1.Effect of Method of Illumination on the Visual Contrast Sensitivity Funetion.

To find out the good visual environment for reading, the methods of illumination were studied using Arden Contrast Sensitivity System. Among various factors for comfortable reading condition such as air condition, temperature, humidity, noise and light, the intensity and characters of illumination were the most important factors for eye health. If the character of illuminations are fairly good, to make the brighter condition is better for eye hdalth. But in artificial illumination, there are so many limitations for supplying light of good characters. So we set the luminosity on the desk to 500 lx in which tbe best visual acuity presented, and checked the contrast sensitivity by changing the luminosity of the room. The results suggested that the condition in which luminosity on the desk was higher twice than the room was favorable for reading.

2.The difference of interieukin - 4 and interferon - r production of Der p I stimulated T cells and effects of immunomodulator in house dust mite sensitive atopic and non - atopic individuals.

BACKGROUND: Allergic diseases are characterized by immediate - and late - phase reactions to various allergens by the selective activation of a subset of CD4 + T cells. In response to allergen, T cells isolated from atopic donors are biased to low levels of IFN - y and high levels of IL - 4, and vice versa by T cells from non - atopic donor. Objective : The aim of this study was to evaluate the patterns of IL - 4 and IFN - y production after Der p I stimulation and the effect on the cytokine production from T cells by budesonide, disodium cromoglycate and cyclosporin A in atopic and non - atopic individuals. MATERIAL AND METHOD: Seven Der p I specific atopic and 7 non - atopic individuals were selected. We decided the 50% inhibiting concentration of each immunomodulator by lymphocyte proliferation assay, and measured their effects on the cytokine production in vitro by intracellular IL - 4, IFN - y staining and flow cytometry. Results and CONCLUSION: There was significant difference on stimulation index ( SI ) of production of IFN - y as well as IL-4 after Der p I stimulation between atopic and non-atopic individuals ( IL - 4 ; 1.57 +/- 0.7 : 0.98 +/- 0.2, p = 0.026, IFN - r : 1.45 +/- 0.5 : 0.95 +/- 0.2, p = 0.048 ). The synthesis of IL - 4, and IFN - r were significantly inhibited after the stimulation of every immunomodulators in atopic individuals, DSCG couldnt inhibit IL - 4 and IFN - r in nonatopic individuals. There was no significant difference in the inhibiting effect of these immunomodulators in both of them.

3.The Factors Associated with Becoming Obese Children: in 6th Grade Children of Elementary Schools in Busan.

BACKGROUND: The purpose of this study was to find the factors associated with becoming obese children during the past 2 years in 6th grade children of elementary school and then to evaluate the high risk groups for childhood obesity. METHODS: The 738 children from May to June, 2001 in 6th grade of 5 elementary schools in Busan were the subjects of this study. The children's height and weight in 4th grade and 6th grade were obtained from school health records. We calculated the body mass index in 4th and 6th grades and classified into 3 weight groups (normal, at risk for obesity, and obese) according to the age and sex-specific BMI established by the Korean Academy of Pediatrics. If children's weight group in 4th grade was changed to heavier weight group in 6th grade, we defined it as increased group. Otherwise, we defined it as non-increased group. The association between the increase in weight group and parental and children's characteristics were analyzed by x2-test, x2-test for the trend, t-test, ANOVA and multiple logistic regression. RESULTS: Among the total, 3.3% of children with normal weight and 16.9% of children at the risk for obesity in 4th grade were transformed to increased group in 6th grade, respectively (P<0.001). Children who had excessive appetite (P<0.001), watched TV greater than or equal to 2 hours per day (P=0.004) or had an obese father (P=0.044) were more likely to become increased group. After adjusting for father's obesity, appetite and TV watching time in children, the odds ratio of transforming to increased group after 2 years in children at risk for obesity or in obese child in 4th grade was 2.5 (95% CI: 1.09~5.55) compared to children with normal weight. Children having excessive appetite were 3.2 times (95% CI: 1.42~7.01) more likely to become the increased group after adjusting for father's obesity, TV watching time and weight in 4th grade. Children watching TV greater than or equal to 2 hours/day was 2.8 times (95% CI: 1.27~6.18) more likely to become increased group after adjusting for father's obesity, appetite and weight in 4th grade. CONCLUSION: The risk of becoming obese was higher in children at risk for obesity or obese children in those with excessive appetite or those who have watched TV> or =2 hours/day for the past two years. These findings suggest promotion of educational plan to prevent childhood obesity.

Behcet's disease is a systemic vasculitis that is characterized mainly by recurrent oral and genital aphthous ulcers, uveitis, and skin findings. Its neurological manifestations are well recognized. Recurrent meningitis in Behcet's disease is exceptional. We describe herein the case of a 31-year-old man who presented with recurrent aseptic meningitis without any specific cause. A few years later he developed oral and genital ulcers, and uveitis. Behcet's disease should always be considered in a differential diagnosis of recurrent aseptic meningitis without viral infection, particularly in the context of multisystem manifestations.

5.The Clinical Effectiveness of Trabeculectomy in Glaucoma.

