1.Orbital Venography in Cavernous Hemangioma of the Orbit.

Cavernous hemangioma is the venous malformation that most frequently involves the orbit, And in orbital venography, the injection of contrast medium(conray) is made into a branch of frontal vein. The films are taken after delivery of 5 ml and immediately prior to the end of the injection. A preliminary control film is also obtained for this subtraction studies. Recently, authors experienced a case of orbital hemangioma which is caused unilateral proptosis of right eye(6 yr, old boy) and confirmed this patient as an orbital cavernous hemangioma by the technical aid of orbital venography under general anesthesia. Subtraction technique of R. orbital venograms following catheterization of one of the frontal veins revealed that 3rd segment of R. superior ophthalmic vein including 1st and 2nd segments is slightly displaced medially and superiorly of the frontal projection. They were somewhat dilated in caliber. Some dilated and tortuous vessels were filled with contrast media in the inferomedial aspect of the R. oribit from possible Srd segment of SOV. on the R. down lateral projection, there were abnormaly abundant dilated and tortuous vessels in the lateral portion of the orbit. It is fed from the R. angular vein, and CM are drained from the lesion to the dilated IOV at its posterior portion. R transfemoral arteriograms(AP, Lat) showed mild elevation of 3rd portion of the ophthalmic artery superiorly on the lateral projection, but it appeared normal caliber. The supraorbital and muscular branches as well as choroid crescent were not remarkable in this case. R. external carotid angiogram(LAT) showed within normal patterns. Angiographic diagnosis was a large venous malformation involving the lateral and posterior portions of the R. orbit, most likely cavernous hemangioma. Final diagnosis was confirmed by histopathological study of an excised hemangioma. After surgical removal the visual acuity was improved to normal without any disability of the eye.

2.Hemi-central Retinal Vein Occlusion in a Patient With Neurofibromatosis 1.

PURPOSE: We report on a case of hemi-central retinal vein occlusion (Hemi-CRVO) which occurred in a patient with Type 1 neurofibromatosis. CASE SUMMARY: A 31-year-old woman presented with a chief complaint of a one-month history of decreased visual acuity in the left eye. The patient had Type 1 neurofibromatosis but had no other systemic diseases. The best corrected visual acuity (BCVA) was 0.8 in the right eye and 0.05 in the left eye. Multiple Lisch nodules were present in the irises of both eyes on slit lamp examination. Fundus examination revealed a hemi-central retinal artery occlusion in the left eye. Optical coherence tomograms showed that the central macular thickness was 192 microns in the right eye and 732 microns in the left eye. There was cystoid macular edema in the left eye. A fluorescein angiogram showed delayed filling in the superior branch retinal vein of the left eye. There was an extensive area of non-perfusion of the retinal capillary in the corresponding area, including the perifoveal area. Blood tests for hyper-coagulability, autoimmune disorders, connective tissue disorders and other tests for the brain and carotid artery and coronary artery disease showed no abnormal findings. The clinical course was monitored following an intravitreal injection of bevacizumab (Avastin(R)). Eight months later, the BCVA had improved to 0.2. CONCLUSIONS: Although it is rare, hemi-CRVO occurring in younger patients who have no underlying diseases could be related to neurofibromatosis.

3.Clinical Results of Internal Fixation of Subcapital Femoral Neck Fractures.

BACKGROUND: Subcapital femoral neck is known to cause many complications, such as avascular necrosis (AVN) of the femoral head or nonunion, compared with other femoral neck fractures. The purpose of this study was to analyze the incidence of AVN and fixation failures in patients treated with internal fixation using cannulated screws for the subcapital femoral neck fractures. METHODS: This study targeted a total of 84 cases of subcapital femoral neck fractures that underwent internal fixation using cannulated screws. The average follow-up time after surgery was 36.8 months (range, 24 to 148 months). RESULTS: Nine hips (10.7%) showing AVN of the femoral head and 6 hips (7.1%) showing fixation failures were observed. The factors affecting the incidence of AVN of the femoral head after sustaining fractures correlated well with fracture types in the Garden classification (p = 0.030). The factors affecting fixation failure were the degree of reduction (p = 0.001) measured by the Garden alignment index and firm fixation (p = 0.009) assessed using the technique of 3-point fixation through the inferomedial cortical bone of the femoral neck. CONCLUSIONS: The complication rates for subcapital femoral neck fractures were lower than those previously reported; hence, internal fixation could be a primary treatment option for these fractures.

