle can be used instead of the true femur-tibia shaft angle. Less knee flexion angle makes good arthroscopic view during the tibio-femoral tunneling and interference screw fixation.

7.The Effect of Ultraviolet lrradiation on Susceptibility of Listeria monocytogenes lnfection and the Production of Cytokines.

Although the effect of ultraviolet (UV) irradiation on immune response was reported to supress cellular immune response, the exact mechanism was not elucidated. As recent development in cytokine research progress, it is well-known that immune response is regulated by cytokines and especially cellular immune response is induced by interferon (IFN)-r and interleukin (IL)-12 which is mainly produced from lymphocytes and macrophages respectively. Therefore our purpose was to elucidate the UV effect on cellular immune response and its mechanism. We have investigated the changes of host resistance by injection of Listeria monocytogenes which is an intracellular parasite after UVB irradiation in C57BL/6 mice which is known to have relatively strong cellular immune response. In addition we also have investigated the changes in the production of IFN-r from lymphocytes and the production of tumor necrosis factor (TNF)-a and IL-12 from macrophages in mice by UVB irradiation. The increase of mouse spleen index and susceptibility of iisteria monocytogenes infection was correlated with the decreased production of IFN-r, TNF-a and IL-12, which was known to induce the suppression of cellular imrnune response.

9.A case of soft fibroma of the vulva.

We report a case of soft fibroma occured in a 27-year-old female. The lesion was multiple, pea-to walnut-sized, baglike, pedunculated growth on the vulva. The histopathologic findings of excisional biopsy specimen evaled a slightly t.hinned epidermis and loosely arranged collagen fibers in the dermis.
