1.Primary Teratocarcinoma of the Pineal Gland: A case report.

Teratocarcinoma is a form of mixed germ cell tumor composed of a mixture of teratoma and embryonal carcinoma. It is rarely found in the brain, but when present it is most commonly found in the pineal region. We investigated a case of primary teratocarcinoma in the pineal region. The patient was a 10 year old boy who had suffered from a bitemporal headache for one month. Brain CT and MRI revealed a rather well defined ovoid heterogenous mass in the pineal gland region, measuring 4.3 x 3.8 x 3.0 cm in size. Microscopically the tumor contained areas of immature teratoma and embryonal carcinoma. Immunohistochemical staining revealed positive reactions for cytokeratin, epithelial membrane antigen, carcinoembryonic antigen, and alpha-fetoprotein in the embryonal carcinoma component.

3.Bile Duct Stenosis & Intrahepatic Stones after a Transcather Hepatic Arterial Embolization: A case report .

Transcatheter hepatic arterial embolization (THAE) is one of the treatment modalities that can be applied to hepatocellular carcinomas (HCC) and metastatic tumors of the liver. Complications such as cholecystitis and gallbladder necrosis, are common with THAE, but liver and peripheral bile duct necrosis are rare, and intrahepatic stones with main bile duct necrosis have never been reported. To prevent intrahepatic spread during operative manupulation and to decrease the vascularity and size of the tumor, we performed a THAE on a huge-sized HCC five times before performing the hepatectomy. We succesfully undertook a right lobectomy after the THAE with lipiodol, gelform, and adriamycin. However, severe bile duct stricture and intrahepatic stones were confirmed during the operation.

4.Antimicrobial Resistance in Escherichia coli Isolated from Healthy Volunteers of the Community.

BACKGROUND: We monitored the prevalence of antimicrobial resistance and the pattern of multiple drug resistance in Escherichia coli isolated from healthy people in the community. METHODS: We performed antimicrobial susceptibility testing on 491 isolates of E. coli from 692 healthy people in Incheon from February to July in 2006. The results were interpreted according to the CLSI guidelines. RESULTS: The highest rate of resistance was observed against tetracycline (46.6%), ampicillin (41.1%), ticarcillin (37.9%), streptomycin (31.0%), and nalidixic acid (23.6%). Twenty six percent of isolates were observed to be resistant to five or more of the antimicrobials tested. CONCLUSION: In order to maintain a low level of antimicrobial use and resistance, the surveillance of antimicrobial resistance in the community would be very important, as it provides epidemical data to set up and control antibiotic guidelines and serves as an early warning for resistance in pathogenic bacteria.

5.The Effect of External Levator Resection in Blepharoptosis with Poor Levator Function.

The postoperative complications of frontalis sling for severe blepharoptosis with poor levator function have included undercorrection, overcorrection, exposure keratitis due to lagophthalmos, lid lag. upper lid asymmetry etc, External levator resection with aponeurotic approach was performed for 77 blepharoptosis patients (106 eyelids) with poor levator function from October 1987 through April 1996 at the Department of Ophthalmology, Yeungnam University, College of Medicine. Unilateral blepharoptosis were 44 eyelids and bilateral cases included 62 eyelids. Levator function was measured 4nim in 23 eyelids, Slum in 22 eyelids and 0-2iBin in 61 eyelids, After the follow up periods of 12-108 months (mean 36.3 months) , satisfactory resuite have been achieved in 44 eyelids (100%) of unilateral ptosis and in 30 eyelids (90.9%) of bilateral ptosis. All eyelids with 4mm levator function was successful, 22 eyelids (86.4%) with 3mm levator function were satisfactory and 61 eyelids (95. 1%) with 0-2mm levator function revealed satisfactory result. Postoperative complications included entropion (4 cases) , undercorection (2 cases) , corneal opacity (I case) and corneal ulcer (1 case). Based on this study, external levator resection could were one of the ideal techniques for severe ptosis with poor to absent. levator function.

8.Sonographic Evaluation of Nasal Bone Fractures.

PURPOSE: To describe the sonographic findings of nasal bone fracture and evaluate the diagnostic value of sonography (US). MATERIALS AND METHODS: Thirty-six patients with sustained nasal trauma underwent plain radiography, US and CT, and the CT findings were used as the gold standard. The sonographic findings were retrospectively reviewed and compared with those of radiography and CT. Twelve patients underwent follow-up US within 3 months of reduction. RESULTS: Sonographic findings of nasal bone fractures were irregularity (n=7), discontinuity (n=25) and dis-placement with discontinuity (n=28) of the thin smooth echogenic line formed by the ventral cortex of the nasal bone. Sixty-five nasal bone fractures (36 patients) were identified by CT; 60 of these were revealed on US but only 41 by plain radiographs. CT demonstrated unilateral involvement of nasal bone fractures in seven bones and bilateral involvement in 58, while US revealed unilateral involvement in 12 and bilateral involvement in 48. Plain radiographs, unilat-eral involvement in 21 bones and bilateral fractures in 20. Plain radiographs were more limited in detecting the location of nasal bone fractures than was US. On follow-up US in 12 patients, normal alignment without residual abnormality was observed in seven patients, residual cortical irregularities in four, and minimal cortical depression in one. CONCLUSION: The sonographic findings of nasal bone fractures were similar to those of CT in detecting the location and degree of depression or displacement of nasal bone fractures. We believe that US is very useful for the evaluation of such fractures, especially in cases in which plain radiographs reveal equivocal abnormality, or postreduction status is uncertain.

9.Solitary Dural Plasmacytoma: Case Report.

The authors present a case of solitary dural plasmacytoma. A 43-year-old woman was admitted our department due to headache. Brain magnetic resonance(MR) image showed homogeneous enhanced lesion at the left occipital area. We performed surgical excision and it was confirmed plasmacytoma, histologically. The solitary dural plasmacytomas are exceedingly rare. Before this case report, only 17 cases had been reported in the literature

Colorectal cancer consists of 8.2 percent of all malignancy in korea, and shows more increaing tendency. Despite the advances in the detection of calorectal cancer no single screening technique has achieved both high degree of diagnostic accuracy and minimal patient morbidity.