4.Effect of Serous Retinal Detachment on the Measurement of Axial Length in Central Serous Chorioretinopathy

PURPOSE: To evaluate the changes of axial length (AXL) in eyes with unilateral idiopathic central serous chorioretinopathy (CSC) after resolution of serous retinal detachment. METHODS: A total of 31 patients diagnosed with idiopathic unilateral CSC were included in this study. The changes of AXL according to serous retinal detachment were examined. The keratometric value and AXL were measured using partial coherence interferometry. Serous retinal detachment and central macular thickness (CMT) were measured by spectral domain optical coherence tomography. RESULTS: The mean age of the 31 CSC patients, including 19 males, was 42.7 years. The AXL was significantly increased from 23.41 to 23.58 mm after resolution of serous retinal detachment (p < 0.001). The CMT was significantly decreased from 413.4 to 226.8 µm after resolution of serous retinal detachment (p < 0.001). The differences in AXL correlated with CMT differences and subretinal fluid height (r = −0.616, p < 0.001 and r = −0.637, p < 0.001, respectively), and the best-corrected visual acuity was significantly different after resolution of serous retinal detachment (p < 0.001). CONCLUSIONS: In unilateral idiopathic CSC with serous retinal detachment, a shortened AXL in the acute phase was restored after resolution of serous retinal detachment.

5.Inhibitory Effect of IFN-beta, on the Antitumor Activity of Celecoxib in U87 Glioma Model.

OBJECTIVE: Interferon-beta, (IFN-beta) has been used in the treatment of cancers. Inhibition of the enzyme cyclooxygenase (COX) with celecoxib had a significantly suppressive effect on tumor growth, angiogenesis, and metastasis in a variety of tumors. The aim of this study was to elucidate the antiglioma effect of combined treatment with IFN-beta and celecoxib in U87 glioma model. METHODS: The in vitro effects of IFN-beta (50-1,000 IU/mL) and celecoxib (50-250 micrometer) alone or combination of both on the proliferation and apoptosis of U87 cells were tested using MTT assay, FACS analysis and DNA condensation. To determine the in vivo effect, nude mice bearing intracerebral U87 xenograft inoculation were treated with IFN-beta intraperitoneally (2x10(5) IU/day for 15 days), celecoxib orally (5, 10 mg/kg) or their combination. RESULTS: IFN-beta or celecoxib showed an inhibitory effect on the proliferation of U87 cells. When U87 cells were treated with IFN-beta and celecoxib combination, it seemed that IFN-beta interrupted the antiproliferative and apoptotic activity of celecoxib. No additive effect was observed on the survival of the tumor bearing mice by the combination of IFN-beta and celecoxib. CONCLUSION: These results suggest that IFN-beta seems to inhibit the antiglioma effect of celecoxib, therefore combination of IFN-beta and celecoxib may be undesirable in the treatment of glioma.

6.A Case of Unusual (1q;21q) Translocation Down Syndrome Inherited from a t (1q;21q) Balanced Carrier Mother.

Down syndrome is the most common autosomal syndrome in man. The incidence of trisomy 21 due to translocation is about 3.5-5%. Translocations are usually centric fusions between a 21 and a D group (54.2%) or a G group (40.9%) chromosome. Since the short arm of 21 carries no phenotypically active genes, even if the short arm of 21 is lost, usually there is no imbalance with clinical significance. Rare translocations in Down syndrome may also occur between a 21 and other chromosomes (4.9%). We experienced a case of unusual (1q;21q) translocation Down syndrome inherited from a t (1q;21q) balanced carrier mother. The segmental trisomy was confirmed by FISH analysis. This is the first case of a familial Down syndrome with t (1q;21q) in Korea.

7.Clinical features of atypical femur fracture.