Thirty-three trabeculectomies were performed in various types of glaucoma during the periolf of May 1976 to May 1977. Mishima's modified method was applied as a surgical technique following the possible medical treatment controlling the normal I.O.P. before operations: the operations were performed without surgical microscoping in the most cases. Success was achieved in 26 (78.8%) of the 33 cases. Trabeculectomy was very effective in all pur cases of 5 chronic simple glaucoma and 3 chronic narrow angle glaucoma. However, in acute narrow angle glaucoma, we gained the good results in normotensive cases following medical treatment before operation, the results were not good in the cases that I.O.P. was not controlled despite of the vigrous medical treatment. It seemed that trabeculectomy was more advisable operation than goniotomy in long standing congenital glaucoma and in previously failed case with goniotomy. Success was achieved in 5 of 6 congenital glaucoma(4 previously failed with goniotomy and 1 with very severe buphthalmos). It seemed that, in the previously operated eye(e.g. in failed case with other glaucoma surgery or in aphakic glaucoma) the sound lower half was rather preferable to the upper half for operating site.

6.ERG b/a ratio and retinal circulation time of CRVO.

Fourteen cases of central retinal vein occlusion (CRVO) were studied with electroretinogram (ERG) and fluorescein angiogram. The cases were divided into a venous stasis retinopathy group (VSR,9 cases) and a hemorrhagic retinopathy group(HR, 5 cases). The b/a ratio and retinal circualtion time (RCT) were measured and compared with the control group. The mean b/a ratio of the HR group (0.86) was decreased as compared with the VSR group (1.18) and the control group (1.23). The RCT of the HR group was markedly delayed to 13.68 seconds as compared with the VSR group (11.09 sec) and the control group (6.4 sec). These facts suggest that both the b/a ratio and the RCT are possible parameters for estimating retinal ischemia and that the ERG is a reliable examination method for classification of CRVO.

7.The Intracho roidal Changes in Harada`s Disease.

The intrachoroidal changes in Harada's disease were studied by indocyanine green(ICG)angiography. ICG angiography using confocal laser scanning ophthalmoscope (Heidelberg Retina Angiograph, HRA)and fluorescein angiography were performed in 12 eyes of 6 patients during the acute stage before the initiation of corticosteroid treatment and recovery stage, and the findings were evaluated retrospectively. During the acute stage, early phase HRA images disclosed a dark background fluorescence in all eyes due to the diffuse filling delay and the vague contour of choroidal vessels. In the midphase, Intrachoroidal hyperfluorescence resulting from vascular leakage and scattered hypofluorescent spots were intermingled, and in some eyes patchy hypofluorescence representing focal filling defect was observed. These choroidal changes could be observed before any visible retinal changes appeared. During the recovery stage, choroidal filling was more rapid and choroidal vessels were more clearly visible. However the scattered hypofluorescent spots due to the blockage by the inflammatory precipitates were still observed in small numbers, and in some eyes focal vascular leakage remained. According to these findings, we hypothesize the pathogenesis of Harada`s disease as follows. Severe inflammatory precipitates and vascular leakage can make the choroid edematous with highly viscous fluid. It adds mechanical damage to choroidal vessels already injured by the inflammatory process, leading to reversible circulatory disturbances. The retinal pigment epithelium and the sensory retina may be damaged secondarily.

8.Evaluation of Confocal Laser Scanning Indocyanine Green Angiography Images of Occult Choroidal Neovascularization.

For the interpretation of the confocal laser scanning indocyanine green angiography(Heidelberg Retina Angiograph, HRA) in patients with choroidal neovascularization(CNV) secondary to age-related macular degeneration(AMD), we analyzed the early and the late phase images comparatively. HRA was carried out in 41 eyes of 36 patients that revealed occult CNV or undetectable CNV on fluorescein angiograms. In all patients, the image qualitites of HRA was excellent in the early as well as in the late phase studies and CNV was detected in 38 eyes(93%). In 25 eyes(66%), we could get all the possible information of CNV with the early phase images which revealed CNV as fine neovascular networks or vascular structures of lnear or dot pattern usually surrounded by hypofluorescent margins. And when the hypofluorescent margin surrounded the neovascular structure completely, a well-defined CNV could be diagnosed. In 9 eyes(24%), more accurate evaluation of the nature, size and geometry of CNV could be made with the help of more late phase images. In these cases, as the initially identified neovascular structures were stained in the late phase, more intensely leaking portions or more widespread minimally stained areas could be detected. In remaining 4 eyes(10%), CNV could b detected in the late phase without apparent evidence of vascular structure in the early phase. Our study suggests that careful reading of the whole sequence of ICG angiograms and comparative analysis of the early and late phase images are required to get the clinically useful information from HRA.

10.Comparative Analysis of Prescription for Splitted Tablet using the HIRA-NPS (Health Insurance Review & Assessment Service-National Patient Sample)

OBJECTIVE:
The aim of this study was to analyze the status of split tablet prescription in South Korea.
METHODS:
We conducted this analysis using 2016 National Patient Sample data from the Health Insurance Review and Assessment Service. We computed split tablet prescription rates by sex and age and determined which medicine and medical specialties had the highest split tablet prescribing rates.
RESULTS:
The proportion of prescriptions that included split tablets was 15.6% (n=6,687,35). The proportion of prescriptions that included split tablets was higher for females (56.7%) than for males (43.3%), while that of prescriptions including split tablets versus total prescriptions for each sex was higher for males (16.4%) than for females (14.9%) (p<0.001). In the age group under 19 years, the proportion of prescriptions including split tablets (53.7%) was more than half of the total. The highest tablet splitting rate was found to be 89.9% for formoterol fumarate (40 µg), and pseudoephedrine hydrochloride (60 mg) had the highest number of prescriptions. Pediatrics (65.6%) was the medical field with the highest rate of split tablet prescription.
CONCLUSION
Split tablets were most prescribed to pediatric patients. To minimize the use of split tablets, it is necessary to develop lower dose tablets and establish a policy that promotes prescription of these lower-dose tablets.