OBJECTIVES: We aim to elucidate the clinical outcomes of bisphosphonate-associated atypical femoral fracture and the clinical results depending on the bisphosphonate therapy period. METHODS: Twenty cases involving 15 patients who had been diagnosed with atypical femoral facture between 2004 and 2014 and who had been followed up for at least 12 months were retrospectively analyzed. The control group was composed of 15 typical femoral facture patients. We used plain radiography and physical examinations to determine the period of time required for fracture healing as well as complication occurrence. We investigated the bisphosphonate administration status and duration and the names of its components, bilateral fracture occurrence status, the period of time required for bone union, and reoperation or bone graft status due to nonunion. RESULTS: Revision surgery involving a bone graft was performed due to nonunion in 1 out of 15 cases. Except in one revision case, the duration of the union was 11.9 months on average in 14 cases of atypical fracture patients, and 4.3 months on average in the control group. This difference was statistically significant (p < 0.05). The bisphosphonate administration duration was positively correlated with the union period (p < 0.05). In contrast, there was no statistically significant correlation between the bisphosphonate administration duration and the incidence of bilateral atypical fractures (p > 0.05). CONCLUSIONS: Atypical femoral fractures required more time for bone union than typical ones and prolonged bisphosphonate administration led to a longer period of time required for bone union.

8.A Preliminary Study of Predictive Factors for Language and Cognitive Development after Cochlear Implant in Congenital Deaf Children.

BACKGROUND AND OBJECTIVES: The cognitive function of the cochlear implant (CI) candidates and psychological factors of their family may have influence on the rehabilitation after CI. However, only a few studies have been reported. The aim of the study was to examine the cognitive and language abilities of deaf children before CI. We also assessed educational opportunities that deaf children have been given, emotional problems of their parents, and investigated the relationship of these variables to the cognitive function of the deaf children. SUBJECTS AND METHOD: Sixteen deaf children from 5 to 7 years old and 100 parents having a deaf child participated in the study. The psychological test battery was administered to the deaf children to assess intelligence, attention and motor coordination. Questionnaires were given to the parents to get the medical and educational history of their child, the familial characteristics, parental attitude and emotional problems. RESULTS: Congenital deaf children showed high social adaptation abilities on Social Maturity Scale and borderline level intelligence in non-linguistic intelligence test. However, they showed low scores on visual attention task and significantly decreased visual-motor coordination on complex tasks. Seventeen parents (18.5%) met the criteria of depressive disorder by Beck Depressive Inventory. The parental stress and depressive mood were negatively correlated with the language and cognitive development of their child. CONCLUSION: The result of this study suggests that preoperative cognitive development of deaf children and emotional problem of their parents may affect on the development of language and cognitive abilities during the rehabilitation period after cochlear implantation.

9.Histologic Change of Liver of Normal Rat After Injection of Ethanol, Hot Normal Saline and Hypertonic Saline.

PURPOSE: The purpose of this study was to evaluate the degree of liver injury after injection of ethanol, hot normal saline and hot hypertonic saline into normal rat liver. MATERIALS AND METHODS: Sixty white rats weighing 200-300g were used. There were three groups (ethanol, hot normal saline, and hot hypertonic saline), and these were divided into subgroups (5 rats in each) according to amount of injected material and duration. Under either anesthesia, each drug (ethanol -0.05ml, 0.1ml, 0.2ml ; hot normal saline -0.1ml, 0.2ml, 0.4ml ; hot hypertonic saline -0.1ml, 0.2ml, 0.4ml ) was introduced directly into the liver. Pathologic specimens were obtained 7 and 14 days after injection. RESULTS: With regard to histologic change after 7 days, the ethanol group showed less tissue damage such as coagulation necrosis and inflammatory infiltration-than the hot saline group. There was however, no significant difference among the three groups in tissue damage 14 days. Nor was there any significant histologic difference between the different saline concentration groups. CONCLUSION: Histologically, there was no significant difference among the three groups in tissue damage at 14 days, and it is suggested that this is due to irreversible change in damaged tissue. Tissue damage was caused mainly by the high temperature of saline rather than by high osmolarity.

10.Sudden rapid growth of an aggressive angiomyxoma after taking pomegranate seeds oil.

Aggressive angiomyxoma (AA) is a rare soft tissue tumor of undetermined etiology that occurs mainly in the female vulva and perineum. It has been demonstrated to increase in size under the estrogenic stimulation and shown to respond to treatment with a GnRH agonist. A 47-year-old woman was presented with a large vulvar mass. The mass had grown slowly before she took pomegranate seeds oil on a regular basis for a year, which is known to contain natural estrogen for a year. The almond-sized mass enlarged into a fetal head-sized one in a year. Our case is implying that external administration of estrogenic substance may be a contributory factor to the sudden accelerated growth of AA. Although the past medical history in this case relies on an unverified observation by the patient alone, the unique clinical course and impressive gross appearance of AA may help clinicians with diagnosis in practice.
